Abstract: The objective was to develop evidence -based recommendations and a research and educational agenda for the non-pharmacological management of hip and knee osteoarthritis (OA). The multidisciplinary task force comprised 21 experts: nurses, occupational therapists, physiotherapists, rheumatologists, orthopaedic surgeons, general practitioner, psychologist, dietician, clinical epidemiologist and patient representatives. After a preliminary literature review, a first task force meeting and five Delphi rounds, provisional recommendations were formulated in order to perform a systematic review. A literature search of Medline and eight other databases was performed up to February 2012. Evidence was graded in categories I-IV and agreement with the recommendations was determined through scores from 0 (total disagreement) to 10 (total agreement). Eleven evidence-based recommendations for the non-pharmacological core management of hip and knee OA were developed, concerning the following nine topics: assessment, general approach, patient information and education, lifestyle changes, exercise, weight loss, assistive technology and adaptations, footwear and work. The average level of agreement ranged between 8.0 and 9.1. The proposed research agenda included an overall need for more research into non-pharmacological interventions for hip OA, moderators to optimise individualised treatment, healthy lifestyle with economic evaluation and long-term follow-up, and the prevention and reduction of work disability. Proposed educational activities included the required skills to teach, initiate and establish lifestyle changes. The 11 recommendations provide guidance on the delivery of non-pharmacological interventions to people with hip or knee OA. More research and educational activities are needed, particularly in the area of lifestyle changes.

Abstract: SummaryVery rapidly after Varroa destructor invaded apiaries of Apis mellifera, the devastating effect of this mite prompted an active research effort to understand and control this parasite. Over a few decades, varroa has spread to most countries exploiting A. mellifera. As a consequence, a large number of teams have worked with this organism, developing a diversity of research methods. Often different approaches have been followed to achieve the same goal. The diversity of methods made the results difficult to compare, thus hindering our understanding of this parasite. In this paper, we provide easy to use protocols for the collection, identification, diagnosis, rearing, breeding, marking and measurement of infestation rates and fertility of V. destructor. We also describe experimental protocols to study orientation and feeding of the mite, to infest colonies or cells and measure the mite's susceptibility to acaricides. Abstract: One-third of all primary central nervous system tumors in adults are meningiomas. Rarely, meningiomas occur at multiple sites, usually occurring in individuals with type 2 neurofibromatosis (NF2). We sequenced the exomes of three unrelated individuals with familial multiple spinal meningiomas without NF2 mutations. We identified two individuals with heterozygous loss-of-function mutations in the SWI/SNF chromatin-remodeling complex subunit gene SMARCE1. Sequencing of SMARCE1 in six further individuals with spinal meningiomas identified two additional heterozygous loss-of-function mutations. Tumors from individuals with SMARCE1 mutations were of clear-cell histological subtype, and all had loss of SMARCE1 protein, consistent with a tumor suppressor mechanism. Our findings identify multiple-spinal-meningioma disease as a new discrete entity and establish a key role for the SWI/SNF complex in the pathogenesis of both meningiomas and tumors with clear-cell histology.

Abstract: Wildlife trade is a big and burgeoning business, but its welfare impacts have not been studied comprehensively. We review the animal welfare impacts of the wildlife trade as they were reported in the literature between 2006 and 2011. Rarely was the term welfare mentioned, evidence of welfare impact documented, or welfare improvement recommended. Literature focused on mammals and on animals killed on site, for luxury goods or food, and for traditional medicine. Welfare impacts may be underreported, particularly in international, illegal, and wild-caught trade and trade in reptiles. Greater attention should perhaps be paid to the welfare of animals traded alive and in larger numbers (e.g., birds, reptiles, amphibians) and to those—including mammals—potentially subject to greater impacts through live use (e.g., as pets). More evidence-based research is needed. Animal welfare should be integrated with wider issues; collaboration between conservationists and welfarists and the development of health and welfare levers to influence trade offer benefits to both people and wildlife.

