for hypertension prevention and management. The following are the key messages(1) In persons older than 50 years, systolic blood pressure (BP) of

lifestyle modifications to prevent CVD; (4) Thiazide-type diuretics should
be used in drug treatment for most patients with uncomplicated hypertension,
either alone or combined with drugs from other classes. Certain high-risk
conditions are compelling indications for the initial use of other antihypertensive
drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor
blockers, β-blockers, calcium channel blockers); (5) Most patients with
hypertension will require 2 or more antihypertensive medications to achieve
goal BP (<140/90 mm Hg, or <130/80 mm Hg for patients with diabetes
or chronic kidney disease); (6) If BP is more than 20/10 mm Hg above goal
BP, consideration should be given to initiating therapy with 2 agents, 1 of
which usually should be a thiazide-type diuretic; and (7) The most effective
therapy prescribed by the most careful clinician will control hypertension
only if patients are motivated. Motivation improves when patients have positive
experiences with and trust in the clinician. Empathy builds trust and is a
potent motivator. Finally, in presenting these guidelines, the committee recognizes
that the responsible physician's judgment remains paramount.

The National High Blood Pressure Education Program presents the complete Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Like its predecessors, the purpose is to provide an evidence-based approach to the prevention and management of hypertension. Introduction: The Losartan Intervention for Endpoint Reduction in Hypertension (LIFE) study was designed to compare the effects of losartan-based therapy with atenolol-based therapy on major clinical end points in 9193 patients with untreated or treated hypertension and left ventricular hypertrophy. Treatment with losartan reduced risk of the primary composite end point (cardiovascular death, stroke, and myocardial infarction) by 13%; stroke was reduced by 25%. Reduction in offi ce systolic blood pressure was similar with the two regimens.Aims: The present substudy was undertaken to assess the effect of losartan- and atenolol-based treatment on 24-hour ambulatory blood pressure monitoring (ABPM) and heart rate burden in LIFE patients at baseline and after 1 year of randomized treatment.Methods: The substudy included 110 patients recruited from centers in Denmark. Patients were randomized to 50 mg of either losartan or atenolol. To achieve blood pressure control, doses of either drug could be increased to 100 mg and hydrochlorothiazide and other agents could be added as necessary. ABPM was done at baseline and after 1 year of treatment.Results: Baseline characteristics, including office and daytime and nighttime ambulatory blood pressure and heart rate did not differ significantly between groups. Minor differences between groups in hourly average blood pressure were observed at baseline, especially during the night and early morning. Similar reductions in office and mean 24-hour ambulatory blood pressure were observed in the two groups from baseline to 1 year. No significant differences in daytime or nighttime blood pressure were observed between groups at 1 year. Early morning surges in systolic and diastolic blood pressure were similar in both groups. Nondipping status was more frequent in the losartan than the atenolol group at 1 year.Discussion: The comparable blood pressures achieved with losartan and atenolol determined by 24-hour ABPM provide compelling evidence that the difference in outcome seen in the LIFE study is due to differences in treatment regimens that are independent of blood pressure control. The early morning surge in blood pressure, which is associated with increased stroke risk, was similar in both groups at 1 year of treatment; therefore, this could not offer an explanation for the LIFE outcome. Paradoxically, nondipping status, which is generally associated with worse outcome, was more frequent in the losartan group.

Beyond brachial pressure effect.

Inflammation that develops with the release of chemokines and cytokines during acute kidney injury (AKI) has been shown to participate in functional renal recovery. While a major research focus has been on the role of macrophages, the family of CXC motif chemokines that promote neutrophil adherence and activation also increase with kidney ischemia/reperfusion (I/R) injury. This study tested the hypothesis that intravenous delivery of endothelial cells that overexpress CXCR1 and CXCR2 improves outcomes in kidney I/R injury. Overexpressing CXCR1/2 enhanced homing of ECs to I/R-injured kidneys and limited interstitial fibrosis, capillary rarefaction and tissue injury biomarkers (serum creatinine concentration and urinary KIM-1) following AKI and also reduced expression of P-selectin, the rodent CXC motif chemokine, CINC-2β, and the number of MPO-positive cells in the post-ischemic kidney. The serum chemokine/cytokine profile, including CINC-1, showed similar reductions. These findings were not observed in rats given endothelial cells transduced with empty adenoviral vector (Null-EC) or vehicle alone. These data indicate that extrarenal endothelial cells that overexpress CXCR1 and CXCR2, but not Null-EC or vehicle alone, reduce I/R kidney injury and preserve kidney function in a rat model of AKI.

Endothelial Cells Overexpressing CXCR1/2 are Renoprotective in Rats with Acute Kidney Injury.

Abstract Aims To ascertain whether depressive symptoms and cognitive impairment (CI) are associated with mortality among patients with heart failure (HF), adjusting for sociodemographic, comorbidities, and biomarkers. Methods and results We utilized Medicare-linked data from the Reasons for Geographic and Racial Differences in Stroke (REGARDS) Study, a biracial prospective ongoing cohort of 30 239 US community-dwelling adults, recruited in 2003–07. HF diagnosis was ascertained in claims analysis. Depressive symptoms were defined as a score ≥4 on the four-item Center for Epidemiological Studies-Depression scale. Cognitive impairment was defined as a score of ≤4 on the six-item screener that assessed three-item recall and orientation to year, month, and day of the week. Sequentially adjusted Cox proportional hazard models were used to estimate the risk of death. We analyzed 1059 REGARDS participants (mean age 73, 48%—African American) with HF; of those 146 (14%) reported depressive symptoms, 136 (13%) had CI and 31 (3%) had both. Over the median follow-up of 6.8 years (interquartile range, 3.4–10.3), 785 (74%) died. In the socio-demographics-adjusted model, CI was significantly associated with increased mortality, hazard ratio 1.24 (95% confidence interval 1.01–1.52), compared with persons with neither depressive symptoms nor CI, but this association was attenuated after further adjustment. Neither depressive symptoms alone nor their comorbidity with CI was associated with mortality. Risk factors of all-cause mortality included: low income, comorbidities, smoking, physical inactivity, and severity of HF. Conclusion Depressive symptoms, CI, or their comorbidity was not associated with mortality in HF in this study. Treatment of HF in elderly needs to be tailored to cognitive status and includes focus on medical comorbidities.
