Abstract: Factor XI deficiency is a rare condition with an estimated prevalence of about one in one million and is more commonly seen in Ashkenazi Jews (8-9%) due to consanguinity. It occurs because of mutations in the factor XI gene (F11) on chromosome 4(4q35). Patients with this disorder may remain asymptomatic until they undergo any surgical procedure or delivery. The most common sites of bleeding include the oral cavity, pharynx, and genitourinary tract, where there is high fibrinolytic activity. Our patient was asymptomatic his whole life. He never had spontaneous bleeding or bruising; however, he had severe bleeding requiring multiple transfusions of fresh frozen plasma during and after surgeries.

Abstract: Introduction: Direct laryngoscopy is associated with sympathetic stimulation and altered hemodynamics. A long intubation time may result in a greater in stress response. Alternative techniques using video laryngoscopes have been developed that do not require direct vocal cord visualization and may decrease the hemodynamic response. This study aimed to compare the difference between hemodynamic changes and intubation time with Airtraq video laryngoscope and conventionl Macintosh direct laryngoscope. Methods: A prospective randomized comparative study was conducted involving 100 adult patients who were undergoing elective surgeries under general anesthesia and endotracheal intubation. The patients were randomly assigned to group V (Video laryngoscope) or group D (Direct laryngoscope). In addition to the baseline vitals and vitals at various time intervals, intubation time was also recorded. We considered a difference in Heart Rate and Mean Arterial pressure of 20% to be clinically significant and statistical significance was p-value <0.05. Results: Significant difference was found in heart rates immediately after laryngoscopy (110.40 vs. 105.02 beats/minute; p<0.01) and 1 minute after intubation (109.30 vs. 106.20 beats/minute; p<0.01) with attenuation seen in video laryngoscopy group. Blood pressures were similar in both the groups at all times. Time for intubation was prolonged in video laryngoscopy group than that for direct laryngoscopy group (26.54 vs. 22.80 seconds; p<0.05). There were no adverse events associated with either of the techniques. Conclusions: The Airtraq video laryngoscopy resulted in lesser change in heart rate and longer intubation time. However, clinical impact of such a difference seemed to be insignificant.

Abstract: The main aim of research is to improve quality education using digital tools like ICT tools with the combination of dialog system. By using digital tools create a more interactive and interesting environment for teachers and student. To improve performance of education used dialog system it takes input in the form of speech is natural dialog rather than text means natural dialog between humans and machines. This process is done by using artificial intelligence. Now days Virtual Personal Assistants (VPAs) used by many companies for their own proposed application like Google Assistant etc. The process of proposed digital education system first step is used natural dialog as input to dialog system and in second step it gives output in the form of animated graph like When user call ‘A for apple’ then on projector show the Drawing of ‘letter A’ and retrieve ‘picture of apple’ from highly secure database. And second example is when user say “two friends walking on road” then dialog manager show “relationship between two animated persons” and show “running status of them.” Means this particular dialog system has a total knowledge of alphabets and numbers with grammar. As well as relations, different position and behavior of human. Means indirectly this dialog system work as well educated person. This application is more use full in primary education system at primary level there is required practice of student in good observation. When teacher say ‘A for Apple’ then dialog manager do its work means it Draw ‘letter A’ and retrieve ‘picture of apple’ from respective database. And when user say “two friends walking on road” then dialog manager show “relationship between two animated persons” and show “running status of them.” Means student virtually imagine the each statement of story is show on projector board with the help of dialog manager. This time of teacher is utilized for give more attention on student and make student perfect. Means indirectly reduced the effort of teacher and improve accuracy of education. This application also used in many area such as write any letter, speech, sentence. Indirectly reduced the effort of user and improve accuracy by reducing grammatical mistake.

Abstract: INTRODUCTION: Ocular surface squamous neoplasia (OSSN) consists of a wide range of conjunctival and corneal lesions ranging from dysplastic lesions to invasive squamous cell carcinoma. In recent times, the incidence of OSSN seems to be on the rise, especially in developing countries. The present study was aimed to analyse demographic pattern, clinical characteristics, and histopathology findings of OSSN in a tertiary care centre of western region of Nepal. MATERIAL AND METHODS: It was a retrospective study. We analyzed 94 cases of OSSN who presented to cornea department of Lumbini Eye Institute, Bhairahawa, Nepal over a period 1.5 years from 1st July 2017 to 31st December 2018. All the patients with OSSN, detailed clinical history and examination were recorded. Lesions were excised with a 3 mm margin clearance and sent for histopathological examination. RESULTS: Mean age of our patients with OSSN was 48.89±17.955 years ranging from 17 to 85 years. There were 52 (55.32%) male and 42 (44.68%) female. Mean duration of presentation was 6.34±6.17 months. A solitary nodule at the limbus was the commonest presentation. Right eyes were involved more than left eyes. Lesions were found most commonly on temporal site 52 (55.32%) followed by nasal 36 (38.30%). On histopathological examination benign lesions were found in 31 (32.98%) eyes, preinvasive lesion in 50 (53.19%) eyes and invasive lesions in 13 (13.83%) eyes. CONCLUSION: OSSN were seen more commonly in young adults with male predominance. Benign and pre invasive lesions are found more commonly than invasive lesions.

Abstract: In this paper, we introduce a method of user loading for interleave division multiple access (IDMA) based two-tier Heterogeneous network (HetNet) to achieve improved performance in terms of lower bit-error rate (BER) for active micro-cell users present in the coverage area of a macro-cell base station (BS). Based on the fading scenario, the proposed method adjusts the load distribution among the macro-cell BS and the participating micro-cell BSs in the HetNet structure to guarantee relatively low BER to the mobile users. In this study, we propose a HetNet employing an adaptive cyclic interleaving technique at the IDMA transmitter and a multiuser–multipath (MUMP) detector at the receiver to support load distribution. With the aid of cyclic interleaving, MUMP detector resolves each received multipath component through multipath detection rather than conventional single path detection. Then, via theoretical study and extensive simulations, first, we demonstrate how the cyclic interleaving mechanism introduced in IDMA transmitter exploits the diversity property of the maximal-length sequence (m-sequence). Next, the superiority of the MUMP detector over single path detectors in multipath Rayleigh fading channel is shown and also the effect of equal-gain combining (EGC) and maximal-ratio combining (MRC) schemes in system performance is analysed for different BS load distribution in IDMA based HetNet.