Abstract: The construction industry believes that Building In formation Modelling (BIM) is a platform that has th e potential to promote collaborative activities in the construction indust ry. Thus, clients are gradually enforcing the use o f BIM in their projects, resulting in many construction companies investing in BIM techno logy in order to fulfill clients' needs. Therefore, the goal of this study is to explore the process and the level of BIM implementa tion in the Malaysian construction industry. This s tudy was conducted as an exploratory study through literature review and int erviewing the organisations that have had BIM exper ience. The result reveals that Malaysian construction industry players are having difficulties to implement BIM because they do not k now where, when and how to start as there is no national BIM standard and guid eline for them to follow. Lack of government involv ement worsens the current situation besides having the resistance to change f rom people. To reduce the resistance from people, m ost of the companies came out with their own strategy such as developing new BIM unit, developing training and education program, ch anging management style and developing new roles and responsibilities. Thes e activities could facilitate people and the organi sations in adopting BIM because BIM is not only a technology but it is also involve d in changing the current practices and processes i ncluding changing managerial function and hierarchy, including roles and respons ibilities. Small to medium scale pilot projects can be described as the best practice to adopt BIM in reducing the risk when migrating fr om the traditional approach to a new approach. This study also identified that BIM Level in the Malaysian construction industry is between Level 0 and 1.

Abstract: The past decade has seen a significant interest on the problem of inducing decision trees that take account of costs of misclassification and costs of acquiring the features used for decision making. This survey identifies over 50 algorithms including approaches that are direct adaptations of accuracy-based methods, use genetic algorithms, use anytime methods and utilize boosting and bagging. The survey brings together these different studies and novel approaches to cost-sensitive decision tree learning, provides a useful taxonomy, a historical timeline of how the field has developed and should provide a useful reference point for future research in this field.

Abstract: Doping of graphene via low energy ion implantation could open possibilities for fabrication of nanometer-scale patterned graphene-based devices as well as for graphene functionalization compatible with large-scale integrated semiconductor technology. Using advanced electron microscopy/spectroscopy methods, we show for the first time directly that graphene can be doped with B and N via ion implantation and that the retention is in good agreement with predictions from calculation-based literature values. Atomic resolution high-angle dark field imaging (HAADF) combined with single-atom electron energy loss (EEL) spectroscopy reveals that for sufficiently low implantation energies ions are predominantly substitutionally incorporated into the graphene lattice with a very small fraction residing in defect-related sites.

Abstract: Arsenic in drinking water may cause major deleterious health impacts including death. Although arsenic in rice has recently been demonstrated to be a potential exposure route for humans, there has been to date no direct evidence for the impact of such exposure on human health. Here we show for the first time, through a cohort study in West Bengal, India, involving over 400 human subjects not otherwise significantly exposed to arsenic through drinking water, elevated genotoxic effects, as measured by micronuclei (MN) in urothelial cells, associated with the staple consumption of cooked rice with >200 μg/kg arsenic. Further work is required to determine the applicability to populations with different dietary and genetic characteristics, but with over 3 billion people in the world consuming rice as a staple food and several percent of this rice containing such elevated arsenic concentrations, this study raises considerable concerns over the threat to human health.

Abstract: With the rapid growth of user-generated content on the internet, automatic sentiment analysis of online customer reviews has become a hot research topic recently, but due to variety and wide range of products and services being reviewed on the internet, the supervised and domain-specific models are often not practical. As the number of reviews expands, it is essential to develop an efficient sentiment analysis model that is capable of extracting product aspects and determining the sentiments for these aspects. In this paper, we propose a novel unsupervised and domain-independent model for detecting explicit and implicit aspects in reviews for sentiment analysis. In the model, first a generalized method is proposed to learn multi-word aspects and then a set of heuristic rules is employed to take into account the influence of an opinion word on detecting the aspect. Second a new metric based on mutual information and aspect frequency is proposed to score aspects with a new bootstrapping iterative algorithm. The presented bootstrapping algorithm works with an unsupervised seed set. Third, two pruning methods based on the relations between aspects in reviews are presented to remove incorrect aspects. Finally the model employs an approach which uses explicit aspects and opinion words to identify implicit aspects. Utilizing extracted polarity lexicon, the approach maps each opinion word in the lexicon to the set of pre-extracted explicit aspects with a co-occurrence metric. The proposed model was evaluated on a collection of English product review datasets. The model does not require any labeled training data and it can be easily applied to other languages or other domains such as movie reviews. Experimental results show considerable improvements of our model over conventional techniques including unsupervised and supervised approaches.

