Abstract: Haemangiopericytomas of central nervous system (CNS) were first defined as a separate entity in 1942. Previously they were either considered to be a histological variant of an angioblastic meningioma or a distinctive mesenchymal neoplasm. Most commonly they are located in parasagittal and falcine region. Tumours in the sellar/parasellar location are very rare and commonly escape diagnosis before operation. They are characterised by high vascularity, a high rate of local recurrence and extraneuronal metastasis. We report a 35-year-old man with a suprasellar hemangiopericytoma who presented with bilateral diminution of vision in both eyes and frontal headache. Six months after the first operation, he developed a large local recurrence. He again underwent tumour decompression followed by postoperative conformal radiotherapy and is currently asymptomatic and stable clinically and radiologically. The various differential diagnoses, the importance of a preoperative suspicion of this diagnosis and management are issues discussed in this illustrated review.

Abstract: Context: Literature suggests glottic view is better with straight blades while tracheal intubation is easier with curved blades. Aims: To compare glottic view and ease of intubation with Macintosh, Miller, McCoy blades and the Trueview® laryngoscope. Settings and Design: This prospective randomised study was undertaken in operation theatres of a 550 bedded tertiary referral cancer centre after approval from the Institutional Review Board. Methods: We compared the Macintosh, Miller, McCoy blades and the Trueview® laryngoscope for glottic visualisation and ease of tracheal intubation; in 120 patients undergoing elective cancer surgery; randomly divided into four groups. After induction of anaesthesia laryngoscopy was performed and trachea intubated. We recorded: Visualisation of glottis (Cormack Lehane grade), ease of intubation, number of attempts; need to change the blade and need for external laryngeal manipulation. Statistical Analysis: Demographic data, Mallampati classification were compared using the Chi-square test. A P<0.05 was considered significant. Results: Grade 1 view was obtained most often (87% patients) with Trueview® laryngoscope. Intubation was easier (Grade 1) with Trueview® and McCoy blades (93% each). Seven patients needed two attempts; one patient in Miller group needed three attempts. No patient in McCoy and Trueview® Groups required external laryngeal manipulation. Conclusions: We found that in patients with normal airway glottis was best visualised with Miller blade and Trueview® laryngoscope however, the trachea was more easily intubated with McCoy and Macintosh blades and Trueview® laryngoscope.

Abstract: This study evaluated the clinicopathological and prognostic implications of genetic alterations characterizing oral squamous cell carcinoma (OSCC). Comparative genomic hybridization was used to identify chromosomal alterations present in primary OSCCs obtained from 97 patients. In this population, tobacco use was a significant risk factor for OSCC. By contrast, the 97 samples were negative for human papillomavirus (HPV) DNA integration, another known risk factor for OSCC in certain populations. Results of the Fisher’s exact test, followed by the Benjamini-Hochberg correction for multiple testing, showed a correlation of 7p gain and 8p loss with node-positive OSCC (p≤0.04 for both genetic alterations) and an association of 11q13 gain with high-grade OSCC (p≤0.05). Univariate Cox-proportional hazard models, also corrected for multiple testing, showed a significant association of 11q13 gain and 18q loss with decreased survival (p≤0.05). The findings were supported by multivariate analysis, which revealed that 11q13 gain and 18q loss together serve as a strong bivariate predictor of poor prognosis. In conclusion, our study identified genetic alterations that correlate significantly with nodal status, grade and the poor survival status of OSCC. These potential biomarkers may aid the current tumor node metastasis system for better prediction of clinical outcome.
