Abstract: We evaluated the efficacy of the human papillomavirus (HPV)-16/18 AS04-adjuvanted vaccine in preventing HPV-related disease after surgery for cervical lesions in a post-hoc analysis of the PApilloma TRIal against Cancer In young Adults (PATRICIA; NCT00122681). Healthy women aged 15-25 years were randomized (1:1) to receive vaccine or control at months 0, 1 and 6 and followed for 4 years. Women were enrolled regardless of their baseline HPV DNA status, HPV-16/18 serostatus, or cytology, but excluded if they had previous or planned colposcopy. The primary and secondary endpoints of PATRICIA have been reported previously; the present post-hoc analysis evaluated efficacy in a subset of women who underwent an excisional procedure for cervical lesions after vaccination. The main outcome was the incidence of subsequent HPV-related cervical intraepithelial neoplasia grade 2 or greater (CIN2+) 60 days or more post-surgery. Other outcomes included the incidence of HPV-related CIN1+, and vulvar or vaginal intraepithelial neoplasia (VIN/VaIN) 60 days or more post-surgery. Of the total vaccinated cohort of 18,644 women (vaccine = 9,319; control = 9,325), 454 (vaccine = 190, control = 264) underwent an excisional procedure during the trial. Efficacy 60 days or more post-surgery for a first lesion, irrespective of HPV DNA results, was 88.2% (95% CI: 14.8, 99.7) against CIN2+ and 42.6% (-21.1, 74.1) against CIN1+. No VIN was reported and one woman in each group had VaIN2+ 60 days or more post-surgery. Women who undergo surgical therapy for cervical lesions after vaccination with the HPV-16/18 vaccine may continue to benefit from vaccination, with a reduced risk of developing subsequent CIN2+.

Abstract: With the increasing recognition that agricultural growth and development do not necessarily translate into improved nutrition outcomes, policymakers are increasingly grappling with how to design and implement agricultural policies and programs that can also achieve nutritional objectives. Agriculture has direct links to nutrition in that it provides a source of food and nutrients and a broad-based source of income, as well as directly influencing food prices. Gender roles mediate these linkages, particularly in relation to increased food availability and increased income. Thus, one possible pathway through which agricultural development could improve health and nutrition outcomes is by considering gender roles and gender equity in agriculture. Using household survey data from Nepal, we investigate the impact of women’s empowerment in agriculture and production diversity on dietary diversity and anthropometric outcomes of mothers and children. Production diversity is positively associated with mothers’ dietary diversity and body mass index. Production diversity is positively associated with dietary diversity for children under two and predicts weight-for-age (WAZ), weight-for-height (WHZ), and height-for-age (HAZ) z-scores of children over two years of age. Indicators of empowerment are significantly associated with maternal outcomes but have a variable effect on child outcomes. Women’s autonomy in production and hours worked improve maternal and children’s dietary diversity and child HAZ.

Abstract: Summary Two separate experiments were conducted to determine the dietary requirements of juvenile Asian sea bass Lates calcarifer Bloch for lysine and arginine. Fish (average initial weight: lysine experiment, 13.12 a 0.12 g; arginine experiment, 2.56a0.13 g) were given amino acid test diets for 12 weeks containing fish meal, zein, squid meal, and crystalline amino acids. Each set of isonitrogenous and isocaloric test diets contained graded levels of L-lysine or L-arginine. The feeding rate in the lysine experiment was at 4‐2.5% of the body weight day )1 , while in the arginine experiment it was at 10‐4% of the body weight day )1 . The fish (20 per tank, lysine experiment; 15 per tank, arginine experiment) were reared in 500-L fibreglass tanks with continuous flowthrough sea water at 27 ∞C and salinity of 31 ppt in the lysine experiment and at 29 ∞C and salinity of 29 ppt in the arginine experiment. The experiments were in a completely randomized design with two replicates per treatment. Survival was high in fish given adequate lysine or arginine. Mean percentage weight gains were significantly diAerent in fish fed varying levels of lysine or arginine. Fish fed high levels of L-arginine suAered high mortalities. No significant diAerences were obtained in the feed eAciency ratios (FER, g gain g )1 feed) of fish fed graded lysine, although the values tended to increase as the dietary lysine level was increased up to the requirement level. In contrast, in the arginine experiment, significant diAerences in FER of fish among treatments were obtained; the highest FER was observed in fish fed the diet containing an optimum arginine level. On the basis of the growth response, survival, and FER, the lysine and arginine requirements of juvenile Asian sea bass were estimated to be 20.6 g kg )1 dry diet (4.5% protein) and 18.2 g kg )1 dry diet (3.8% protein), respectively. These data will be useful in the further refinement of practical diet formulations for the Asian sea bass.

Abstract: The purification, characterization, and synthesis of conantokin-R (Con-R), an N -methyl-d-aspartate (NMDA) receptor peptide antagonist from the venom of Conus radiatus , are described. With the use of well defined animal seizure models, Con-R was found to possess an anticonvulsant profile superior to that of ifenprodil and dizocilpine (MK-801). With voltage-clamp recording of Xenopus oocytes expressing heteromeric NMDA receptors from cloned NR1 and NR2 subunit RNAs, Con-R exhibited the following order of preference for NR2 subunits: NR2B ≈ NR2A > NR2C ≫ NR2D. Con-R was without effect on oocytes expressing the α-amino-3-hydroxy-5-methylisoxazole-4-propionic acid (AMPA) receptor subunit GluR1 or the kainate receptor subunit GluR6. In mouse cortical neurons voltage-clamped at −60 mV, Con-R application produced a slowly developing block of inward currents evoked by 10 μM NMDA and 1 μM glycine (IC50 = 350 nM). At 3 μM, Con-R did not affect γ-aminobutyric acid- or kainate-evoked currents. Con-R prevented sound-induced tonic extension seizures in the Frings audiogenic seizure-susceptible mice at i.c.v. doses below toxic levels. It was also effective at nontoxic doses in CF#1 mice against tonic extension seizures induced by threshold (15 mA) and maximal (50 mA) stimulation, and it partially blocked clonic seizures induced by s.c. pentylenetetrazol. In contrast, MK-801 and ifenprodil were effective only at doses approaching (audiogenic seizures) or exceeding (electrical and pentylenetetrazol seizures) those required to produce significant behavioral impairment. These results indicate that the subtype selectivity and other properties of Con-R afford a distinct advantage over the noncompetitive NMDA antagonists MK-801 and ifenprodil. Con-R is a useful new pharmacological agent for differentiation between the anticonvulsant and toxic effects of NMDA antagonists.
