Abstract: The presenting features and their subsequent evolution in 36 patients with pathologically proven or clinically probable corticobasal degeneration are described. The most common initial complaint was of a unilateral 'clumsy, stiff or jerky arm' (n = 20). Typically the arm was akinetic, rigid and apraxic. In about a third of these the arm was held in a striking and characteristic fixed dystonic posture. Jerking of the arm, due to action and stimulus-sensitive myoclonus accompanied these symptoms in about a third of the cases. The next most common presentation (n = 10) was difficulty walking due to clumsiness and loss of fine motor control of one leg due to apraxia or dysequilibrium, or a combination of both. Sensory symptoms in the affected arm heralded the onset of illness in three and accompanied a motor disturbance in two cases. Presentation with dysarthria or a behavioural syndrome were rare. The symptoms progressed slowly, usually involving first the ipsilateral arm and leg, but gradually spread to affect all four limbs. After a mean follow-up of 5.2 years (range 2-8 years) gait difficulties and a supranuclear ophthalmoplegia had emerged in most patients and dysarthria and pyramidal signs were common. Higher mental function was relatively preserved in most and a cortical sensory loss was evident in a quarter of cases.

Abstract: A novel approach to study electroweak physics at one-loop level in generic SU(2)L×U(1)Y theories is introduced. It separates the 1-loop corrections into two pieces: process specific ones from vertex and box contributions, and universal ones from contributions to the gauge boson propagators. The latter are parametrized in terms of four effective form factors $$\bar e^2 (q^2 ), \bar s^2 (q^2 ), \bar g_Z^2 (q^2 )$$ and $$\bar g_W^2 (q^2 )$$ corresponding to the γγ, γZ,ZZ andWW propagators. Under the assumption that only the Standard Model contributes to the process specific corrections, the magnitudes of the four form factors are determined atq 2=0 and atq 2=m 2 by fitting to all available precision experiments. These values are then compared systematically with predictions of SU(2)L×U(1)Y theories. In all fits α s (m Z ) and $$\bar \alpha (m_Z^2 )$$ are treated as external parameters in order to keep the interpretation as flexible as possible. The treatment of the electroweak data is presented in detail together with the relevant theoretical formulae used to interpret the data. No deviation from the Standard Model has been identified. Ranges of the top quark and Higgs boson masses are derived as functions of α s (m Z ) and $$\bar \alpha (m_Z^2 )$$ . Also discussed are consequences of the recent precision measurement of the left-right asymmetry at SLC as well as the impact of a top quark mass and an improvedW mass measurement.

Abstract: Verification method has been developed for determining the safety and operability of programmable logic controller (PLC) based systems. The method automatically checks sequential logic embedded in PLCs and provides counterexamples if it finds errors. The method consists of a system model, assertions, and a model checker. The model is a Boolean-based representation of a PLC's behavior. Assertions are questions about the behavior of the system, expressed in temporal logic. The model checker generates a state space based on the above two inputs, searches the space efficiently, determines the consistency of the model and assertions, and supplies counterexamples. A modeling technique has been developed to verify relay ladder logic (RLL), a PLC programming language. The performance of the model checker is studied in a series of alarm designs. >

Abstract: In this study, we investigate whether a tissue-level constraint can explain the general inverse relationship between growth rate and precocity of development in birds. On the whole, altricial (dependent) chicks grow three to four times faster than the less dependent, more able chicks of precocial species of similar adult mass. We suggest that an antagonism between growth and acquisition of mature function in skeletal muscle constrains postnatal growth and development in most species of birds. Altricial species, represented by European starlings in this study, hatch with skeletal muscle having low capacity for generating force but grow rapidly. Conversely, precocial species (northern bobwhite quail and Japanese quail), hatch with relatively mature skeletal muscle, especially in their legs, but grow more slowly. As development proceeds in all species, exponential growth rates decrease as muscles acquire adult levels of function. Among four variables associated with muscle function, exponential growth rate (EGR) was negatively correlated with pyruvate kinase activity (glycolysis), potassium concentration (electrical potential), and dry weight fraction (contractile proteins) in both pectoral and leg muscles but not with citrate synthase activity (aerobic metabolism) in either set of muscles. For pectoral muscle, these variables accounted for 87% of the total variation in EGR in all three species combined despite a twofold difference in growth rates between the starling and quail. EGRs of leg muscle (51% of variation accounted for) were less than predicted by the pectoral-muscle equation in quail during the early part of the postnatal period and in starlings during the late postnatal period. This result would not contradict a growth rate/maturity constraint hypothesis if EGRs were down-regulated for allometric or other consid- erations.

