Coronary CT Angiography–derived Fractional Flow Reserve: Machine Learning Algorithm versus Computational Fluid Dynamics Modeling

Abstract
The results demonstrate that coronary CT angiography–derived fractional flow reserve using machine learning performs equally to computational fluid dynamics modeling in detecting lesion-specific ischemia; both algorithms outperform coronary CT angiography alone and quantitative coronary angiography.

TL;DR: Investigation of the influence of coronary artery calcium (CAC) score on the diagnostic performance of machine-learning-based coronary computed tomography (CT) angiography (cCTA)-derived fractional flow reserve (CT-FFR) showed superior diagnostic performance over cCTA alone in CAC with a significant difference in the performance of CT- FFR as calcium burden/Agatston calcium score increased.

TL;DR: Although the study did not achieve its prespecified primary outcome goal for the level of per-patient diagnostic accuracy, use of noninvasive FFR(CT) plus CT among stable patients with suspected or known CAD was associated with improved diagnostic accuracy and discrimination vs CT alone for the diagnosis of hemodynamically significant coronary stenosis.
