Abstract: INTRODUCTION The aim of this study was to assess patient satisfaction in the surgical emergencies department at the National Hospital of Zinder, Niger. MATERIALS AND METHOD This was a cross-sectional study focusing on patients admitted to the surgical emergency department of the National Hospital of Zinder (NHZ). The study was conducted over a period of 2 month - from July 1st to August 31st, 2015. During a face to face interview, patients were asked a series of questions. RESULTS Of the 227 patients surveyed, 54.62% (n=124) were satisfied with their experience of the surgical emergencies department. The main factors associated with high satisfaction scores were patient reception (OR: 0.27, 95% CI=0.14 to 0.52; p <0.001), and the management of symptoms (OR: 0.28, 95 % CI=from 0.14 to 0.58; p < 0.001). Dissatisfaction factors were: waiting time before receiving treatment (OR 16.57, 95% CI=8.52 to 32.23; P <0.0001), the environment (OR: 3.89, 95% CI=2.12 to 7.12; p <0.001), accessibility (OR: 5.85, 95% CI=2.54 to 13.46, p < 0.001) and poor staff-patient communication (OR, 13.76; 95% CI 6.54 to 28.98; P <0.0001). CONCLUSION Despite the shortcomings of the surgical emergencies department of NHZ, a good welcome and prompt patient management have been the key components associated with patient satisfaction.

Abstract: Rapporter les resultats du traitement des volvulus du colon sigmoide (VCS) a l’hopital national de Niamey et proposer une classification therapeutique. Il s’agissait d’une etude retrospective, descriptive et analytique sur dix ans incluant tous les adultes traites pour VCS a l’hopital national de Niamey. Une association entre la morbimortalite et les variables a ete recherchee, et les resultats obtenus ont ete analyses et interpretes en fonction des donnees de la litterature afin de proposer une classification therapeutique. La serie incluait 168 patients, soit 17,31 % des occlusions intestinales et 2,64 % des urgences abdominales. Il s’agissait de 166 hommes et de deux femmes. L’âge moyen des patients etait de 50,80 ans (±15,52). Le delai moyen de consultation etait de 4,18 jours (±2,85). La douleur abdominale etait le maitre symptome present chez tous les patients. Le diagnostic paraclinique etait essentiellement fait par une radiographie de l’abdomen sans preparation. Un noeud ileosigmoidien etait retrouve chez 4,76 % des patients. Le taux de necrose etait de 22,02 %. Une colectomie selon Hartmann etait realisee chez 68,45 % des patients, suivie d’une colectomie ideale (25,60 %). La morbidite globale etait de 14,28 %, et 7,15 % des patients etaient decedes dont 5,95 % apres colectomie selon Hartmann. La survenue de complications etait influencee par la necrose (p = 0,04) et le long delai de consultation (p = 0,04). La mortalite etait influencee par la colectomie selon Hartmann (p = 0,01). L’analyse de nos resultats a la lumiere de la litterature nous a permis de definir quatre groupes therapeutiques de patients. La determination de quatre groupes de patients pourrait aider a la gestion du VCS.

Abstract: Au Niger, lors des crises alimentaires, les graines de Boscia senegalensis, appele anza en langue local, constituent un des principaux aliments de cueillette de subsistance pour les menages ruraux. L’objectif de la presente etude etait de determiner la composition en acides amines des graines de Boscia issues de differentes methodes de desamerisation. La teneur en proteinestotaux et en 16 acides amines, dont les 8 acides amines essentiels, des graines non cuites et cuites ont ete determinees. Le resultat obtenu montre que la teneur en proteines varie de 16,11 a 22,82%. Les acides amines majeurs sont l’acide glutamique (2,32-3,33 %), acide aspartique (1,71-2,36 %) et la phenylalanine (1,34-2,19 %) tandis que la methionine, l’histidine, la threonine et la tyrosine sont les acides amines limitant dans toutes les graines. Des faibles teneurs sont aussi trouvees pour la lysine (0-1,31 %). L’acide amine essentiel majeur est la phenylalanine. Les essentiels mineurs sont la methionine et la threonine. De facon generale, les methodes de traitements traditionnelles influencent positivement la teneur en proteines et en certains acides amines essentiels des graines de Boscia senegalensis. In Niger, during food crises, Boscia senegalensis seeds, called Anza in the local language, are one of the main subsistence food crops for rural households. The objective of this study was to determine the amino acid composition of Boscia seeds from different deamerization methods. The protein and 16 amino acid content, including all 8 essential amino acids, of uncooked and cooked seeds were determined according to standard methods. The result obtained shows that the protein content varies from 16.11 to 22.82%. The major amino acids are glutamic acid (2.32-3.33%), aspartic acid (1.71-2.36%) and phenylalanine (1.34-2.19%) while methionine, histidine, threonine and tyrosine are the limiting amino acids in all seeds. Low levels are also found for lysine (0-1.31%). The major essential amino acid is phenylalanine and the minor essential amino acids are methionine and threonine (1.34-2.19%). In general, traditional processing methods positively influence the content of proteins and certain essential amino acids in Boscia senegalensis seeds.

Abstract: Neuromyelitis optica (Devic's disease) is an inflammatory demyelinating disease of the central nervous system that mainly affects spinal cord, optic nerve and brain regions with high aquaporin 4 antigen expression. This is a severe autoimmune disease caused by autoantibodies directed against aquaporin 4 and associated with high morbidity and mortality. Unlike other inflammatory conditions such as multiple sclerosis or rheumatoid polyarthritis, pregnancy does not seem to influence the activity of neuromyelitis optica, hence the need for a thorough treatment during pregnancy. Corticosteroid therapy is the treatment of choice for neuromyelitis optica during pregnancy. Other treatments may also be used including rituximab, some immunosuppressive agents and immunoglobulins. Immunosuppressive treatment or rituximab is recommended when the long-term corticosteroid treatment is contraindicated, in case of inefficiency or if side effects are intolerable. Immunoglobulins are administered to patients with serious outbreaks of neuromyelitis optica which do not respond to bolus methylprednisolone. Immunoglobulins alone can also be continued at a dose of 0.4 g/kg/day for 6-8 weeks until delivery. Plasmapheresis is also a good alternative to bolus methylprednisolone when outbreaks are extremely severe.
