Abstract: Objectives/Hypothesis: To identify upper airway and craniofacial abnormalities is the principal goal of clinical examination in patients with obstructive sleep apnea-hypopnea syndrome. The aim was to identify anatomical abnormalities that could be seen during a simple physical examination and determine their correlation with apnea-hypopnea index (AHI). Study Design: Consecutive patients with obstructive sleep apnea-hypopnea syndrome who were evaluated in a public otorhinolaryngology center were studied. Methods: Adult patients evaluated previously with polysomnography met the inclusion criteria. All subjects underwent clinical history and otolaryngological examination and filled out a sleepiness scale. Physical examination included evaluation of pharyngeal soft tissue, facial skeletal development, and anterior rhinoscopy. Results: Two hundred twenty-three patients (142 men and 81 women) were included (mean age, 48 ± 12 y; body mass index, 29 ± 5 kg/m 2 ; AHI, 23.8 ± 24.8 events per hour). Patients were distributed into two groups according to the AHI: snorers (18.4%) and patients with sleep apnea (81.7%). Sleepiness and nasal obstruction were reported by approximately half of patients, but the most common complaint was snoring. There was a statistically significant correlation between AHI and body mass index (P <.000), modified Mallampati classification (P = .002), and ogivale-palate (P <.001). The retrognathia was not correlated to AHI, but the presence of this anatomical alteration was much more frequent in patients with severe apnea when compared with the snorers (P =.05). Other correlations with AHI were performed considering multiple factors divided into two groups of anatomical abnormalities: pharyngeal (three or more) and craniofacial (two or more) abnormalities. There was a statistically significant correlation between pharyngeal landmarks and AHI (correlation coefficient [r] = 0.147, P =.027), but not between craniofacial landmarks and AHI. The combination of pharyngeal anatomical abnormalities, modified Mallampati classification, and body mass index were also predictive of apnea severity. Conclusions: Systematic physical examination that was used in the present study indicated that, in combination, body mass index, modified Mallampati classification, and pharyngeal anatomical abnormalities are related to both presence and severity of obstructive sleep apnea-hypopnea syndrome. Hypertrophied tonsils were observed in only a small portion of the patients. The frequency of symptoms of nasal obstruction was high in sleep apnea patients. Further studies are needed to find the best combination of anatomical and other clinical landmarks that are related to obstructive sleep apnea.

Abstract: We present a dedicated complementarity study of gravitational wave and collider measurements of the simplest extension of the Higgs sector: the singlet scalar augmented Standard Model. We study the following issues: (i) the electroweak phase transition patterns admitted by the model, and the proportion of parameter space for each pattern; (ii) the regions of parameter space that give detectable gravitational waves at future space-based detectors; and (iii) the current and future collider measurements of di-Higgs production, as well as searches for a heavy weak diboson resonance, and how these searches interplay with regions of parameter space that exhibit strong gravitational wave signals. We carefully investigate the behavior of the normalized energy released during the phase transition as a function of the model parameters, address subtle issues pertaining to the bubble wall velocity, and provide a description of different fluid velocity profiles. On the collider side, we identify the subset of points that are most promising in terms of di-Higgs and weak diboson production studies while also giving detectable signals at LISA, setting the stage for future benchmark points that can be used by both communities.

Abstract: Objectives: To evaluate the reliability and validity of the Brazilian version of the Pediatric Quality of Life InventoryTM 4.0. The instrument was administered by interviews in two groups: 240 apparently healthy children and adolescents from Sao Paulo (SP, Brazil) and 105 patients with chronic rheumatic diseases matched by age, as well as their respective parents or caregivers. The parent proxy-report was administered to the children's parents or caregivers separately on the same day. Results: Cronbach's alpha values were between 0.6 and 0.9 for all dimensions, demonstrating adequate internal consistency. Patients with rheumatic diseases reported significantly lower PedsQL TM scores on all dimensions when compared to the healthy control group (p < 0.0001). Construct validity of the Brazilian Portuguese version of the PedsQL TM 4.0 was also confirmed. Parent proxy-report of patients with rheumatic diseases highly correlated with child self-report for physical functioning (r = 0.77, p < 0.001) and school functioning (r = 0.73, p < 0.001). Lower correlations were observed for emotional and social functioning (r = 0.40 and 0.59, respectively, p < 0.001). Conclusions: The tool demonstrated reliability, validity, and the administration was fast and easy. Quality of life in patients with rheumatic diseases was significantly lower than in the healthy control group, supporting the necessity of a comprehensive approach to rheumatic disease management, focused on the psychosocial dimensions.

Abstract: There is a fair amount of epidemiological evidence showing that nutritional stunting causes increased risks of obesity. Obesity is increasing dramatically not only in developed countries but also in developing countries, such as Brazil, especially among the poorer. The mere coexistence of undernutrition and obesity among poor people has a great impact, as the burden in the social, economic, and health care systems is remarkable. In addition, an increasing number of studies have shown that nutritional stunting causes a series of important long-lasting changes such as lower energy expenditure, higher susceptibility to the effects of high-fat diets, lower fat oxidation, and impaired regulation of food intake. These findings suggest that a broader and more detailed understanding of the long-lasting effects of early undernutrition, direct cause of nutritional stunting, is needed. Within this context, we present data of some physiological mechanisms that substantiate the association between previous undernutrition and future obesity.

Abstract: Attention deficit hyperactivity disorder (ADHD) is related to a deficiency of central catecholamines (CA) in cognitive, biochemical, and physical tests, and pharmaceutical intervention may have no effect if it is not accompanied by changes in the environment The objective of our study was to test the hypothesis that central CA are responsible for the increase in speed reaction seen after physical activity (PA) and to measure the impact of high intensity PA on the sustained attention of 25 children diagnosed with ADHD consistent with the Disease Statistical Mental-IV (DSM-IV) criteria It is possible that practicing sports assists in the management of the disorder The children were divided between users (US) and non-users (NUS) of methylphenidate (MTP), and the groups were compared to evaluate the effect of the drug on cognition after PA Post-exercise performance on Conner’s Continuous Performance Test-II (CPT) was not affected by MTP, we observed significant improvements in response time, and we saw normalization in the impulsivity and vigilance measures These results suggest that the improvements in cognition after physical effort are not CA dependent Additionally, our results suggest that children’s attention deficits can be minimized through PA irrespective of treatment with MTP Additional studies are necessary to confirm that exercise mitigates the harmful symptoms of ADHD
