Abstract: A 48-year-old White man with a history of congenital distal renal tubular acidosis and diabetes presented with low urine output for 6 months. He was diagnosed with distal renal tubular acidosis when he was 9 years old. He was not compliant to his alkali treatment. Labs were notable for a creatinine of 7 mg/dL, serum bicarbonate of 13 mEq/L with a positive urine anion gap and urine pH of 6. Plain radiographs of the abdomen and renal ultrasonograph are shown in Figs. 1 and 2, respectively. Nephrocalcinosis is a rare condition inwhich there is calcium deposited in the kidneys. It can be seen in a variety of congenital and acquired diseases. Diagnosis is oftenmade incidentally by imaging such as plain abdominal radiography, kidney ultrasonography and abdominal computed tomography. In general, nephrocalcinosis is recognized as systemic disease and appropriate evaluation is needed to clarify underlying disease. Differential diagnosis of nephrocalcinosis includes primary hyperparathyroidism, sarcoidosis, hypervitaminosis D, medullary sponge kidney, distal renal tubular acidosis and other inherited tubulopathies. Initial laboratory evaluation including urinalysis, urine electrolyte, urine anion gap, serum calcium and phosphate, parathyroid hormone level and serum vitamin D level should be obtained based on clinical suspicion.

Abstract: Introduction: Several chemotherapy agents and combinations have proven effective in the therapy of advanced enteropancreatic neuroendocrine tumors (EP-NETs). However, their toxicity can be significant. Recent understanding of the molecular mechanisms of these tumors, especially the central role of tumor angiogenesis, has led to the identification of new therapeutic targets and agents directed at the molecular level. Areas covered: This paper gives a comprehensive evaluation of the existing therapeutic armamentarium for EP-NETs. Narrated in a historical perspective, this review analyzes the available information on traditional chemotherapy agents, interferon-α and somatostatin analogs, as well as newer therapies and experimental agents. Abstract: Service learning is a form of experiential learning that cultivates academic development, personal growth, and civic engagement. Students contribute to and learn from community. Service learning empowers students, enabling them to recognize their ability to act as agents of social change. Service learning is gaining momentum as a movement, given its ability to prepare students for the “real world” after graduation. The authors of this article come from health sciences, psychology, and environment and sustainability. Here, we illustrate service learning through four case studies: 1) An innovative team-based service-learning course partnering with older adults, healthcare providers and community agencies (Gerontology in Practice, Western University); 2) A unique curriculum design that includes service learning and interdisciplinary graduate problem-based training and research focused on experimental education (Environmental Sustainability, University of Saskatchewan); 3) An international service learning course that combines intensive coursework and a 3-month placement with a non-profit, community-based organization in Africa (Psychology and Developing Societies, University of British Columbia); and 4) An extraordinary example of an institutional-level commitment to service learning involving 50 courses, 40 faculty, 100 community agencies, and 900 students per year (St. Francis Xavier University). Our goal is to inspire other educators to engage in the pursuit of excellence in higher education through service learning.

Abstract: Tongue lesions in the pediatric population are rare. The differential diagnosis of these lesions is broad, and rapid growth of the lesion is concerning for a neoplastic process. We present a rare case of a fungal lesion mimicking a neoplastic growth in a 22-month-old girl. She underwent complete excision successfully. Full evaluation for benign and malignant neoplasms was negative. Tissue culture demonstrated growth of a rare Candida species to be the cause of the lesion. Postoperatively, she continues to do well, without regrowth 6 months later. This case reinforces the role of tissue culture when histology fails to demonstrate a diagnosis and emphasizes the need for efficient communication between the pediatrician, otolaryngologist, and pathologist for timely excision.

Abstract: By removing the light emitting diode (LED) chips from a commercial compact calculator and reconnecting the LED chips by cable to the calculator, the author produced an inexpensive device to display numerical data on any ultrasound oscilloscope screen. The chip is clipped to the face of the oscilloscope and photographed as part of the Polaroid or 70mm scan. Eight digits and one symbol can be displayed showing date, patient number, decibel level and position of scan.
