Chronic obstructive pulmonary disease (COPD) remains a major public health problem. It is the fourth leading cause of chronic morbidity and mortality in the United States, and is projected to rank fifth in 2020 in burden of disease worldwide, according to a study published by the World Bank/World Health Organization. Yet, COPD remains relatively unknown or ignored by the public as well as public health and government officials. In 1998, in an effort to bring more attention to COPD, its management, and its prevention, a committed group of scientists encouraged the U.S. National Heart, Lung, and Blood Institute and the World Health Organization to form the Global Initiative for Chronic Obstructive Lung Disease (GOLD). Among the important objectives of GOLD are to increase awareness of COPD and to help the millions of people who suffer from this disease and die prematurely of it or its complications. The first step in the GOLD program was to prepare a consensus report, Global Strategy for the Diagnosis, Management, and Prevention of COPD, published in 2001. The present, newly revised document follows the same format as the original consensus report, but has been updated to reflect the many publications on COPD that have appeared. GOLD national leaders, a network of international experts, have initiated investigations of the causes and prevalence of COPD in their countries, and developed innovative approaches for the dissemination and implementation of COPD management guidelines. We appreciate the enormous amount of work the GOLD national leaders have done on behalf of their patients with COPD. Despite the achievements in the 5 years since the GOLD report was originally published, considerable additional work is ahead of us if we are to control this major public health problem. The GOLD initiative will continue to bring COPD to the attention of governments, public health officials, health care workers, and the general public, but a concerted effort by all involved in health care will be necessary.

Global strategy for the diagnosis, management, and prevention of chronic obstructive pulmonary disease: GOLD executive summary.

The National High Blood Pressure Education Program presents the complete Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Like its predecessors, the purpose is to provide an evidence-based approach to the prevention and management of hypertension. The key messages of this report are these: in those older than age 50, systolic blood pressure (BP) of greater than 140 mm Hg is a more important cardiovascular disease (CVD) risk factor than diastolic BP; beginning at 115/75 mm Hg, CVD risk doubles for each increment of 20/10 mm Hg; those who are normotensive at 55 years of age will have a 90% lifetime risk of developing hypertension; prehypertensive individuals (systolic BP 120-139 mm Hg or diastolic BP 80-89 mm Hg) require health-promoting lifestyle modifications to prevent the progressive rise in blood pressure and CVD; for uncomplicated hypertension, thiazide diuretic should be used in drug treatment for most, either alone or combined with drugs from other classes; this report delineates specific high-risk conditions that are compelling indications for the use of other antihypertensive drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers, beta-blockers, calcium channel blockers); two or more antihypertensive medications will be required to achieve goal BP (<140/90 mm Hg, or <130/80 mm Hg) for patients with diabetes and chronic kidney disease; for patients whose BP is more than 20 mm Hg above the systolic BP goal or more than 10 mm Hg above the diastolic BP goal, initiation of therapy using two agents, one of which usually will be a thiazide diuretic, should be considered; regardless of therapy or care, hypertension will be controlled only if patients are motivated to stay on their treatment plan. Positive experiences, trust in the clinician, and empathy improve patient motivation and satisfaction. This report serves as a guide, and the committee continues to recognize that the responsible physician's judgment remains paramount.

The rapid technological developments of the past decade and the changes in echocardiographic practice brought about by these developments have resulted in the need for updated recommendations to the previously published guidelines for cardiac chamber quantification, which was the goal of the joint writing group assembled by the American Society of Echocardiography and the European Association of Cardiovascular Imaging. This document provides updated normal values for all four cardiac chambers, including three-dimensional echocardiography and myocardial deformation, when possible, on the basis of considerably larger numbers of normal subjects, compiled from multiple databases. In addition, this document attempts to eliminate several minor discrepancies that existed between previously published guidelines.

Recommendations for cardiac chamber quantification by echocardiography in adults: an update from the American Society of Echocardiography and the European Association of Cardiovascular Imaging.

