Abstract: Nonalcoholic fatty liver disease (NAFLD) spans steatosis through nonalcoholic steatohepatis, cirrhosis, and hepatocellular carcinoma (HCC) associated with striking systemic features and excess cardiovascular and liver-related mortality The pathogenesis of NAFLD is complex and multifactorial Endocrine derangements are closely linked with dysmetabolic traits For example, in animal and human studies, female sex is protected from dysmetabolism thanks to young individuals' ability to partition fatty acids towards ketone body production rather than very low density lipoprotein (VLDL)-triacylglycerol, and to sex-specific browning of white adipose tissue Ovarian senescence facilitates both the development of massive hepatic steatosis and the fibrotic progression of liver disease in an experimental overfed zebrafish model Consistently, estrogen deficiency, by potentiating hepatic inflammatory changes, hastens the progression of disease in a dietary model of nonalcoholic steatohepatitis (NASH) developing in ovariectomized mice fed a high-fat diet In humans, NAFLD more often affects men; and premenopausal women are equally protected from developing NAFLD as they are from cardiovascular disease It would be expected that early menarche, definitely associated with estrogen activation, would produce protection against the risk of NAFLD Nevertheless, it has been suggested that early menarche may confer an increased risk of NAFLD in adulthood, excess adiposity being the primary culprit of this association Fertile age may be associated with more severe hepatocyte injury and inflammation, but also with a decreased risk of liver fibrosis compared to men and postmenopausal status Later in life, ovarian senescence is strongly associated with severe steatosis and fibrosing NASH, which may occur in postmenopausal women Estrogen deficiency is deemed to be responsible for these findings via the development of postmenopausal metabolic syndrome Estrogen supplementation may at least theoretically protect from NAFLD development and progression, as suggested by some studies exploring the effect of hormonal replacement therapy on postmenopausal women, but the variable impact of different sex hormones in NAFLD (ie, the pro-inflammatory effect of progesterone) should be carefully considered

Abstract: We collected 75 primary pulmonary carcinomas with pleomorphic, sarcomatoid, or sarcomatous elements to better define their clinical, histologic, and immunohistochemical profile. The patient's age ranged from 42 to 81 years (mean 65 years), and the male-to-female ratio was 9.7:1. Sixty-nine patients (92%) were smokers. Cough and hemoptysis were the most frequent presenting symptoms. Fifty-nine patients (65%) died of disease: only stage significantly predicts overall survival (p = 0.0273). Microscopically, based on the WHO criteria, 58 cases were classified as pleomorphic carcinoma (51 with an epithelial component, 7 composed exclusively of spindle and giant cells), 10 as spindle cell carcinoma, 3 as giant cell carcinoma, 3 as carcinosarcoma, and 1 as pulmonary blastoma. Immunohistochemically, in the tumors composed exclusively of spindle and/or giant cells, thyroid transcription factor-1 (TTF-1) and cytokeratin 7 were positive in 55% and 70% of the cases, respectively, whereas surfactant protein-A was always negative. In pleomorphic carcinomas with an epithelial component, cytokeratin 7, TTF-1, and surfactant protein-A were positive in the sarcomatoid component in 62.7%, 43.1%, and 5.9% of the cases, respectively, whereas they were always negative in the sarcomatous part of carcinosarcomas and blastoma. In the epithelial component of pleomorphic carcinomas, cytokeratin 7, TTF-1, and surfactant protein-A were positive in 76.4%, 58.8%, and 39.2% of the cases, respectively, whereas the same antibodies did not react with the epithelial component of carcinosarcomas; in the case of blastoma, the epithelial part of the tumor was positive for cytokeratin 7 and TTF-1, whereas it was negative for surfactant protein-A. Cytokeratin 20 was always negative. In our opinion, this study: 1) supports the metaplastic histogenetic theory for this group of tumors; 2) shows that cytokeratin 7 and TTF-1, but not surfactant protein-A, are useful immunohistochemical markers in this setting; 3) confirms that stage is at the moment the only significant prognostic parameter, as in conventional non-small cell lung carcinomas; and 4) shows that this group of tumors has a worse prognosis than conventional non-small cell lung carcinoma at surgically curable stages I, justifying their segregation as an independent histologic type in the WHO classification.

Abstract: Although lacking the components that characterize the acquired immunity systems of vertebrates, invertebrates nevertheless possess effective general innate immune mechanisms which exhibit striking parallels with those of vertebrates. These innate immune systems include both cellular and humoral elements. Invertebrate phagocytes synthesize both oxygen-dependent and oxygen-independent molecules to combat infectious agents. Cytotoxic substances employed by invertebrates include reactive intermediates of oxygen and nitrogen, antimicrobial peptides, lectins, cytokine- and complement-like molecules, and quinoid intermediates of melanin. The signal transduction pathways that are involved in mediating the production of these substances appear to be very similar among animal species, suggesting a common ancestral origin for the innate immune systems.

Abstract: In this paper, we propose a methodology for optimizing a solar harvester with maximum power point tracking for self-powered wireless sensor network (WSN) nodes. We focus on maximizing the harvester's efficiency in transferring energy from the solar panel to the energy storing device. A photovoltaic panel analytical model, based on a simplified parameter extraction procedure, is adopted. This model predicts the instantaneous power collected by the panel helping the harvester design and optimization procedure. Moreover, a detailed modeling of the harvester is proposed to understand basic harvester behavior and optimize the circuit. Experimental results based on the presented design guidelines demonstrate the effectiveness of the adopted methodology. This design procedure helps in boosting efficiency, allowing to reach a maximum efficiency of 85% with discrete components. The application field of this circuit is not limited to self-powered WSN nodes; it can easily be extended in embedded portable applications to extend the battery life.
