TLDR
Examination of brain revealed Lewy bodies as well as marked neuronal loss in brainstem monoaminergic nuclei-particularly locus coeruleus and substantia nigra-that inhibit cholinergic neurons in the pedunculopontine nucleus mediating atonia during REM sleep.Abstract:
A patient with REM sleep behavior disorder who subsequently developed probable Lewy body dementia is now reported to have a definite pathologic diagnosis of Lewy body dementia. Examination of brain revealed Lewy bodies as well as marked neuronal loss in brainstem monoaminergic nuclei-particularly locus coeruleus and substantia nigra-that inhibit cholinergic neurons in the pedunculopontine nucleus mediating atonia during REM sleep.read more

Control of Sleep and Wakefulness
TL;DR: Genetic studies suggest that brain mechanisms controlling waking and NREM sleep are strongly conserved throughout evolution, underscoring their enormous importance for brain function.

TL;DR: The data suggest that many patients with 'idiopathic' RBD are actually exhibiting an early clinical manifestation of an evolving neurodegenerative disorder, and may be appropriate for future drug therapies that affect synuclein pathophysiology, in which the development of parkinsonism and/or dementia could be delayed or prevented.

The scientific and clinical basis for the treatment of Parkinson disease (2009)
TL;DR: This monograph provides an overview of the management of PD patients, with an emphasis on pathophysiology, and the results of recent clinical trials to provide physicians with an understanding of the different treatment options that are available for managing the different stages of the disease and the scientific rationale of theDifferent approaches.

Visual hallucinations in Lewy body disease relate to Lewy bodies in the temporal lobe.
TL;DR: Cognitive fluctuations and/or visual hallucinations were not good predictors of DLB in pathologically proven patients, although the absence of these features early in the disease course was highly predictive of PDD.

REM sleep behavior disorder
TL;DR: Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia manifested by vivid, often frightening dreams associated with simple or complex motor behavior during REM sleep as discussed by the authors.

TL;DR: This work identified progressive disabling mental impairment progressing to dementia as the central feature of DLB, and identified optimal staining methods for each of these and devised a protocol for the evaluation of cortical LB frequency based on a brain sampling procedure consistent with CERAD.

TL;DR: A group of predominantly male patients with a characteristic association of RBD and degenerative dementia is reported, and it is hypothesized that the underlying pathology in these patients is DLB.
