Abstract: Objectives. Chikungunya and scrub typhus infection are important causes of undifferentiated fever in tropical zones. The clinical manifestations in both conditions are nonspecific and often overlap. This study compares the clinical manifestations and the outcome of chikungunya with chikungunya-scrub typhus coinfection in children. Methods. A hospital-based observational study was conducted in children below 15 years of age over 16-month duration in 2017-2018. Chikungunya was diagnosed by IgM ELISA. All positive chikungunya cases were subjected to scrub typhus testing, dengue testing, leptospira testing, and malaria testing. Clinical manifestations and outcomes of all patients were recorded. Results. Out of the 382 admitted cases with fever, 11% ( ) were diagnosed with chikungunya, and the majority ( , 71.4%) were male. Among the 42 chikungunya cases, 17 (40.5%) tested positive for scrub typhus and one positive for falciparum malaria. Out of a total of 42 chikungunya cases, myalgia, nausea/vomiting, headache, abdominal pain, lymphadenopathy, hepatomegaly, splenomegaly, and edema were 81%, 73.8%, 66.7%, 64.3%, 59.5%, 52.4%, 40.5%, and 38.1%, respectively. Besides, altered sensorium (31%), jaundice (26.2%), dry cough (21.4%), shortness of breath (19%), and seizures (16.7%) were other clinical manifestations present in this group of children. Patients with chikungunya-scrub typhus coinfection reported headaches, pain in the abdomen, dry cough, shortness of breath, seizures, and splenomegaly, significantly more ( value < 0.05) compared to those with chikungunya only. Thirteen (31%) children developed shock, five in the chikungunya group and eight in the chikungunya-scrub typhus coinfection group. Six children in the coinfection group received inotrope. Among the chikungunya-only cases, 22 recovered and one died, whereas in the chikungunya-scrub typhus coinfection group, fourteen recovered and three died. Conclusions. Both the chikungunya and scrub typhus coinfection groups shared many similar clinical manifestations. In children, coinfection with scrub typhus often leads to modification of the clinical profile, complications, and chikungunya outcome.

Abstract: Title : Correlation between BMI and pregnancy Outcome among postnatal mothers with pregnancy Induced hypertension in selected hospital Bangalore. Objectives : To identify and correlate BMI and pregnancy Outcome among postnatal mothers with pregnancy Induced hypertension. Method : A non experimental correlation design was utilized among 80 postnatal mothers who were diagnosed as Pregnancy induced hypertension during their antenatal period selected as samples by using purposive sampling technique. Demographic data were collected by interview method, their BMI was calculated, pregnancy outcomes were identified from records by using an outcome checklist. Results : Underweight mothers had low birth weight babies and received NICU care. Among normal weight mothers 17.5 % delivered by LSCS, 15% babies were low birth weight babies 12.5 were preterm babies among them 10% received NICU care. In the overweight group 18.75% undergone LSCS, 18.75% were LBW and 1.25% VLBW, 8.75% babies were preterm, 12.5% newborn received NICU care. Among Obese mothers 8.75% delivered by LSCS, 6.25% of LBW babies, 5% were preterm and all of them received NICU care. There is a positive correlation between BMI and diagnosis and type of delivery. Significant at .01 and .05 Level (p value .008 and .019 respectively). Negative correlation between birth weight and diagnosis and gestational age. r = -.499 significant at .01 level (p value .000) Conclusion : Obesity and under weight is a leading, preventable cause of mortality worldwide. Preeclampsia increases maternal and perinatal morbidity and mortality rates. All women who are in reproductive age group and under risk to develop pregnancy induced hypertension need to be educated about to maintenance of normal weight before pregnancy. Nurses have more responsibility on creating awareness among women how to maintain normal weight to avoid development of complications to the mother and newborn.

Abstract: Introduction Ventilator-associated pneumonia (VAP) is the most frequent intensive care unit (ICU) acquired infection among patients receiving mechanical ventilation. Accurate clinical and microbiologic diagnosis of VAP is essential not only for selection of appropriate antimicrobials but also to prevent their misuse. As the organisms and their sensitivity pattern may differ in every ICU, the knowledge of the resident flora and their behaviour should be known for successful treatment. Methods The study was conducted to evaluate the organisms responsible for VAP and their Antibiotic Sensitivity Pattern for the study setting. A prospective, open, epidemiological clinical study was performed in a tertiary care hospital in Nepal. 100 patients admitted to ICU and Mechanically Ventilated were evaluated about VAP. Clinical Pulmonary Infection Score (CPIS) was used to diagnose VAP. Results Among 60 patients ventilated for more than 48 hours, 25 (41.6%) developed VAP. The VAP was caused predominantly by Klebsiella pneumonia in 34.5% of cases, followed by Acinetobacter calcoaceticus baumanni in 27.6%, Acinetobacter wolffi and Pseudomonas aeruginosa in 13.8% each and Escheresia coli in 10.3%. The most sensitive antibiotics were Colistin, followed by Polymyxin B and Amikacin with sensitivity rates of 67%, 60% and 58%, respectively. Conclusion Based on these results, an empiric approach to antibiotic treatment can be made tailored to the specific settings. Given the magnitude of drug resistance and its implicated financial and societal burden, there is an urgent need for broad implementation of Antibiotic Stewardship programs across all health care settings.

Abstract: Introduction: Third molar extractions are one of the most common reasons of injury to mandibular nerve and postoperative complications. Pre-operative assessment of pending doom is a must. In order to study the relations between change in Inferior Alveolar Canal (IAC) and Type, Class, and Level of impaction with radiographic root change of impacted third mandibular molar, we designed a study to assess the relationship of the mandibular canal to the roots of impacted third molar in reference to changes in roots and factors affecting it. Methods: This was a retrospective cross-sectional study wherein the panoramic radiographic records of subjects with impacted third molar were assessed for change in root and its correlation with change in IAC, Type, Class and Level of impacted mandibular third molar. Results: Roots of impacted third molar when in contact with IAC presented most often in our study subjects with the Darkening of roots in both sides {right side (16.79%) and left side (11.29%)} and both genders {males (right-14.29%, left-8.75%) and females (right-19.39%, left-13.94%)}. They were statistically significantly associated with interruption of white line of IAC, vertically impacted third molars and Pell and Gregory Class II, Level A of impaction. Conclusion: There is a need to properly identify radiographic risk predictor signs to avoid any surprises at the time of extraction and post operation complications following it.

Abstract: BACKGROUND: Femoral neck fracture is a difficult problem in adult patients and without proper treatment leads to high functional impairment and frequent complications. OBJECTIVE: The aim of this study was to see the functional outcome in patients with non union of fracture neck of femur treated with intertrochanteric osteotomy. MATERIAL AND METHODS: All 14 cases operated and non operated (neglected) fracture neck of femur were enrolled for the study 2003 to 2009 in National Medical College, Birganj and Janaki Medical College, Janakpur. Patients were treated conservatively and in some patients failure of fixation was used to evaluate the functional outcome. RESULTS: There were 14 patients with average age of 30 years with age range 14-50 years. 8 patients were male and six were female. 10 patients were previously treated conservatively and 4 patients presented with failure of fixation. Average duration of fracture was 7 months (2 months -3 years with pauwel angle of 32.8. CONCLUSION: Neglected neck of femur fracture should be treated by three cancellous screw fixation with molded and angled narrow DCP plate, valgus osteotomy and autogenous bone grafting round the fracture to achieve a high rate of patient satisfaction and satisfactory functional outcome.
