This article provides an update on the global cancer burden using the GLOBOCAN 2020 estimates of cancer incidence and mortality produced by the International Agency for Research on Cancer. Worldwide, an estimated 19.3 million new cancer cases (18.1 million excluding nonmelanoma skin cancer) and almost 10.0 million cancer deaths (9.9 million excluding nonmelanoma skin cancer) occurred in 2020. Female breast cancer has surpassed lung cancer as the most commonly diagnosed cancer, with an estimated 2.3 million new cases (11.7%), followed by lung (11.4%), colorectal (10.0 %), prostate (7.3%), and stomach (5.6%) cancers. Lung cancer remained the leading cause of cancer death, with an estimated 1.8 million deaths (18%), followed by colorectal (9.4%), liver (8.3%), stomach (7.7%), and female breast (6.9%) cancers. Overall incidence was from 2-fold to 3-fold higher in transitioned versus transitioning countries for both sexes, whereas mortality varied <2-fold for men and little for women. Death rates for female breast and cervical cancers, however, were considerably higher in transitioning versus transitioned countries (15.0 vs 12.8 per 100,000 and 12.4 vs 5.2 per 100,000, respectively). The global cancer burden is expected to be 28.4 million cases in 2040, a 47% rise from 2020, with a larger increase in transitioning (64% to 95%) versus transitioned (32% to 56%) countries due to demographic changes, although this may be further exacerbated by increasing risk factors associated with globalization and a growing economy. Efforts to build a sustainable infrastructure for the dissemination of cancer prevention measures and provision of cancer care in transitioning countries is critical for global cancer control.

INTRODUCTION
Presence of anti-E1E2 antibodies was previously associated with spontaneous cure of hepatitis C virus (HCV) and predictive before treatment of a sustained virological response (SVR) to bi- or tri-therapy in naïve or experienced patients, regardless of HCV genotype. We investigated the impact of anti-E1E2 seroprevalence at baseline on treatment response in patients receiving direct-acting antiviral (DAA) therapy.


MATERIAL AND METHODS
We screened anti-E1E2 antibodies by ELISA in serum samples collected at treatment initiation for two groups of patients: 59 with SVR at the end of DAA treatment and 44 relapsers after DAA treatment. Nineteen patients received a combination of ribavirin (RBV) or PEG-interferon/ribavirin with sofosbuvir or daclatasvir and others received interferon-free treatment with DAA±RBV. HCV viral load was measured at different time points during treatment in a subgroup of patients.


RESULTS
A significant association was observed between presence of anti-E1E2 and HCV viral load<6log10 prior treatment. Among patients with anti-E1E2 at baseline, 70% achieved SVR whereas among patients without anti-E1E2, only 45% achieved SVR. Conversely, 66% of patients experiencing DAA-failure were anti-E1E2 negative at baseline. In the multivariate analysis, presence of anti-E1E2 was significantly associated with SVR after adjustment on potential cofounders such as age, sex, fibrosis stage, prior HCV treatment and alanine aminotransferase (ALT) level.


CONCLUSIONS
The presence of anti-E1E2 at treatment initiation is a predictive factor of SVR among patients treated with DAA and more likely among patients with low initial HCV viral load (<6log10). Absence of anti-E1E2 at baseline could predict DAA-treatment failure.

Si l'actualité sur les hépatites virales est aujourd'hui largement dominée par les progrès thérapeutiques pour le traitement de l'hépatite C, les récents développements de la recherche pourraient permettre une avancée aussi remarquable pour le traitement des infections par le virus de l'hépatite B (VHB). La prévalence mondiale de l'hépatite B chronique demeure supérieure à celle des infections par le virus de l'hépatite C (VHC) ou le virus de l'immunodéficience humaine (VIH) et le VHB est le premier facteur de risque de carcinome hépatocellulaire (CHC). La problématique de la prise en charge thérapeutique de l'hépatite B chronique diffère de celle de l'hépatite C. La particularité du VHB est la formation, dès la primo-infection, d'un minichromosome viral ou ADN circulaire clos de façon covalente (ADNccc) au sein des cellules infectées. Ce contingent intra-hépatocytaire d'ADNccc n'est pas significativement affecté par les traitements actuels. D'autre part, les réponses immunitaires antivirales sont défectives chez les patients infectés de façon chronique par le VHB. Les traitements des hépatites B chroniques actuellement disponibles sont l'interféron pégylé (PEG IFN) et les analogues de nucléos(t)ides (NUC). Le PEG IFN est un traitement immuno-modulateur dont le but est de stimuler la réponse immunitaire antivirale pour obtenir un contrôle de la réplication du VHB. Ce traitement présente l'avantage d'avoir une durée limitée (12 mois) mais l'efficacité virologique est limitée à certains patients et les effets secondaires sont nombreux [1]. Les NUCs sont bien tolérés et ont une très bonne efficacité virologique [2]. Ils n'ont que peu d'effet sur le contingent d'ADNccc intracellulaire et une réactivation virale survient très fréquemment après interruption du traitement. Since the discovery, substantial improvement in the understanding of hepatitis B virus (HBV) virology, host immune response, and natural course, combined with the advent of effective vaccine and antiviral drugs, has led to better control for chronic HBV infection, as well elaborated and demonstrated in the chapters in this book. In particular, 183 countries have implemented vaccination program as of 2013. The estimated global population with chronic HBV infection has decreased from 350 million to 240 million in the most recent estimate in 2012. In addition, potent nucleos(t)ide analogs (NUC) with high genetic barrier to drug resistance are able to maintain long-term HBV suppression, improve liver histology, reverse hepatic fibrosis, and reduce hepatocellular carcinoma (HCC). However, there are challenges ahead to achieve eradication of HBV.

Towards HBV Eradication: Future Perspective

L es hépatites B chroniques demeurent un problème majeur de santé publique à l’échelon mondial. Ce point a été particulièrement souligné lors de la dernière conférence de consensus européenne à Genève au mois de septembre 2002. Des traitements efficaces sont nécessaires pour prévenir la progression de l’hépatite B chronique vers la cirrhose, le carcinome hépatocellulaire et le décès [1]. Le traitement par interféron alpha nécessite une administration parentérale et est à l’origine de nombreux effets secondaires qui peuvent nécessiter un ajustement de doses ou son arrêt. Le risque de progression de la maladie hépatique est diminué chez les malades qui font une séroconversion HBe après un traitement par interféron alpha [2]. Chez les malades infectés par un mutant précore du virus de l’hépatite B (VHB), l’interféron alpha ne permet d’induire une réponse prolongée après l’arrêt du traitement que chez une minorité de malades. De plus, un traitement prolongé par interféron alpha est nécessaire dans cette forme clinique particulière [3], mais est rendu difficile compte tenu de la voie de l’administration et des effets secondaires.
