Abstract: PROBLEM/CONDITION Higher rates of death in nonmetropolitan areas (often referred to as rural areas) compared with metropolitan areas have been described but not systematically assessed. PERIOD COVERED 1999-2014 DESCRIPTION OF SYSTEM: Mortality data for U.S. residents from the National Vital Statistics System were used to calculate age-adjusted death rates and potentially excess deaths for nonmetropolitan and metropolitan areas for the five leading causes of death. Age-adjusted death rates included all ages and were adjusted to the 2000 U.S. standard population by the direct method. Potentially excess deaths are defined as deaths among persons aged <80 years that exceed the numbers that would be expected if the death rates of states with the lowest rates (i.e., benchmark states) occurred across all states. (Benchmark states were the three states with the lowest rates for each cause during 2008-2010.) Potentially excess deaths were calculated separately for nonmetropolitan and metropolitan areas. Data are presented for the United States and the 10 U.S. Department of Health and Human Services public health regions. RESULTS Across the United States, nonmetropolitan areas experienced higher age-adjusted death rates than metropolitan areas. The percentages of potentially excess deaths among persons aged <80 years from the five leading causes were higher in nonmetropolitan areas than in metropolitan areas. For example, approximately half of deaths from unintentional injury and chronic lower respiratory disease in nonmetropolitan areas were potentially excess deaths, compared with 39.2% and 30.9%, respectively, in metropolitan areas. Potentially excess deaths also differed among and within public health regions; within regions, nonmetropolitan areas tended to have higher percentages of potentially excess deaths than metropolitan areas. INTERPRETATION Compared with metropolitan areas, nonmetropolitan areas have higher age-adjusted death rates and greater percentages of potentially excess deaths from the five leading causes of death, nationally and across public health regions. PUBLIC HEALTH ACTION Routine tracking of potentially excess deaths in nonmetropolitan areas might help public health departments identify emerging health problems, monitor known problems, and focus interventions to reduce preventable deaths in these areas.

Abstract: Persons living in rural areas are recognized as a health disparity population because the prevalence of disease and rate of premature death are higher than for the overall population of the United States. Surveillance data about health-related behaviors are rarely reported by urban-rural status, which makes comparisons difficult among persons living in metropolitan and nonmetropolitan counties.

Abstract: PROBLEM/CONDITION Drug overdoses are a leading cause of injury death in the United States, resulting in approximately 52,000 deaths in 2015. Understanding differences in illicit drug use, illicit drug use disorders, and overall drug overdose deaths in metropolitan and nonmetropolitan areas is important for informing public health programs, interventions, and policies. REPORTING PERIOD Illicit drug use and drug use disorders during 2003-2014, and drug overdose deaths during 1999-2015. DESCRIPTION OF DATA The National Survey of Drug Use and Health (NSDUH) collects information through face-to-face household interviews about the use of illicit drugs, alcohol, and tobacco among the U.S. noninstitutionalized civilian population aged ≥12 years. Respondents include residents of households and noninstitutional group quarters (e.g., shelters, rooming houses, dormitories, migratory workers' camps, and halfway houses) and civilians living on military bases. NSDUH variables include sex, age, race/ethnicity, residence (metropolitan/nonmetropolitan), annual household income, self-reported drug use, and drug use disorders. National Vital Statistics System Mortality (NVSS-M) data for U.S. residents include information from death certificates filed in the 50 states and the District of Columbia. Cases were selected with an underlying cause of death based on the ICD-10 codes for drug overdoses (X40-X44, X60-X64, X85, and Y10-Y14). NVSS-M variables include decedent characteristics (sex, age, and race/ethnicity) and information on intent (unintentional, suicide, homicide, or undetermined), location of death (medical facility, in a home, or other [including nursing homes, hospices, unknown, and other locations]) and county of residence (metropolitan/nonmetropolitan). Metropolitan/nonmetropolitan status is assigned independently in each data system. NSDUH uses a three-category system: Core Based Statistical Area (CBSA) of ≥1 million persons; CBSA of <1 million persons; and not a CBSA, which for simplicity were labeled large metropolitan, small metropolitan, and nonmetropolitan. Deaths from NVSS-M are categorized by the county of residence of the decedent using CDC's National Center for Health Statistics 2013 Urban-Rural Classification Scheme, collapsed into two categories (metropolitan and nonmetropolitan). RESULTS Although both metropolitan and nonmetropolitan areas experienced significant increases from 2003-2005 to 2012-2014 in self-reported past-month use of illicit drugs, the prevalence was highest for the large metropolitan areas compared with small metropolitan or nonmetropolitan areas throughout the study period. Notably, past-month use of illicit drugs declined over the study period for the youngest respondents (aged 12-17 years). The prevalence of past-year illicit drug use disorders among persons using illicit drugs in the past year varied by metropolitan/nonmetropolitan status and changed over time. Across both metropolitan and nonmetropolitan areas, the prevalence of past-year illicit drug use disorders declined during 2003-2014. In 2015, approximately six times as many drug overdose deaths occurred in metropolitan areas than occurred in nonmetropolitan areas (metropolitan: 45,059; nonmetropolitan: 7,345). Drug overdose death rates (per 100,000 population) for metropolitan areas were higher than in nonmetropolitan areas in 1999 (6.4 versus 4.0), however, the rates converged in 2004, and by 2015, the nonmetropolitan rate (17.0) was slightly higher than the metropolitan rate (16.2). INTERPRETATION Drug use and subsequent overdoses continue to be a critical and complicated public health challenge across metropolitan/nonmetropolitan areas. The decline in illicit drug use by youth and the lower prevalence of illicit drug use disorders in rural areas during 2012-2014 are encouraging signs. However, the increasing rate of drug overdose deaths in rural areas, which surpassed rates in urban areas, is cause for concern. PUBLIC HEALTH ACTIONS Understanding the differences between metropolitan and nonmetropolitan areas in drug use, drug use disorders, and drug overdose deaths can help public health professionals to identify, monitor, and prioritize responses. Consideration of where persons live and where they die from overdose could enhance specific overdose prevention interventions, such as training on naloxone administration or rescue breathing. Educating prescribers on CDC's guideline for prescribing opioids for chronic pain (Dowell D, Haegerich TM, Chou R. CDC guideline for prescribing opioids for chronic pain-United States, 2016. MMWR Recomm Rep 2016;66[No. RR-1]) and facilitating better access to medication-assisted treatment with methadone, buprenorphine, or naltrexone could benefit communities with high opioid use disorder rates.

