Abstract: Objective To identify current risk factors for hepatitis C virus (HCV) transmission in Greater Cairo. Design and Setting A 1∶1 matched case-control study was conducted comparing incident acute symptomatic hepatitis C patients in two “fever” hospitals of Greater Cairo with two control groups: household members of the cases and acute hepatitis A patients diagnosed at the same hospitals. Controls were matched on the same age and sex to cases and were all anti-HCV antibody negative. Iatrogenic, community and household exposures to HCV in the one to six months before symptoms onset for cases, and date of interview for controls, were exhaustively assessed. Results From 2002 to 2007, 94 definite acute symptomatic HCV cases and 188 controls were enrolled in the study. In multivariate analysis, intravenous injections (OR = 5.0; 95% CI = 1.2–20.2), medical stitches (OR = 4.2; 95% CI = 1.6–11.3), injection drug use (IDU) (OR = 7.9; 95% CI = 1.4–43.5), recent marriage (OR = 3.3; 95% CI = 1.1–9.9) and illiteracy (OR = 3.9; 95% CI = 1.8–8.5) were independently associated with an increased HCV risk. Conclusion In urban Cairo, invasive health care procedures remain a source of HCV transmission and IDU is an emerging risk factor. Strict application of standard precautions during health care is a priority. Implementation of comprehensive infection prevention programs for IDU should be considered.

Abstract: An improved performance of a photovoltaic (PV) pumping system employing a synchronous reluctance motor (SynRM) under partial shading conditions is proposed. The system does not include the dc-dc converter that is predominantly being utilized for maximizing the output power of the PV array. In addition, storage batteries are also not contained. A conventional inverter connected directly to the PV array is used to drive the SynRM. Further, a control strategy is proposed to drive the inverter so that the maximum output power of the PV array is achieved while the SynRM is working at the maximum torque per Ampere condition. Consequently, this results in an improved system efficiency and cost. Moreover, two maximum power point tracking (MPPT) techniques are compared under uniform and partial shadow irradiation conditions. The first MPPT algorithm is based on the conventional perturbation and observation (P&O) method and the second one uses a differential evolution (DE) optimization technique. It is found that the DE optimization method leads to a higher PV output power than using the P&O method under the partial shadow condition. Hence, the pump flow rate is much higher. However, under a uniform irradiation level, the PV system provides the available maximum power using both MPPT techniques. The experimental measurements are obtained to validate the theoretical work.

Abstract: Silymarin is a naturally occurring flavonoid drug; evidence from recent research has highlighted its use as a potential treatment for atopic dermatitis (AD). Both poor water solubility and drug permeability have hindered the percutaneous absorption of silymarin. Formulation of silymarin into pluronic-lecithin organogel (PLO) basis for topical skin delivery is the main aim of this work. Six different PLO formulations were prepared containing various pluronic to lecithin ratios using two cosolvent systems of ethyl alcohol and dimethyl sulfoxide. Formulation 2 (20% pluronic and 3% lecithin) was found to be the optimal base for topical delivery of silymarin as it showed optimum pH, viscosity, drug content, and satisfactory in vitro silymarin permeation. The silymarin PLO formulation significantly relieved inflammatory symptoms of AD such as redness, swelling, and inflammation. These findings warrant the ability for application of these novel silymarin PLO formulations as a novel treatment for AD.
