Collateral blood vessels in acute ischaemic stroke: a potential therapeutic target

Imaging of the brain and vessels has shown that collateral flow can sustain brain tissue for hours after the occlusion of major arteries to the brain, and the augmentation or maintenance of collateral flow is therefore a potential therapeutic target.Abstract:
Summary Ischaemic stroke results from acute arterial occlusion leading to focal hypoperfusion. Thrombolysis is the only proven treatment. Advanced neuroimaging techniques allow a detailed assessment of the cerebral circulation in patients with acute stroke, and provide information about the status of collateral vessels and collateral blood flow, which could attenuate the effects of arterial occlusion. Imaging of the brain and vessels has shown that collateral flow can sustain brain tissue for hours after the occlusion of major arteries to the brain, and the augmentation or maintenance of collateral flow is therefore a potential therapeutic target. Several interventions that might augment collateral blood flow are being investigated.read more

An increase in AMPK/e-NOS signaling and attenuation of MMP-9 may contribute to remote ischemic perconditioning associated neuroprotection in rat model of focal ischemia.

TL;DR: RIPerC resulted in reduction of infarction volume, decreased apoptotic cell death and attenuated MMP-9 activity, which together with the increased activity of p-AMPK and increase in p-eNOS may, in part explain the neuroprotection and sustained increase in blood flow observed with RIPerC following acute stroke.

Brouwer,Sandra Cornelissen +17 more
TL;DR: The study shows that the presence of anterior and posterior communicating arteries is significantly associated with good functional outcomes and reduced mortality, respectively, independent of the leptomeningeal circulation status.

Asymmetry of medullary veins on multiphase CT-angiography in patients with acute ischemic stroke.
TL;DR: This study shows that medullary veins can be reliably assessed on multiphase CTA, and in patients with AIS, asymmetric appearance of MV is associated with poor clinical outcome.

TL;DR: It is demonstrated that the rs1870377 in the hypoxia-induced VEGFA/VEGFR2 axis predicts the 3-month outcome of patients with LAA stroke and that A allele was a protective factor for stroke outcome in both validation set and combined set.

Triiodothyronine levels were positively correlated with opening of collateral circulation in cerebral infarction patients with large artery atherosclerosis.
Xiaofeng Dong,Fan-Zhen Kong,Ming-Qiang Shen,Jiang Huang,Zong-En Gao,Qian-Zhu Guo,Zhong Zhao,Weifeng Luo,Qing-Zhang Cheng,Guan-Hui Wu +9 more
TL;DR: Poor opening of collateral circulation was likely to mediate the prediction of NTIS for prognosis of LAA-CI patients, as well as positively correlated with but advanced age.

Thresholds in cerebral ischemia - the ischemic penumbra.
TL;DR: Recent evidence indicates that immediate failure of basic functions such as synaptic transmission, ion pumping and energy metabolism in the ischemic brain, is critically dependent on residual blood flow, and that these functions fail at certain critical flow thresholds.
