Abstract: Sigmoid volvulus is very uncommon cause of intestinal obstruction in pediatrics population withhigh rate of mortality. To date, few cases of sigmoid volvulus in children and association with several condition has been reported in literature, of them very few cases are with mental disability. We report a challenged (mentally disabled) 14-year old adolescent boy presented asan emergency with feature of complete bowel obstruction. Abdominal X-rays shows dilated loop of large bowel with inverted U shaped. Volvulus of sigmoid colon was found during laparotomy and successfully managed with resection of a redundant colon with colocolic end to end anastomosis. Sigmoid volvulus is relatively uncommon in children as compared to adults. Surgeons should be attentive of this rare entity, cause of large bowel obstruction to allow for early diagnosis and to enable better patient outcomes by reducing the morbidity and mortality.

Abstract: Introduction: The outbreak of coronavirus disease in Nepal led medical colleges to suspend in person teaching-learning activities and ultimately online platform was introduced to deliver the contents of medical education. The objective of this study was to describe the perception of medical students towards online teaching-learning introduced during the COVID-19 outbreak in Nepal. Methods: An online survey using a descriptive cross-sectional study design was carried out among 515 undergraduate medical students currently enrolled in medical colleges in Nepal. Ethical approval was sought from Nepal Health Research Council to conduct this study, and digital informed consent was taken from study respondents. A semi-structured questionnaire in Google form was utilized to collect data. The link of the Google form was sent to the potential respondents through email and social media. Descriptive statistics, including frequency, percentage, mean, and standard deviation were used to analyze data in Stastical Package for the Social Sciences version 20. Results: The overall score of perception of online teaching-learning was 17.61±7.19, which indicated many problems in this method of teaching-learning. The mean score of perception of online teaching-learning was found to be different across sex, location of enrolled medical colleges, having a personal electronic device, having an internet connection at residence, having separate room/space for attending online classes, and self-rated computer skills. Moreover, only 28 (5.4%) of respondents had perceived online teaching-learning as a better method of delivering content of medical curricula. Conclusions: Surveyed medical students in Nepal were found to perceive many problems in online teaching-learning. Moreover, management and faculty members need to take the necessary measures for enhancing the online teaching-learning quality.

Abstract: Objectives: to assess and compare the knowledge on self-care management of pregnancy induced hypertension among primi gravid and multi gravida mothers. Method: A non-experimental descriptive comparative was conducted between 40 primi gravida mother and 40 multi gravida mothers who were selected by using purposive sampling to assess knowledge on self-care management of pregnancy induced hypertension. Self-structured questionnaire was used to collect the data. Appropriate Descriptive and inferential statistics were used to analyse data and to draw inferences. Results: 55% of primi gravida mothers and 40 % of multi gravid mothers had inadequate knowledge. Only 2.5% of primi gravida and 5% of multi gravida mothers had adequate knowledge. Respondents from primigravida had knowledge about 31.8 % and 38.6% regarding signs & symptoms and Preventive measures. 35% and 44.7 % of multi mothers were aware about signs & symptoms and preventive measures respectively. Which shows all the respondent were less aware about pregnancy induced hypertension. And there is an association between age & Monthly income on knowledge level. Conclusion: There is a necessity to provide information for those specific at risk population. Which helps to increase the knowledge, develop positive attitude, prevent complications and to have better maternal and child health.

Abstract: With the current advancement in wireless networks and various technologies implemented for automation, more innovative and improved ideas are developed to build automation systems facilitating remote controlling and monitoring of devices. In this paper, a GSM based wireless home automation system is proposed and deployed which executes its function of controlling and monitoring appliances remotely. It is compliant, cost-effective, low power consumption; highly efficient GSM (Global System for Mobile Communication) based wireless home system. The Interactive Voice Response System (IVRS) is improvised to embellish the system's security and ease of operation. With the help of this system the user can access his home appliances from anywhere and at anytime as per the requirement aiding convenience. The system permits the user direct devices through his mobile by sending voice commands using IVRS system. It also enables the user to monitor the status of loads with SMS update. A detailed system analysis is carried out and presented in this paper.

Abstract: In the image processing domain, researchers all over the world aim at designing algorithms which posses the feature of facilitating multiple advantages amalgamated in one scheme viz. compression, fusion, enhancement, security etc. In various classified applications such as military, medical, forensics etc, the image databases are huge and contain a lot of information. Hence, compression of these high-quality images and securing these databases becomes imperative. This paper proposes a Discrete Wavelet Transform (DWT) guided image compression as well as image security technique. The RGB color images are converted into textured images by exploiting DWT properties and thereby facilitating image compression. Since this process is reversible, the colors in the image are retrieved back from the texture patterns. In addition, the security of the image is enhanced based on a novel probabilistic triangular shuffling scheme. The overall proposed algorithm assists in achieving 67–70% compression ratio and improved security. Evaluating the performance of the algorithm, various graphs pertaining to PSNR, MSE, probability of security breach are realized.