Abstract: The prognosis for patients who survive carbon monoxide (CO) poisoning is uncertain, particularly in those who develop persistent neurological complications after recovery from the initial coma. Thirty-one patients with the sequelae of CO poisoning, followed for a year, are described. Eight had a progressive course, and 23 had a delayed relapse after an initial recovery period of approximately 20 days (range, 1-36 days). Those with a progressive course developed a persistent akinetic-mute state, and four of the eight died. Those with the delayed relapsing course either developed a parkinsonian state with behavioral and cognitive impairment but could walk (nine cases), or progressed further to an akinetic-mute state, and were bed-bound (14 cases); the deterioration to either condition occurred rapidly over a few days to a week. Fourteen of the patients with the delayed relapses (61%) subsequently improved, but three (13%) died. Those with a progressive course without initial recovery were younger (mean age, 37.0 years) than those with a delayed relapsing course (55.2 years; p < 0.01). The mean duration of their initial coma (9.8 days) was longer than that in delayed relapsing cases (2.0 days; p < 0.01). The mean initial CO hemoglobin level was not different in the two groups. Brain computed tomography (CT) scans were obtained at the onset of sequelae in both groups. Ten patients had a normal CT scan, 13 had white matter low-density lesions, and four had globus pallidus low-density lesions.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract: We carried out a retrospective review of 75 children with unicameral bone cysts, who had sustained 52 pathological fractures, to determine which cysts were likely to be at risk and whether healing was accelerated after a fracture. Most lesions were in the metaphysis of a tubular bone, the commonest sites being the the upper part of the humerus and femur. The percentage of bone occupied by the cyst in the transverse plane was more than 85% in both anteroposterior and lateral radiographs in every case of pathological fracture. In most cases, the cyst recurred and sometimes became large without any acceleration of healing.

Abstract: This article presents a critical review of the statist perspective on East Asian political economy. The statist perspective is found lacking in its claims to explain economic performance, neglectful of intra-state dynamics, and inadequate in its depictions of state-society relations. Studies attempting to remedy these shortcomings through an application of concepts from network theory are also examined. These studies provide a conceptually richer description of state-society interaction. However, they fall short of explaining the essentially political nature of interaction, both between state and society and within the state. New directions for research are suggested. The key elements of the alternative approach are politics, institutions and leadership choice. These factors are widely acknowledged to be mutually influencing; however, the nature of their interrelations is only vaguely understood. A crucial task of research lies in explaining how politics, institutions and leadership choices interact to constitute lines of policy and to shape trajectories of economic development.

Abstract: Behcet's disease is a chronic, relapsing, idiopathic, multi-system condition characterized by mucocutaneous, ocular, gastrointestinal, articular, vascular, urogenital, and neurologic involvement. Usually the onset occurs between 20 and 30 years of age; it is very uncommon in children. We analyzed 40 cases of childhood-onset Behcet's disease. The male:female ratio was 0.67. The most frequent major sign was oral ulceration, appearing in all patients. Other major signs were genital ulcers (33 patients), skin lesions (29), and ocular lesions (11). The average time interval between the initial oral ulceration and the second major manifestation was 8.8 years. After the second major manifestation, the third and fourth features rapidly developed within one to two years. The most frequent minor sign was arthritis, occurring in 11 of 40 patients. Intestinal, neurologic, and renal involvement was also present. From our results, we concluded that oral ulceration, which is the most common initial manifestation, should not be neglected in children, since it may signal Behcet's disease.

