TLDR
The frequent occurrence of micro-involvement is a strong argument favouring routine D2 lymph node dissection in gastric cancer surgery in patients with lymph node metastasis.Abstract:
A retrospective immunohistological analysis of 100 patients with pT1-3 N0 and pT1-3 N1 gastric adenocarcinoma demonstrated a high frequency of micro-involvement in the removed lymph nodes. The presence of three or more tumour cells in more than 10 per cent of the lymph nodes was of significant prognostic value in the pN0 cases. Multivariate analysis identified micro-involvement as an independent prognostic factor. The results explain why patients benefit from lymphadenectomy even if the removed lymph nodes are not involved by tumour (pN0) in routine histological examination. The frequent occurrence of micro-involvement is a strong argument favouring routine D2 lymph node dissection in gastric cancer surgery in patients with lymph node metastasis.read more

Quantitative detection of micrometastases in the lymph nodes of gastric cancer patients with real-time RT-PCR: a comparative study with immunohistochemistry.

TL;DR: It is indicated that duplex quantitative real‐time RT‐PCR is the most sensitive method for detecting micrometastases and useful for evaluating the prognostic significance of lymph node micronetastasis in gastric cancer patients.

Survival benefit of D2 lymphadenectomy in patients with gastric adenocarcinoma

TL;DR: The increment of surgical morbidity and mortality rates attributed to D2 LND is largely caused by the effect of splenectomy and pancreaticosplenectomy, and the value of extended LND in the surgical treatment of gastric cancer is supported.

Evaluation of in vivo endoscopic autofluorescence spectroscopy in gastric cancer

TL;DR: Light-induced autofluorescence spectroscopy is a new and promising bio-optical technique for the endoscopic in vivo diagnosis of gastric adenocarcinoma and the poor diagnostic accuracy for signet-ring cell carcinoma is explained by the diffuse and frequent submucosal growth of this tumor and the presence of collagen fibers.

TL;DR: SN biopsy is feasible in early stage gastric cancer, however, complementary procedures may be needed to improve the detection rate and predictive values.

TL;DR: Given the low probability of lymph node metastasis and recurrence in tumors less than 1 cm in diameter limited to the mucosa, more limited surgery maybe appropriate in these carefully selected instances.

TL;DR: The APAAP technique was found particularly suitable for labeling cell smears and for detecting low numbers of antigen-bearing cells in a specimen and could be used in conjunction with immunoperoxidase methods for double immunoenzymatic staining.
