Abstract: Objective
To examine the incidence and risk factors for incident thoracic spine pain (TSP) in workers representative of a French region's working population.
Methods
In this prospective study, 3,710 workers were assessed in 2002–2005, and 2,332 (62.9%) of them were reassessed in 2007–2010. TSP was assessed by a self-administered Nordic questionnaire at baseline and at followup. At baseline, all participants completed a self-administered questionnaire on personal factors and work exposure. A total of 1,886 subjects (1,124 men and 762 women) without TSP at baseline were eligible for analysis. Associations between incident TSP and risk factors at baseline were analyzed by multivariate logistic regression.
Results
The incidence rate of TSP was 5.2 (95% confidence interval [95% CI] 3.9–6.6) per 100 men and 10.0 (95% CI 7.8–12.1) per 100 women. TSP was often associated with low back pain and neck pain. TSP in men was associated with age (odds ratios [ORs] ranging from 2.6 [95% CI 0.95–7.1] at 30–39 years to 6.0 [95% CI 2.1–17.3] at ≥50 years), being tall (OR 2.2 [95% CI 1.2–3.9]), frequent/sustained trunk bending (OR 3.0 [95% CI 1.5–6.1]), lack of recovery period or change in the task (OR 2.0 [95% CI 1.2–3.6]), and driving vehicles (OR 2.8 [95% CI 1.4–5.5]). Being overweight or obese was associated with lower risk (OR 0.5 [95% CI 0.3–0.96]). TSP in women was associated with high perceived physical workload (OR 1.9 [95% CI 1.1–3.3]), after adjustment for confounding variables.
Conclusion
The risk model of TSP combined personal and work-related organizational and physical factors. Trunk bending appeared to be a strong independent predictor of TSP in this working population.

Abstract: We report the first case in France of a high-level azithromycin-resistant Neisseria gonorrhoeae (minimum inhibitory concentration (MIC) = 96 mg/L) assigned to MLST7363 (NG-MAST ST6360), also resistant to ciprofloxacin and tetracycline but susceptible to ceftriaxone. The patient was a 51 year-old heterosexual man who returned following 1g azithromycin monotherapy. Mechanisms of azithromycin resistance were a C2599T mutation in the four copies of the rrl gene and a novel mutation in the promoter of the mtrR gene.

Abstract: To develop a European surveillance protocol for Clostridium difficile infection (CDI), existing national CDI surveillance systems were assessed in 2011. A web-based electronic form was provided for all national coordinators of the European CDI Surveillance Network (ECDIS-Net). Of 35 national coordinators approached, 33 from 31 European countries replied. Surveillance of CDI was in place in 14 of the 31 countries, comprising 18 different nationwide systems. Three of 14 countries with CDI surveillance used public health notification of cases as the route of reporting, and in another three, reporting was limited to public health notification of cases of severe CDI. The CDI definitions published by the European Society of Clinical Microbiology and Infectious Diseases (ESCMID) and the European Centre for Disease Prevention and Control (ECDC) were widely used, but there were differing definitions to distinguish between community- and healthcare-associated cases. All CDI surveillance systems except one reported annual national CDI rates (calculated as number of cases per patient-days). Only four surveillance systems regularly integrated microbiological data (typing and susceptibility testing results). Surveillance methods varied considerably between countries, which emphasises the need for a harmonised European protocol to allow consistent monitoring of the CDI epidemiology at European level. The results of this survey were used to develop a harmonised EU-wide hospital-based CDI surveillance protocol.

Abstract: La promotion de la vaccination en France depuis 1983 a abouti a une reduction de 97% de la morbidite et de 60% de la mortalite de la rougeole. Cependant, le taux de couverture stagnant et sub-optimal a 84% entraine un deplacement de l’âge des cas dans des tranches ou les complications sont plus frequentes et severes. Ainsi, la proportion des plus de 10 ans est passee de 13% en 1985 a 48% en 1997, la transmission de la rougeole se maintenant en France. Pour eliminer la maladie, une couverture vaccinale a 2 doses et a plus de 95% serait necessaire.

Abstract: From January to September 2013, a marked increase in notifications of Salmonella Paratyphi A infections among travellers returning from Cambodia occurred in France. An investigation revealed 35 cases without a common source: 21 in France, five in Germany, three in the Netherlands, one in Norway, one in the United Kingdom, four in New-Zealand. Data suggest an ongoing event that should trigger further investigation. Travellers to Cambodia should observe preventive measures including good personal hygiene and food handling practices.