Abstract: We have developed a self-creating and organizing unsupervised learning algorithm for artificial neural networks. In this study, we introduce SCONN and SCONN2 as two versions of self-creating and organizing neural network (SCONN) algorithms. SCONN creates an adaptive uniform vector quantizer (VQ), whereas SCONN2 creates an adaptive nonuniform VQ by neural-like architecture. SCONN's begin with only one output node, which has a sufficiently wide activation level, and the activation level decrease depending upon the time or the activation history. SCONN's decide automatically whether to adapt the weights of existing nodes or to create a new "son node." They are compared with two famous algorithms-the Kohonen's self organizing feature map (SOFM) (1988) as a neural VQ and the Linde-Buzo-Gray (LBG) algorithm (1980) as a traditional VQ. The results show that SCONN's have significant benefits over other algorithms. >

Abstract: Summary form only given. FT-IR and non-resonance FT-Raman spectra have been measured in three kinds of polyanilines (PANIs) chemically synthesized, i.e., leucoemeraldine base (LB), emeraldine base (EB) and pernigraniline base (PB) with different oxidation levels. In Raman spectra shown in Fig. 1, the peaks at about 1600 cm/sup -1/ and 1176 cm/sup -/ are attributed to benzenoid unit (B), while the peak at 1380 cm/sup -1/ are to benzenoid unit (Q). In IR spectra, we also observed several characteristic peaks in three PANIs, which are attributed to N-H stretching (3380 cm/sup -1/), C=C and C-C streching and bending modes (B : 1600, 1500, 1150 cm/sup -1/, Q : 1380, 1300 cm/sup -1/). The variations of oscillator strength at each IR and Raman peak among three PANIs are consistently interpreted in the viewpoint of the ratio of benzenoid and quinoid moieties.

Abstract: Augmentation of the body contour by localized injection of hard and soft paraffin rose to a zenith of popularity in the early 1900s, whereafter the severe destructive consequences of such injections became widely recognized. However in Korea, these injections are still performed much by nonmedical person. Paraffin or other mineral oil injection into body is no more a useful method to change body contour. We reviewed 26 cases of sclerosing lipogranuloma of penis with complications in recent 13 years after mean 18.5 months from previous injection of paraffin or vaseline. We conclude that the public should be informed of detrimental effects of paraffin injections and that the best treatment of penile paraffinoma is complete excision and appropriate penoplasty.

Abstract: Physical and microwave dielectric properties of complex perovskite Ba(Mg1/3Ta2/3)O3 ceramics have been investigated as a function of the amount of BaWO4 in the temperature range from 20° to 80°C at 10.5 GHz. Up to 0.05 mol BaWO4 addition, the lattice constant ratio (c/a), ordering parameter, apparent density, and unloaded Q all increase, due to the increase in the substitution of Ta5+ ions of Ba(Mg1/3Ta2/3)O3 by W6+ ions from the melted BaWO4 at above 1430°C. With further addition of BaWO4, the unloaded Q decreases, due to an increase of the BaWO4 phase. The temperature coefficient of resonant frequency (TCF) can be controlled by the volume mixture rule of Ba(Mg1/3Ta2/3)O3 and BaWO4. When 0.09 mol BaWO4 is added, TCF becomes 0 ppm/°C.