Diabetes mellitus and hypertension are commonly associated conditions. The prevalence of hypertension in type 2 diabetes is higher than in the general population. At the age of 45 around 40% of patients with type 2 diabetes are hypertensive, the proportion rising to 60% by the age of 75. Hypertension potentiates the already high risk of cardiovascular complications associated with diabetes mellitus. In 1997, the Sixth Report of the Joint National Committee on Prevention, Detection, Evaluation and Treatment of High Blood Pressure recommended the use of angiotensin-converting enzyme inhibitors, alpha-blockers, calcium-channel blockers or low-dose diuretics to initiate antihypertensive drug treatment in hypertensive diabetic patients. In patients with diabetic nephropathy, angiotensin-converting enzyme inhibitors were proposed as the first-line treatment. At variance with these guidelines, two studies published in 1998 raised major concerns about the use of dihydropyridines in diabetic patients. Subsequent papers published in 1998 and 1999, based on larger numbers of patients, demonstrated that longacting calcium-channel blockers are not only safe to use in diabetic patients, but that they may even confer particular benefit to this important subgroup of the hypertensive population. These recent studies are shortly reviewed (Table 1) and put into perspective.

Clustering of metabolic risk factors in young adults: genes and environment.

Blood pressure increases with age in most people. Being more marked for systolic pressure, this increase leads to an increase in pressure amplitude. This is illustrated by Figure 1 which shows the systolic and diastolic blood pressures of a representative sample of a Belgian village [1]. There is no certainty, however, that this phenomenon continues at an extremely advanced age. Cross-sectional studies are contradictory in this respect. In the 1753 subjects over 60 years old in Edwards’ study [2] the systolic pressure shows a quasilinear rise up to the age of 70 and then starts to decrease, while diastolic blood pressure remains stable after the age of 60. In Masters’ study as well, systolic and diastolic blood pressure in 5757 subjects over 65 years old decreased in women and became stabilized in men after the age of 75 years [3].

Hypertension in Elderly

A meta-analysis is presented of 8 therapeutic trials in elderly hypertensive patients. In an intention-to-treat analysis, cardiovascular mortality was decreased on average by 22% (95% confidence interval ranging from -32% to -10%). This decrease was explained by both a reduction in coronary mortality by 26% (-40% to -9%) and in cerebrovascular mortality by 33% (-50% to -9%). The effectiveness of therapy in terms of reducing cardiovascular mortality is not established with confidence in those trials where the diastolic blood pressure at randomization is below 95 mmHg or in the patients above 75 years of age. A goal blood pressure is not definitively established, but a reduction of the systolic blood pressure to about 150 mmHg may be optimal. Extrapolation of the trial results to the elderly population with systolo-diastolic hypertension at large seems acceptable for the western population, but may be premature for the Asian and African elderly. Beta-blockers and especially diuretics are recommended as first-line drugs in elderly patients with symptomless, uncomplicated hypertension, since the effectiveness of other drugs in reducing morbidity and mortality is not yet established. Recommendation for treatment of symptomless patients with isolated systolic hypertension may be premature. The ongoing Syst-Eur and Syst-Chin trials may provide further information.

The older hypertensive. Assessment and treatment.

The hypotensive action of the angiotensin-converting enzyme inhibitor captopril was investigated in 13 hypertensive patients in relation to their age, body weight, the pretreatment level of plasma renin activity (PRA), serum creatinine concentration and suppression of angiotensin II (PA II) by captopril. Captopril was administered in biweekly doubling doses (15 mg, 50 mg and finally 100 mg t.i.d.). The change in systolic blood pressure produced by captopril was significantly (p less than or equal to 0.05 or less) related to age (r = 0.67), to weight (r = 0.55), to the initial PRA levels (r = -0.63) and to the drop in PA II (r = 0.67) but not to the serum creatinine concentration. The change in diastolic blood pressure was also (p less than or equal to 0.05 or less) related to age (r = 0.59), to the pretreatment PRA level (r = -0.71) and to the fall in PA II (r = 0.70) but not to weight or to serum creatinine concentration. Our data suggest that the hypotensive action of captopril is more pronounced in younger hypertensive patients.