Abstract: This article documents the diversity of political attitudes and voting patterns along the urban-rural continuum of the United States. We find that America’s rural and urban interface, in terms of political attitudes and voting patterns, is just beyond the outer edges of large urban areas and through the suburban counties of smaller metropolitan areas. Both Barack Obama and Hillary Clinton performed well in densely populated areas on the urban side of the interface, but they faced increasingly difficult political climates and sharply diminished voter support on the rural side of the interface. The reduction in support for Clinton in 2016 in rural areas was particularly pronounced. Even after controlling for demographic, social, and economic factors (including geographic region, education, income, age, race, and religious affiliation) in a spatial regression, we find that a county’s position in the urban-rural continuum remained statistically significant in the estimation of voting patterns in presidential ...

Abstract: Urban development in southern Europe differs from urbanization patterns observed in other affluent countries. Urbanization processes in the Mediterranean region reflect heterogeneous spatial forms and more similar socioeconomic dynamics. Rejecting a unique ‘Mediterranean city’ model, this study proposes a thorough analysis of post-war urban development in four southern European countries (Greece, Italy, Portugal and Spain) focusing on homogeneous patterns and possible sources of heterogeneity in spatio-temporal trends of individual city expansion. Urbanization without industrialization - boosted by the informal economy and the development of traditional services - has driven the growth of large cities through agglomeration economies enhanced by internal immigration. This trend has deflated shortly after the end of the baby boom, favoring slow dynamics towards spatially-balanced settlements and determining a recovery of medium-sized cities, which have been further consolidated with the economic crisis.

Abstract: The main aim of this paper is to analyze the road accidents in India at national, state, and metropolitan city level. Analysis shows that the distribution of road accidental deaths and injuries in India varies according to age, gender, month and time. Age group 30-59 years is the most vulnerable population group, though males face higher level of fatalities and injuries than their female counterparts. Moreover, road accidents are relatively higher in extreme weather and during working hours. Analysis of road accident scenario at state and city level shows that there is a huge variation in fatality risk across states and cities. Fatality risk in 16 out of 35 states and union territories is higher than the all India average. Although, burden of road accidents in India is marginally lower in its metropolitan cities, almost 50% of the cities face higher fatality risk than their moffusil counterparts. In general, while in many developed and developing countries including China, road safety situation is generally improving, India faces a worsening situation. Without increased efforts and new initiatives, the total number of road traffic deaths in India is likely to cross the mark of 250,000 by the year 2025. There is thus an urgent need to recognize the worsening situation in road deaths and injuries and to take appropriate action.