Abstract: BACKGROUND After induction of anesthesia, lung resistance increases. We hypothesized that prophylactic bronchodilator treatment before tracheal intubation would result in a lower lung resistance after placement of the endotracheal tube. METHODS Forty-two adult patients were randomized to receive one of three inhaled medications 1 h before surgery. All patients first underwent pulmonary function tests. Patients then received either inhaled albuterol (360 micrograms) (n = 12), inhaled ipratropium bromide (72 micrograms) (n = 15) or a placebo inhalation (n = 15). Two, 5, and 15 min after tracheal intubation, lung resistance was measured using the method of von Neergard and Wirtz. RESULTS Patients who received either bronchodilator had significantly lower lung resistance after intubation than those receiving placebo. At 2 min, lung resistances were 12.7 +/- 1.4 cmH2O.l-1.s-1 (mean +/- SEM) for the placebo group, 6.4 +/- 3.1 cmH2O.l-1.s-1 for the ipratropium-treated group (P < 0.05 vs. placebo), and 7.2 +/- 0.8 cmH2O.l-1.s-1 for the albuterol-treated group (P < 0.05 vs. placebo). The differences in lung resistance persisted through the final measurement at 15 min. Three of fifteen placebo-treated patients developed audible wheezing whereas no patients developed wheezing in either bronchodilator-treated group (P < 0.05 by Fisher's exact test). Although smokers and nonsmokers in the placebo group developed similar resistances after intubation, bronchodilator treatment resulted in lower resistance in nonsmokers than in smokers (P < 0.05). CONCLUSIONS Prophylactic treatment with either an inhaled beta 2-adrenergic agonist or an inhaled cholinergic antagonist produced lower lung resistance after intubation when compared with an inhaled placebo medication. The effect was more pronounced in nonsmokers than in smokers.

Abstract: A review was done on 631 patients with acute myocardial infarction who underwent coronary angiography within 30 days after onset of myocardial infarction at Yonsei University Severance Hospital from January, 1985, to August, 1993. The incidence of acute myocardial infarction in patients under 40 years of age was 10.3% (65/631). Acute myocardial infarction below the fourth decades was the predominant disease of men. Risk factor analysis revealed a history of cigarette smoking and hypercholesterolemia were more frequently found in the young patients, but a history of hypertension and diabetes were more frequently found in the elderly patients. Angiographically, the incidence of one vessel disease and normal or minimal lesion coronary anatomy were more frequent in the young patients and incidence of multi-vessel disease were more frequent in the elderly patients. Of the 65 patients under 40 with acute myocardial infarction, the patients with multi-vessel disease tended to have a history of diabetes mellitus in comparison with those with normal coronary anatomy or one vessel disease.

Abstract: PURPOSE To investigate whether MR imaging is an accurate means of assessing mandibular invasion in patients with carcinoma. METHODS We retrospectively studied the MR scans of 22 patients with pathologic or surgical confirmation of mandibular invasion from oral and oropharyngeal cancers. The MR images were blindly analyzed using primary criteria of: (a) cortical breakdown, (b) replacement of bone marrow fat, or (c) gadopentetate dimeglumine enhancement of a mass at the bone marrow defect. Secondary criteria of: (a) contiguous soft-tissue mass, and (b) mass on both sides of the mandibular cortex were also examined. Mandibular invasion was graded as periosteal/cortical, medullary, or no invasion. RESULTS Primary positive findings of cortical breakdown and abnormal bone marrow signal were highly sensitive (100%) for periosteal/cortical invasion and medullary involvement, respectively. However, a high rate of false-positive studies hampered the MR accuracy, which fell into the 73% to 77% range. A negative MR study was highly predictive, but a positive study was less valuable. Gadolinium enhancement added little to the MR study9s accuracy. False-positive studies mainly occurred in the setting of prior irradiation, osteoradionecrosis, and odontogenic infections. CONCLUSIONS MR imaging is a sensitive method for detecting mandibular invasion but has a low positive predictive value. A negative study virtually excludes cortical/periosteal or bone marrow invasion.

Abstract: The aim of this study is to evaluate the proliferating activity of ameloblastomas and its correlation to the biological behaviour according to each histological type. 38 cases of solid and unicystic ameloblastomas were reviewed including 1 case of ameloblastic carcinoma and 1 recurrent case. An anti-PCNA antibody, PC10(Dako), was applied for the detection of proliferating cell nuclear antigen (PCNA) in paraffin embedded tissue sections. Also 20 cases of dentigerous cysts were reviewed. In conclusion, we observed that there was no difference between the proliferating activities of the different histological types of solid ameloblastomas. Because 1 case of ameloblastic carcinoma and one recurrent case revealed remarkably high PCNA reactivity, we believe that PCNA would be useful in showing the differentiation between benign and malignant ameloblastomas. In unicystic ameloblastomas, plexiform intraluminal growth was considered to be an important feature in tumorous transformation of cystic epithelium.

