Abstract: Violations of cardiovascular and other autonomous functions are common in cerebrovascular disease. Various cortical and subcortical anatomical regions of the brain are involved in autonomic regulation, as cortex insularis has the most important role crust. Stimulation of the right cortex insularis increases sympathetic cardio-vascular tone, whereas the parasympathetic activity increased more frequently in stimulation of the left cortex insularis. The evidence regarding the impact of lateralization of stroke for cardiovascular risk are contradictory. Most authors demonstrated that patients with stroke affecting the right cortex insularis are more likely to develop cardiovascular autonomic dysfunction leading to reduced heart rate variability, cerebrogenic arrhythmias and an increased incidence of cardiac mortality.

Abstract: Retinopathy in premature babies, also called retrolental fibroplasia, is one of the leading causes of visual impairment in the pediatric population. The condition occurs in developed countries and the pathophysiological mechanisms for its development are due to the overall immaturity of the body, in particular the respiratory system. The need for the baby to be kept in a highly oxygenated environment, against the background of pantissue hypoxia, results in the proliferation of retinal vessels subsequently sprouting into the posterior ocular chamber. As the respiratory system matures, the vessels degenerate and are replaced by connective tissue, which can lead to many complications. In order to detect early vascular changes in the retina, the ocular bulb of a pediatric autopsy case was examined, associated with early changes of premature retinopathy and a single control pediatric autopsy without clinical data on retinal pathology. Both patients were of comparable age - 3 months. In the patient with clinical manifestations of retrolental fibroplasia in the outer layers of the retina, there was profound proliferation of blood vessels, including thick-walled ones, with impaired histoarchitectonics of the retina. The second patient presented with no clinical data and conventional retinal morphology without vascular proliferation. The condition does not require morphological diagnosis. The observational findings of fundoscopy, together with the clinical information, are often specific enough and with good therapeutic interventions - lung rehabilitation, control of the oxygen environment, it is possible for the changes to regress without pathological progression. Morphological diagnosis is indicated only in cases with missing medical records, clinical evidence of visual impairment and the need for differential diagnosis with other disorders in the retina, some of which may have a genetic etiology and pose a potential risk to other relatives.

Abstract: Introduction: Dynamic monitoring of the patients presenting with suppressed consciousness during the acute stage of stroke plays an important role in the diagnostic and therapeutic process as a whole. Aim: The objective of the present study was to analyze the initial results from the twofold transcranial Doppler sonography application in acute stroke patients with suppressed consciousness aiming at optimizing their timely treatment. Materials and Methods: The histories of 46 patients, 21 males and 25 females, at a mean age of 75 years with acute stroke and a different degree of suppressed consciousness hospitalized during the period from January 1, 2018 to January 31, 2019 were analyzed. All the patients were examined by means of transcranial Doppler sonography. The degree of suppressed consciousness was evaluated by means of Glasgow-Liege Coma Scale. Results: Thirty-four patients presented with acute ischemic stroke and 12 ones with acute hemorrhagic one. Several accompanying cardiovascular diseases that aggravated the course of the acute cerebrovascular accident were diagnosed. Considerable differences in terms of the mean values according to the Glasgow-Liege Coma Scale were observed. The intervals of the maximal, minimal and average sonographic parameters of four cerebral arteries such as left and right middle cerebral artery and left and right vertebral artery varied within broad limits, too. Conclusion: The application of a constellation of various neuroimaging methods in the patients with acute cerebrovascular diseases could identify essential relationships between the parameters analyzed and risk factors as well as to give assistance to the individualized therapeutic approach.

Abstract: Rectal carcinoma represents a significant health problem in the modern world. Its high rate of occurrence and mortality in the late stages of the disease stresses the need for better diagnostic, prognostic and predictive methods. We present a review of the literature concerning the staging modalities, including the characteristics of early rectal carcinoma. Endoscopy, endoluminal ultrasound and magnetic resonance imaging play a major role in the preoperative staging and the restaging of rectal carcinoma. The new devices and technologies improve the diagnostic process thus providing an option for new management strategies and better results.

Abstract: Въведение: Краткотрайното едностранно невралгиформено главоболие с конюнктивална инекция и сълзене (Short-lasting Unilateral Neuralgiform headache attacks with Conjunctival injection and Tearing, SUNCT) е сравнително рядко заболяване от групата на първичните главоболия. До 2015 г. са описани едва 50 случая. Диагностичното уточняване е затруднено поради припокриващи се симптоми с други нозологични единици, епизодичното протичане и преувеличаването или неглижирането от страна на пациента на симптомите, които се свързват и с емоционалното състояние на пациента. Клиничен случай: Представяме жена на 59 г., хоспитализирана във Втора клиника по нервни болести на УМБАЛ „Св. Марина“ по повод на оплаквания от силни болки в дясна лицева половина, със стрелкащ характер, зачестили и засилили се в последните 3 дена. Болките описва като силни, нетърпими, с пристъпен характер, повтарящи се многократно през деня, провокирани при говор, дъвчене, силен шум. По повод епизод с подобни пристъпи преди три години е поставена диагнозата тригеминална невралгия, като е проведена терапия с незадоволителен ефект.