Abstract: Urban spatial structure affects many aspects of urban functions and has implications for accessibility, environmental sustainability, and public expenditures. During the urbanization process, a careful and efficient examination of the urban spatial structure is crucial. Different from the traditional approach that relies on population or employment census data, this research exploits the nighttime light (NTL) intensity of the earth surface recorded by satellite sensors. The NTL intensity is represented as a continuous mathematical surface of human activities, and the elemental features of urban structures are identified by analogy with earth’s topography. We use a topographical metaphor of a mount to identify an urban center or subcenter and the surface slope to indicate an urban land-use intensity gradient. An urban center can be defined as a continuous area with higher concentration or density of employments and human activities. We successfully identified 33 urban centers, delimited their corresponding boundaries, and determined their spatial relations for Shanghai metropolitan area, by developing a localized contour tree method. In addition, several useful properties of the urban centers have been derived, such as 9% of Shanghai administrative area has become urban centers. We believe that this method is applicable to other metropolitan regions at different spatial scales.

Abstract: Swimming pools are together an exemplificative outcome of urban sprawl and an indicator of socio-spatial polarizations in metropolitan regions. A comparative analysis of the spatial distribution of pools in three Mediterranean cities (Barcelona, Rome and Athens) provides an alternative reading of recent urbanization in southern Europe, questioning the supposed homogeneity in socioeconomic patterns and processes across the region. In the present study, the socio-spatial structure underlying dispersed urban expansion in these three cities was studied using 53 background indicators at the spatial scale of municipalities. Four indicators were proposed to study variability in the spatial distribution of pools. Abstract: Drawing attention to the governing role of capital accumulation and its interaction with the state, this study examines the dynamics of the new wave of suburbanization in China, which is characterized by the development of new towns. New towns essentially function as a spatial fix in China’s contemporary accumulation regime. Rather than resulting from capital switching from the primary to the secondary circuits, new towns help to collect funds for the leverage of industrial capital and thus simultaneously sustain both circuits. Meanwhile, the development of new towns is also a process of territorial development, in which municipal governments expand the space of accumulation under strengthened fiscal and land controls and develop a metropolitan structure. Underlying the specific form and dynamics, however, is the worldwide trend of capital switching from declining manufacturing industries in developed countries to the new investment frontier in developing countries.

Abstract: One of the major impacts associated with unplanned rapid urban growth is the decrease of urban vegetation, which is often replaced with impervious surfaces such as buildings, parking lots, roads, and pavements. Consequently, as the percentage of impervious surfaces continues to increase at the expense of vegetation cover, surface urban heat island (SUHI) forms and becomes more intense. The Colombo Metropolitan Area (CMA), Sri Lanka, is one of the rapidly urbanizing metropolitan regions in South Asia. In this study, we examined the spatiotemporal variations of land surface temperature (LST) in the CMA in the context of the SUHI phenomenon using Landsat data. More specifically, we examined the relationship of LST with the normalized difference vegetation index (NDVI) and the normalized difference built-up index (NDBI) at three time points (1997, 2007 and 2017). In addition, we also identified environmentally critical areas based on LST and NDVI. We found significant correlations of LST with NDVI (negative) and NDBI (positive) (p < 0.001) across all three time points. Most of the environmentally critical areas are located in the central business district (CBD), near the harbor, across the coastal belt, and along the main transportation network. We recommend that those identified environmentally critical areas be considered in the future urban planning and landscape development of the city. Green spaces can help improve the environmental sustainability of the CMA.

Abstract: Cities are economically open systems that depend on goods and services imported from national and global markets to satisfy their material and energy requirements. Greenhouse Gas (GHG) footprints are thus a highly relevant metric for urban climate change mitigation since they not only include direct emissions from urban consumption activities, but also upstream emissions, i.e. emissions that occur along the global production chain of the goods and services purchased by local consumers. This complementary approach to territorially-focused emission accounting has added critical nuance to the debate on climate change mitigation by highlighting the responsibility of consumers in a globalized economy. Yet, city officials are largely either unaware of their upstream emissions or doubtful about their ability to count and control them. This study provides the first internationally comparable GHG footprints for four cities (Berlin, Delhi NCT, Mexico City, and New York metropolitan area) applying a consistent method that can be extended to other global cities using available data. We show that upstream emissions from urban household consumption are in the same order of magnitude as cities' overall territorial emissions and that local policy leverage to reduce upstream emissions is larger than typically assumed.

Abstract: This paper estimates the productivity gains from agglomeration economies for a sample of the largest metropolitan areas in the United States using measures of urban agglomeration based on employment density and employment accessibility. The latter is a more accurate measure of economic proximity and allows testing for the spatial decay of agglomeration effects with increasing travel time. We find that the productivity gains from urban agglomeration are consistent between measures, with elasticity values between 0.07 and 0.10. The large majority of the productivity gains occur within the first 20 minutes, and do not appear to exhibit significant nonlinearities.

