Collateral blood vessels in acute ischaemic stroke: a potential therapeutic target

Imaging of the brain and vessels has shown that collateral flow can sustain brain tissue for hours after the occlusion of major arteries to the brain, and the augmentation or maintenance of collateral flow is therefore a potential therapeutic target.Abstract:
Summary Ischaemic stroke results from acute arterial occlusion leading to focal hypoperfusion. Thrombolysis is the only proven treatment. Advanced neuroimaging techniques allow a detailed assessment of the cerebral circulation in patients with acute stroke, and provide information about the status of collateral vessels and collateral blood flow, which could attenuate the effects of arterial occlusion. Imaging of the brain and vessels has shown that collateral flow can sustain brain tissue for hours after the occlusion of major arteries to the brain, and the augmentation or maintenance of collateral flow is therefore a potential therapeutic target. Several interventions that might augment collateral blood flow are being investigated.read more

TL;DR: In this paper, the role of supine positioning after acute stroke in improving cerebral blood flow and the countervailing risk of aspiration pneumonia have led to variation in head positioning in clinical practice.

Timing-Invariant Imaging of Collateral Vessels in Acute Ischemic Stroke

TL;DR: Collateral vessels may not always be visible on standard single time-frame CTA because of delayed contrast arrival, so future prognostic studies in acute stroke should consider delay-insensitive techniques, such as TI-CTA, instead of standard singletime-frame imaging, such as standard CTA.

TL;DR: The short-term safety and efficacy of endovascular stenting for patients with severe symptomatic intracranial atherosclerotic stenosis in China is acceptable, and balloon-mounted stent may have lower degree of residual stenosis than self-expanding stent.

TL;DR: In AIS patients, MFV increased significantly in the side affected by the stroke but not in the unaffected side when they were positioned in a lying flatHead Position at 0 or 15° compared to an upright head position at 30°.

Thresholds in cerebral ischemia - the ischemic penumbra.
TL;DR: Recent evidence indicates that immediate failure of basic functions such as synaptic transmission, ion pumping and energy metabolism in the ischemic brain, is critically dependent on residual blood flow, and that these functions fail at certain critical flow thresholds.
