Abstract: Several inflammatory diseases such as rheumatoid arthritis, systemic lupus erythematosus, inflammatory bowel disease, celiac disease, cystic fibrosis and chronic obstructive pulmonary disease have been associated to bone resorption. The link between osteoclast, macrophage colony stimulating factor and pro-inflammatory cytokines, especially tumor necrosis factor-alpha and interleukin-1 explain the association between inflammation and osteoporosis. These diseases are related to osteoporosis and high fracture risk independent of other risk factors common to inflammatory diseases such as reduced physical activity, poor nutritional status, hypovitaminosis D, decrease in calcium intake and glucocorticoid treatment. Erythrocyte sedimentation rate and C-reactive protein should always be performed, but the indication about when to perform the densitometry test should be analyzed for each disease. Bisphosphonates are nowadays the best choice of therapy but new medications such as denosumab, IL-1 receptor antagonist, and TNF-alpha antibody have risen as new potential treatments for osteoporosis secondary to inflammation.

Abstract: A systematic study of the factorization of long-range azimuthal two-particle correlations into a product of single-particle anisotropies is presented as a function of pT and nu of both particles and as a function of the particle multiplicity in PbPb and pPb collisions. The data were taken with the CMS detector for PbPb collisions at root sNN = 2.76 TeV and pPb collisions at root sNN = 5.02 TeV, covering a very wide range of multiplicity. Factorization is observed to be broken as a function of both particle pT and nu. When measured with particles of different pT, the magnitude of the factorization breakdown for the second Fourier harmonic reaches 20% for very central PbPb collisions but decreases rapidly as the multiplicity decreases. The data are consistent with viscous hydrodynamic predictions, which suggest that the effect of factorization breaking is mainly sensitive to the initial-state conditions rather than to the transport properties (e.g., shear viscosity) of the medium. The factorization breakdown is also computed with particles of different nu. The effect is found to be weakest for mid-central PbPb events but becomes larger for more central or peripheral PbPb collisions, and also for very-high-multiplicity pPb collisions. The nu-dependent factorization data provide new insights to the longitudinal evolution of the medium formed in heavy ion collisions.

Abstract: Resumo O artigo aborda o desenvolvimento de um modelo de atencao a saude do idoso, buscando colaborar com a discussao sobre o envelhecimento populacional trazida pela nova realidade epidemiologica e demografica. Considerando que o processo de envelhecimento no Brasil e relativamente recente, foram descritos movimentos sociais mais relevantes na construcao das politicas de saude voltadas ao idoso. Apos a fase descritiva dos marcos, apresentou-se o modelo considerado mais adequado ao melhor cuidado. A partir de uma analise critica sobre os modelos de atencao a saude para idosos, o artigo apresenta uma proposta de linha do cuidado para esse segmento, tendo como foco a promocao e a prevencao da saude, de modo a evitar a sobrecarga do sistema de saude. Os modelos de cuidados integrados visam resolver o problema dos cuidados fragmentados e mal coordenados nos sistemas de saude atuais. Quanto mais o profissional conhecer o historico do seu paciente, melhores serao os resultados; assim devem funcionar os modelos contemporâneos e resolutivos de cuidado recomendados pelos mais importantes organismos nacionais e internacionais de saude. Um modelo de cuidado de maior qualidade, mais resolutivo e com melhor relacao custo-efetividade e a preocupacao deste texto.

Abstract: In this case-control study, the authors analyzed associations of uterine leiomyoma with atherogenic risk factors and potential sources of uterine irritation. The study included 318 case women with uterine leiomyoma that was first confirmed between 1990 and 1993 in the Baltimore, Maryland, area and 394 controls selected from women visiting the same gynecologists' offices for routine reasons. Telephone interviews were conducted with 77.8% of eligible cases and 78.0% of eligible controls. Compared with participants with no hypertension history, increased risks were observed among participants with any history of hypertension (odds ratio (OR) = 1.7; 95% confidence interval (CI): 1.0, 2.8), hypertension requiring medication (OR = 2.1; 95% CI: 1.1, 4.1), hypertension diagnosed at ages less than 35 years (for hypertension requiring medication, OR = 2.7; 95% CI: 1.0, 7.6), and hypertension of 5 or more years' duration (for hypertension requiring medication, OR = 3.1; 95% CI: 1.2, 8.2). Estimates of associations with diabetes history were very imprecise but followed similar patterns. Adjusted associations were observed with pelvic inflammatory disease (three or more episodes vs. none: OR = 3.7; 95% CI: 0.9, 15.9), chlamydial infection (history vs. no history: OR = 3.2; 95% CI: 0.8, 13.7), and use of an intrauterine device when it caused infectious complications (use vs. no use: OR = 5.3; 95% CI: 1.8, 16.3). Risk of uterine leiomyoma was also associated in a graded fashion with frequency of perineal talc use (daily use vs. no use: OR = 2.2; 95% CI: 1.4, 3.1). The authors conclude that nonhormonal factors may influence risk of uterine leiomyoma.