Abstract: Problem/condition The places in which persons live, work, and play can contribute to the development of adverse health outcomes. Understanding the differences in risk factors in various environments can help to explain differences in the occurrence of these outcomes and can be used to develop public health programs, interventions, and policies. Efforts to characterize urban and rural differences have largely focused on social and demographic characteristics. A paucity of national standardized environmental data has hindered efforts to characterize differences in the physical aspects of urban and rural areas, such as air and water quality. Reporting period 2008-2012 for air quality and 2010-2015 for water quality. Description of system Since 2002, CDC's National Environmental Public Health Tracking Program has collaborated with federal, state, and local partners to gather standardized environmental data by creating national data standards, collecting available data, and disseminating data to be used in developing public health actions. The National Environmental Public Health Tracking Network (i.e., the tracking network) collects data provided by national, state, and local partners and includes 21 health outcomes, exposures, and environmental hazards. To assess environmental factors that affect health, CDC analyzed three air-quality measures from the tracking network for all counties in the contiguous United States during 2008-2012 and one water-quality measure for 26 states during 2010-2015. The three air-quality measures include 1) total number of days with fine particulate matter (PM2.5) levels greater than the U.S. Environmental Protection Agency's (EPA's) National Ambient Air Quality Standards (NAAQS) for 24-hour average PM2.5 (PM2.5 days); 2) mean annual average ambient concentrations of PM2.5 in micrograms per cubic meter (mean PM2.5); and 3) total number of days with maximum 8-hour average ozone concentrations greater than the NAAQS (ozone days). The water-quality measure compared the annual mean concentration for a community water system (CWS) to the maximum contaminant level (MCL) defined by EPA for 10 contaminants: arsenic, atrazine, di(2-ethylhexyl) phthalate (DEHP), haloacetic acids (HAA5), nitrate, perchloroethene (PCE), radium, trichloroethene (TCE), total trihalomethanes (TTHM), and uranium. Findings are presented by urban-rural classification scheme: four metropolitan (large central metropolitan, large fringe metropolitan, medium metropolitan, and small metropolitan) and two nonmetropolitan (micropolitan and noncore) categories. Regression modeling was used to determine whether differences in the measures by urban-rural categories were statistically significant. Results Patterns for all three air-quality measures suggest that air quality improves as areas become more rural (or less urban). The mean total number of ozone days decreased from 47.54 days in large central metropolitan counties to 3.81 days in noncore counties, whereas the mean total number of PM2.5 days decreased from 11.21 in large central metropolitan counties to 0.95 in noncore counties. The mean average annual PM2.5 concentration decreased from 11.15 μg/m3 in large central metropolitan counties to 8.87 μg/m3 in noncore counties. Patterns for the water-quality measure suggest that water quality improves as areas become more urban (or less rural). Overall, 7% of CWSs reported at least one annual mean concentration greater than the MCL for all 10 contaminants combined. The percentage increased from 5.4% in large central metropolitan counties to 10% in noncore counties, a difference that was significant, adjusting for U.S. region, CWS size, water source, and potential spatial correlation. Similar results were found for two disinfection by-products, HAA5 and TTHM. Arsenic was the only other contaminant with a significant result. Medium metropolitan counties had 3.1% of CWSs reporting at least one annual mean greater than the MCL, compared with 2.4% in large central counties. Interpretation Noncore (rural) counties experienced fewer unhealthy air-quality days than large central metropolitan counties, likely because of fewer air pollution sources in the noncore counties. All categories of counties had a mean annual average PM2.5 concentration lower than the EPA standard. Among all CWSs analyzed, the number reporting one or more annual mean contaminant concentrations greater the MCL was small. The water-quality measure suggests that water quality worsens as counties become more rural, in regards to all contaminants combined and for the two disinfection by-products individually. Although significant differences were found for the water-quality measure, the odds ratios were very small, making it difficult to determine whether these differences have a meaningful effect on public health. These differences might be a result of variations in water treatment practices in rural versus urban counties. Public health action Understanding the differences between rural and urban areas in air and water quality can help public health departments to identify, monitor, and prioritize potential environmental public health concerns and opportunities for action. These findings suggest a continued need to develop more geographically targeted, evidence-based interventions to prevent morbidity and mortality associated with poor air and water quality.

