Abstract: Устната микрофлора е сложна система, в която резидентната микрофлора се намира в динамично равновесие с постъпващите отвън патогенни и хранителни елементи. Това равновесие може да бъде нарушено от редица фактори, сред които и наличието на фиксирани ортодонтски апарати, затрудняващи поддържането на добра хигиена на устната кухина и натрупване на зъбна плака. Целта на настоящия обзор е да се обобщят литературните данни относно влиянието на фиксираните ортодонтски апарати върху микрофлората на устната кухина. Някои проучвания съобщават за промяна в качеството и количеството на оралната микрофлора след ортодонтско лечение, например увеличено количество на Streptococcus mutans, Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, участващи в развитието на пародонтит. Това налага при пациенти с продължително носене на фиксирани ортодонтски апарати да се провежда микробиологично изследване на оралната микрофлора с цел превенция на развитие на възпалителни процеси в устната кухина.

Abstract: We review the current research literature on treatment behaviour for neoplasms of the female genital tract during pregnancy. Guidelines for clinical management of cervical cancer, ovarian tumours, and vulvar cancer are presented both regarding gynaecological oncologic treatment and obstetrics. Cervical cancer is the most common malignant tumour of the female genitalia during pregnancy due to the high incidence of this neoplasm in developing countries, including Bulgaria, on the one hand, and on the other, it affects women of reproductive age. Treatment algorithms depending on various factors - gestational age, stage of the disease, tumour lesion size, and presence of pelvic lymph node metastases, are presented. Ovarian tumours are classified into benign, borderline malignant, and malignant tumours. The latter, in turn, are divided into early and advanced stages, as well as epithelial and non-epithelial tumours, which can be detected at different stages of pregnancy.

Abstract: Introduction The definitions of sepsis and septic shock were redefined in 2016 This study compares the performance of qSOFA with that of SIRS criteria for the diagnosis of sepsis and prediction of 30-day mortality Aim The aim of this article is to assess the severity of the infection of patients using SIRS and qSOFA scales and to compare their specificity and predictive value Materials and Methods A prospective, non-interventional single-center clinical trial was conducted at St Marina University Hospital in Varna The sample included 87 patients with sepsis and septic shock The criteria for inclusion in the study were laboratory constellation for systemic exposure; over 18 years of age; with or without co-morbidities; no malignancies Pregnancy, neoplasia and the age of under 18 were the criteria for exclusion Logistic regression was used to test the predictability of both scales ROC curve analysis determined the sensitivity and specificity of SIRS and qSOFA Results Our analysis showed that both SIRS and qSOFA are significant predictors of mortality of septic patients The SIRS scale had a 2050-fold probability of predicting the death of the patient (p = 0004, 95% CI 1255 - 3349), whereas the qSOFA score was 2581 times more likely to predict mortality in patients with sepsis and septic shock (p = 00001, 95% CI 1557 - 4279) Cut-off values for SIRS higher than 25 points showed 91% sensitivity and 60% specificity - (AUC 080, 95% CI - 0712 - 0907), whereas qSOFA scores greater than 15 points indicated sensitivity of 822% and specificity of 703% (AUC 085, 95% CI 0 0770 - 0934) Conclusion SIRS and qSOFA criteria for early detection of sepsis are useful clinical tools for mortality reduction and predictability

Abstract: От 1985 г. международната здравната общност счита, че идеалният процент за цезарово сечение трябва да е между 10-15%. Оттогава оперативното раждане става все по-често срещана интервенция в световен мащаб. Данните за България сочат, че през 2015 г. относителният дял на оперативното раждане е 43% и продължава да нараства. Целта на настоящото изследване е да се проучи и сравни мнението на бременни и небременни (нераждали) жени относно причините за високия рейтинг на оперативните раждания у нас. Материал и методи: Проучено е мнението на 40 бременни жени и на 30 небременни, нераждали жени. За нуждите на изследването е използван документален метод, статистически (анкетен) метод и контекстуален анализ. Изследването е проведено в периода 1-20.07.2016 г. в град Велико Търново. Резултатите сочат, че всички анкетирани бременни жени желаят да родят по нормален път. Потвърждава се мнението, че най-честа причина за секцио е страхът от болката, следван от препоръките на лекаря, на когото жените се доверяват. Всяка трета небременна жена заявява, че се страхува от раждането. Всяка пета бременна изпитва страх от предстоящото раждане. Над половината от бременните (63%) биха избрали услугата „избор на екип` като средство за справяне със страха и тревожността. Половината от бременните биха искали техните партньори да присъстват на раждането, ако това е възможно.

Abstract: The research on medical device maintenance presented in this article is based on the definition of several maintenance strategies. Their main characteristics, advantages and disadvantages are formulated. Areas of application are analyzed. The focus is on Emergency maintenance strategies. Quantitative estimators have been created with which help can be evaluated effect of applying one or another form of maintenance. In this way, it is possible to avoid making subjective decisions about the way maintenance is performed.