Abstract: The authors studied 50 consecutive and nonselected patients (52 hips) who were followed for a minimum of 7 years (range, 84–89 months) after they had a primary total hip arthroplasty with an uncemented Anatomic Medullary Locking (DePuy, Warsaw, IN) hip system. The average age of the patients at operation was 47.6 years (range, 19–88 years). The operative diagnoses were: avascular necrosis of the femoral head in 18 hips (34%), osteoarthritis in 16 (31%), fracture of the femoral neck in 14 (27%), and miscellaneous in 4 (8%). The average preoperative Harris hip score was 59 points (range, 6–67 points) that improved to 91 points (range, 69–100 points) at the 7-year follow-up examination. To assess the adequacy of intramedullary fit, the fit of the stem at the proximal canal and isthmus level was evaluated. Forty-one hips (79%) had a good press-fit at both the proximal canal and isthmus level, five hips (10%) had a good press-fit at the proximal canal only, and the remaining six hips (11%) had a poor fit at both the proximal canal and isthmus level. Of the 46 hips that had a good press-fit at the proximal canal and/or isthmus level or proximal canal only, 32 (70%) had bone ingrowth and 14 (30%) had stable fibrous tissue ingrowth. Of the remaining six hips with a poor press-fit at both the proximal canal and isthmus level, three (6%) had stable fibrous tissue ingrowth and another three (6%) were unstable. Of the three hips (6%) with femoral component loosening, one was revised and the other two were asymptomatic. There was an alarmingly high incidence of perioprosthetic osteolysis in our series: 16 hips (31%) had acetabular and femoral osteolysis and 13 hips (25%) had femoral osteolysis only. Also, there was a strikingly high incidence of polyethylene-liner wear (12 hips or 23%). Although the incidence of component loosening was low, a high incidence of periprosthetic osteolysis and excessive wear in the polyethylene liner remain challenging problems after insertion of the cementless Anatomic Medullary Locking prosthesis.

Abstract: The authors propose a new method for the modeling and call admission control (CAC) of variable bit rate video source, which come to the front of ATM networks as hot issues nowadays. First, the modeling of video source is accomplished using the three-state Markov chains including the effects of scene change at which the bit rate of video source is abruptly increased. Also, using two AR models, they improve the defects which an AR model has in modeling a video source. In addition, they represent the analytical model of a video source so that a network manager can acquire the information which is very important in managing the entire networks. CAC is accomplished using the previously defined analytical model. A routing manager calculates the cell loss probability of a chosen VP where a new call is connected so that the routing manager decides whether this new call is accepted or not. This calculation is accomplished through the GB/D/1-S queuing system. Using BIA (bandwidth increasing algorithm), they check whether the calls rejected by the routing manager could be accepted if possible. Finally, the applicable procedures to suitable allocate bandwidth to each VP on a link are presented in detail. >

Abstract: Fluorite type HfO2 and ZrO2 solid solutions were prepared by doping with 8 to 14 mol% of Ho2O3 and Y2O3, and their lattice parameters were determined. In both HfO2 and ZrO2 systems, the lattice parameters of the solid solutions containing Ho203 were consistently greater than those containing the same amounts of Y2O3. This indicated that the ionic radius of Ho3+ was larger than that of Y3+ in the fluorite structure solid solutions. The effective ionic radius of Y3+ in eightfold coordination was estimated to be 0.1011 nm by using the measured lattice parameters and the empirical equations to predict the lattice parameters of the fluorite-type solid solutions.

Abstract: We encountered a massive retroperitoneal haemorrhage due to perforation of the external iliac vein by a drill tip passing through the antero-superior quadrant of the acetabulum during cementless total hip arthroplasty. An anatomical study of the acetabulum was undertaken to determine the safe zones for transacetabular screw fixation. Five cadavera were studied to find the proper locations for screws using acetabular quadrants. The postero-superior and postero-in-ferior acetabular quadrants have good bone stock and are safe for the transacetabular replacement of screws. The antero-superior and inferior quadrants should be avoided. Screws placed in these quadrants may damage the external iliac artery and vein or the obturator vessels.