Abstract: Growth of urban areas and abandonment of rural areas are phenomena that increase quickly. The main consequences of urbanization are pollution, consumption of resources and energy, waste dumps, and junk yards. These aspects require a better planning and design of European urban metropolitan areas, considering benefits, opportunities, costs and risks (B.O.C.R.), derivable by urban transformations and available resources. The paper consists of five parts. The first part contains some reflections on consequences of urban sprawl. In the second part, some possible kinds of cities are discussed (sustainable city, smart city, and compact city). The third part briefly describes a multicriteria decision-making approach known as the ‘analytic hierarchy process’ to deal with complex decisions. In the fourth part, alternative city models are analyzed (compact city, elevated city, green house city, and water city). Finally, in the fifth part, the criteria selected for the planning and design of the alternative city models are used for the prioritization of some European cities.

Abstract: Purpose Entrepreneurial ecosystems are receiving growing attention from scholars, practitioners and policy-makers in both developed and developing countries. Studies of this phenomenon have focused almost exclusively on ecosystems in large, urbanized regions and metropolitan areas, located primarily in developed economies. However, the prevalence of small cities across the globe and the increasing acknowledgment that entrepreneurship in small towns is a key determinant of their economic development and rejuvenation suggests that entrepreneurial ecosystems research would benefit from a broader lens of inquiry. Thus, the purpose of this paper is to introduce a framework for studying entrepreneurial ecosystems in small towns. Design/methodology/approach This conceptual paper introduces the concept of small town entrepreneurial ecosystems (STEEs), draws from a wide-ranging set of disciplines to delineate the ways in which small town ecosystems are similar to and different than their larger counterparts and theorizes about several strategies STEEs use to overcome their limitations. Findings It is theorized that entrepreneurship in small cities is best conceptualized as the outcome of an ecosystem, which means that although small towns may not have some of the same key components as entrepreneurial ecosystems in large urban centers, other elements of the ecosystem may be able to bolster these deficiencies. It also suggests that those attempting to create or develop small town ecosystems may need to be entrepreneurial in the way they attract, view and utilize resources. Finally, it is theorized that small cities may be able to engage in several strategies to overcome their limitations and create vibrant entrepreneurial communities. Originality/value The theory developed produces implications for scholars focused on entrepreneurial ecosystems, economic development and emerging economies and suggests practical implications for policy-makers and development organizations seeking to improve the economic landscape of small cities.

Abstract: The world is rapidly urbanising, with both positive and negative consequences. One major challenge is how to secure the long-term quality of life for urban residents. Many studies on quality of life are based on ‘material’ ecosystem services (i.e., provisioning and regulating services), with less attention paid to the non-material benefits gained from nature (e.g., green and blue spaces), which have been called cultural ecosystem services (CES). However, these non-material services are often most important to urban residents. Understanding perceptions and awareness of CES provided by urban blue space (i.e., water and wetlands) and green space (i.e., urban forest and grassland) are therefore important to support planning, creation, and protection of these spaces. To tackle these problems, we assessed and quantified the CES provided by urban green and blue space in six metropolitan areas of Beijing. By combining ecosystem services valuation with surveys (466 questionnaires and 16 expert interviews), we gained insights into residents’ perceptions of CES. Surprisingly, blue areas in metropolitan Beijing were valued at least 4.3 times higher than the value of green areas. More than 80% of the residents were willing to pay for maintenance of urban blue and green areas (an average of almost 64 RMB/year). The contributions of cultural services are likely to increase if stakeholders value natural ecosystems more in urban areas than in rural or mountainous areas because of the additional value of these rare resources in densely populated areas.

Abstract: The debate on urban resilience and metabolism has directed increasing attention to the ecological footprint of food consumption, self-sufficiency as a means of food security, and regionalisation of food systems for shortening supply chains. Recently, metropolitan regions have proposed food policies that aim to foster local food systems connected to their cities. Our research thus focused on the relationship between urban food demand and metropolitan land use. We have developed the Metropolitan Foodshed and Self-sufficiency Scenario (MFSS) model, which combines regional food consumption and agricultural production parameters in a data-driven approach to assess the spatial extent of foodsheds as well as the theoretical self-sufficiency of the communities they serve. The model differentiates between food groups, food production systems, levels of food loss and waste as well as food origin. With regard to future urban growth, we applied the model to current and future population projections. Results show substantial variations in the spatial extent of metropolitan foodsheds and self-sufficiency levels between the case study regions London, Berlin, Milan and Rotterdam, depending on population density and distribution, geographical factors and proximity to neighbouring urban agglomerations. The application of the model as a food planning tool offers a new perspective on the potential role of metropolitan regions for strengthening urban self-sufficiency. It also enables the ex-ante assessment of spatial consequences of changes within metropolitan food systems, on both demand and supply sides. In particular, we discuss possible dietary and consumption changes, but also production and supply chain alternatives.
