Abstract: Streptococcus pneumoniae is the leading agent of bacterial meningitis in adult patients in developed countries [1]. Multidrugresistant (MDR) pneumococcal strains are a growing problem for empirical antibacterial therapy in invasive, often life-threatening, infections. Here we report the case of a 54-year-old man who was admitted to the intensive care unit (ICU) of Minsk City Hospital of Infectious Diseases (Minsk, Belarus) in April 2015 because of fever, severe headache, nausea, vomiting and altered consciousness. On examination, he had a temperature of 40 C, a respiratory rate of 22 breaths/min with normal oxygen saturation, a heart rate of 110 beats/min and his blood pressure was 150/90 mm Hg. Cardiac, lung and abdominal examinations revealed no pathology. Neck stiffness and Kernig’s symptom were clearly pronounced. The Glasgow Coma Scale (GCS) was 12. The cranial nerves were intact. One month prior to the onset of fever the patient had facial pain in the projection of the maxillary sinuses but did not seek medical attention. He had not received any systemic antibiotics during the previous 3 months. His past medical history was unremarkable. The patient’s relatives reported the fact of alcohol abuse for the last 3 years. On admission, blood examination revealed leucocytosis (24 500 cells/mL; normal range 4000–9000 cells/mL) and an increased C-reactive protein level (36 IU/L; normal <6 IU/L). Cerebrospinal fluid (CSF) analyses showed a white blood cell (WBC) count of 3904 WBC/mL (95% neutrophils, 5% lymphocytes) (normal range <5 WBC/mL), glucose concentration 3.1 mmol/L (normal range 2.2–3.8 mmol/L) and protein concentration 1.0 g/L (normal range 0.15–0.45 g/L). Treatment with meropenem 2.0 g three times daily and dexamethasone 10 mg every 6 h was started pending laboratory results. CSF-PCR was positive for S. pneumoniae DNA and 40 h later CSF culture grew a MDR S. pneumoniae strain (BLR/CSF/385/2015). According to the susceptibility of the micro-organism, moxifloxacin 400 mg once daily was added. Radiography of the paranasal sinuses revealed bilateral levels of fluids. Following puncture of the maxillary sinus, pus was obtained. The patient was transferred to a multidisciplinary hospital where surgical revision of the maxillary sinuses was performed. Two days later the patient’s fever decreased and he experienced significant improvement. After the full 14-day-course of antibacterial therapy, he was discharged completely recovered.

Abstract: Vacancy-oxygen complexes VnOm (n, m ≥ 1) in crystalline silicon are nucleation centers for oxygen precipitates, which are widely used as internal getters in modern technologies of production of silicon-based electronic devices and integrated circuits. For the controllable formation of oxygen precipitates in Si crystals in the technology processes the methods of determination of concentrations of the VnOm complexes are required. The aim of the present work was to find values of the calibration coefficients for determination of concentrations of the VnOm defects in Si from intensities of infrared (IR) absorption bands associated with the local vibrational modes (LVM) of these complexes. A combined electrical (Hall effect) and optical (IR absorption) study of vacancy-oxygen defects in identical silicon crystals irradiated with 6 MeV electrons was carried out. Based on the analysis of the data obtained, the values of the calibration coefficient for the determination of concentration of the vacancy-oxygen (VO) complex in silicon by the infrared absorption method were established: for measurements at room temperature (RT) – NVO = 8.5 · 1016 · αVO-RT cm–3, in the case of low-temperature (LT, Т ≡ 10 K) measurements – NVO = 3.5 · 1016 · αVO-LT cm–3, where αVO-RT(LT) are absorption coefficients in maxima of the LVM bands due to the VO complex in the spectra measured at corresponding temperatures. Calibration coefficients for the determination of concentrations of other VnOm (VO2, VO3, VO4, V2O and V3O) complexes and the oxygen dimer (O2) from an analysis of infrared absorption spectra measured at room temperature have been also determined.

Abstract: Background Avascular necrosis (AN) represents a significant problem of modern rheumatology. The highest incidence of this disease is in able-bodied men. The risk of disability among patients with avascular necrosis of the femoral head (ANFH) is high, especially in case of bilateral process, as AN tends to progress steadily and results in osteoarthritis due to untimely diagnosis. Patients with AN can be affected by other coexistent diseases, that can influence the disease presentation and management. Objectives This study aimed to analyse the prevalence and pattern of comorbidities in patients affected by ANFH and to determine if there is a differential pattern of comorbidities in patients with bilateral and unilateral ANFH. Methods This was a cross-sectional study, comprising 102 adult patients, aged 48,38–58 with a diagnosis of ANFH according to Ficat and Arlet criteria. Sociodemographic, anthropometric, clinical parameters of interest were collected from medical records. In order to characterise the burden of coexisting diseases or conditions Charlson Comorbidity Index was calculated. The data obtained were processed with STATISTICA 10.0 using descriptive and nonparametric statistics. Results Among a total of 102 patients, a significant prevalence of comorbidities (86,27%) was found. 78,85% of all patients suffered from 2 or more coexistent conditions. The most prevalent comorbidities were: cardiovascular diseases (CVD) (53,92%), including hypertension (50,0%) and cardiosclerosis (34,31%), endocrine and metabolic disorders (66,67%), including hypercholesterolemia (65,70%) and obesity (32,35%), diseases of the digestive system (30,39%), dorsopathies (22,55%). The number of comorbidities was associated with age (ρ=0,42, p In patients with bilateral (n=44), compared with those affected by unilateral ANFH (n=58), prevalence of younger age (43,5 (36–51) vs. 54,43–62 p Conclusions ANFH is associated with high prevalence of comorbidities, especially CVD. CVD, including hypertension, were more likely to be found in patients with unilateral ANFH, as well as higher comorbidity burden. It can be explained by the fact that patients with CVD received treatment according to national guidelines, including antihypertensive drugs, anticoagulants, statins, etc. This indicates that performing secondary prevention of CVD can be important in both CVD and ANFH, as such treatment can influence on intraosseous blood circulation in the contralateral joint. Abstract: The aim of this study was the evaluation of the influence of head surface roughness on the wear of the acetabular component of implant. The experiments were executed for the combination of UHMWPE (Ultra High Molecular Weight Polyethylene) implant cup and metal head of implant with several different surface roughnesses. Testing method was based on Pin-on-Disc principle. Finally the recommendation for head surface roughness was established. The special experiments were carried out in accredited “Mechanical Testing Laboratory” (quality system ISO 17025) in collaboration with the Academy of Sciences of the Czech Republic and company BEZNOSKA Corporation - developing and producing bone-substitute biomaterials and implants.
