Abstract: Aim Whether arteries are affected in mitochondrial disorders (MIDs) was under debate for years but meanwhile there are strong indications that large and small arteries are primarily or secondarily affected in MIDs. Data synthesis When reviewing the literature for appropriate studies it turned out that vascular involvement in MIDs includes primary or secondary micro- or macroangiopathy of the cerebral, cervical, and retinal arteries, the aorta, the iliac arteries, the brachial arteries, or the muscular arteries. Arteriopathy in MIDs manifests as atherosclerosis, stenosis, occlusion, dissection, ectasia, aneurysm formation, or arteriovenous malformation. Direct evidence for primary cerebral microangiopathy comes from histological studies and indirect evidence from imaging and perfusion studies of the brain. Microangiopathy of the retina is highly prevalent in Leber's hereditary optic neuropathy. Macroangiopathy of the carotid arteries may be complicated by stroke. Arteriopathy of the aorta may result in ectasia, aneurysm formation, or even rupture. Further evidence for arteriopathy in MIDs comes from the frequent association of migraine with MIDs and the occurrence of premature atherosclerosis in MID patients without classical risk factors. Conclusions Mitochondrial arteriopathy most frequently concerns the cerebral arteries and may result from the underlying metabolic defect or secondary from associated vascular risk factors. Vascular involvement in MIDs has a strong impact on the prognosis and outcome of these patients.

Abstract: A computerized Decision Support Systems (DSS) can improve the adherence of the clinicians to clinical guidelines and protocols The building of a prescriptive DSS based on breast cancer treatment protocols and its integration with a legacy Electronic Patient Record is the aim of the Oncocure project An important task of this project is the encoding of the protocols in computer-executable form -- a task that requires the collaboration of physicians and computer scientists in a distributed environment In this paper, we describe our project and how semantic wiki technology was used for the encoding task Semantic wiki technology features great flexibility, allowing to mix unstructured information and semantic annotations, and to automatically generate the final model with minimal adaptation cost These features render semantic wikis natural candidates for small to medium scale modeling tasks, where the adaptation and training effort of bigger systems cannot be justified This approach is not constrained to a specific protocol modeling language, but can be used as a collaborative tool for other languages When implemented, our DSS is expected to reduce the cost of care while improving the adherence to the guideline and the quality of the documentation

Abstract: In the recent years, there has been an increasing interest in the potential of internet- and smartphone-based technologies for the support of tinnitus patients. A broad spectrum of relevant approaches, some in the form of studies, others in the form of market products, have been mentioned in literature. They include auditory treatments, internet-based Cognitive Behavioral Therapy (iCBT), serious games, and questionnaires for tinnitus monitoring. The goal of this study is to highlight the role of existing internet-based and smart technologies for the advancement of tinnitus clinical practice: we consider contributions that refer to treatments and diagnostics, and we include contributions refering to self-help measures. We elaborate on the potential and challenges of such solutions and identify constraints associated to their deployment, such as the demand for familiarity with internet-based services and the need to re-design interactive services so that they fit on the small surface of a smartwatch.

Abstract: Various clinical procedures, such as cardiovascular surgery or extracorporeal blood purification, involve systemic anticoagulation using heparin. High concentrations of circulating heparin require neutralization due to possible serious bleeding complications. The intravenous administration of the heparin antagonist protamine sulfate is routinely clinically performed, but is frequently associated with adverse reactions. Therefore, there is a need for a valid and safe alternative to achieve extracorporeal heparin removal from blood or plasma, such as a filter, a matrix, or an adsorbent. Here, we describe the development of a macroporous poly(2-hydroxyethyl methacrylate)-based monolithic cryogel functionalized with l-lysine (pHEMA-lys) and the characterization of its selective heparin adsorption. The maximum binding capacity was quantified in vitro using aqueous and serum solutions under static and dynamic conditions, and fresh human plasma under static conditions. The pHEMA-lys bound 40,500 IU and 32,500 IU heparin/g cryogel at the equilibrium in aqueous solution and 50% serum, respectively. In human plasma spiked with 100 IU/mL of heparin, the binding was still highly efficient (4330 IU/g cryogel after 30 min, i.e., 87% of the initial concentration). The cryogels showed good blood compatibility, as indicated by negligible adsorption of albumin, antithrombin III, and total protein, and may thus be suitable for extracorporeal heparin removal.