Abstract: Dental caries is one of the most common global childhood diseases and is, for the most part, entirely preventable. Good oral health is dependent on the establishment of the key behaviours of toothbrushing with fluoride toothpaste and controlling sugar snacking. Primary schools provide a potential setting in which these behavioural interventions can support children to develop independent and habitual healthy behaviours. To assess the clinical effects of school-based interventions aimed at changing behaviour related to toothbrushing habits and the frequency of consumption of cariogenic food and drink in children (4 to 12 year olds) for caries prevention. We searched the following electronic databases: the Cochrane Oral Health Group's Trials Register (to 18 October 2012), the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2012, Issue 4), MEDLINE via OVID (1948 to 18 October 2012), EMBASE via OVID (1980 to 18 October 2012), CINAHL via EBSCO (1981 to 18 October 2012) and PsycINFO via OVID (1950 to 18 October 2012). Ongoing trials were searched for using Current Controlled Trials (to 18 October 2012) and ClinicalTrials.gov (to 18 October 2012). Conference proceedings were searched for using ZETOC (1993 to 18 October 2012) and Web of Science (1990 to 18 October 2012). We searched for thesis abstracts using the Proquest Dissertations and Theses database (1950 to 18 October 2012). There were no restrictions regarding language or date of publication. Non-English language papers were included and translated in full by native speakers. Randomised controlled trials of behavioural interventions in primary schools (children aged 4 to 12 years at baseline) were selected. Included studies had to include behavioural interventions addressing both toothbrushing and consumption of cariogenic foods or drinks and have a primary school as a focus for delivery of the intervention. Two pairs of review authors independently extracted data related to methods, participants, intervention design including behaviour change techniques (BCTs) utilised, outcome measures and risk of bias. Relevant statistical information was assessed by a statistician subsequently. All included studies contact authors were emailed for copies of intervention materials. Additionally, three attempts were made to contact study authors to clarify missing information. We included four studies involving 2302 children. One study was at unclear risk of bias and three were at high risk of bias. Included studies reported heterogeneity in both the intervention design and outcome measures used; this made statistical comparison difficult. Additionally this review is limited by poor reporting of intervention procedure and design. Several BCTs were identified in the trials: these included information around the consequences of twice daily brushing and controlling sugar snacking; information on consequences of adverse behaviour and instruction and demonstration regarding skill development of relevant oral health behaviours.Only one included study reported the primary outcome of development of caries. This small study at unclear risk of bias showed a prevented fraction of 0.65 (95% confidence interval (CI) 0.12 to 1.18) in the intervention group. However, as this is based on a single study, this finding should be interpreted with caution.Although no meta-analysis was performed with respect to plaque outcomes (due to differences in plaque reporting between studies), the three studies which reported plaque outcomes all found a statistically significant reduction in plaque in the intervention groups with respect to plaque outcomes. Two of these trials involved an 'active' home component where parents were given tasks relating to the school oral health programme (games and homework) to complete with their children. Secondary outcome measures from one study reported that the intervention had a positive impact upon children's oral health knowledge. Currently, there is insufficient evidence for the efficacy of primary school-based behavioural interventions for reducing caries. There is limited evidence for the effectiveness of these interventions on plaque outcomes and on children's oral health knowledge acquisition. None of the included interventions were reported as being based on or derived from behavioural theory. There is a need for further high quality research to utilise theory in the design and evaluation of interventions for changing oral health related behaviours in children and their parents.

Abstract: Cities are responsible for up to 70% of global carbon emissions and 75% of global energy consumption. By 2050 it is estimated that 70% of the world's population will live in cities. The critical challenge for contemporary urbanism, therefore, is to understand how to develop the knowledge, capacity and capability for public agencies, the private sector and multiple users in city-regions (i.e. the city and its wider hinterland) to re-engineer systemically their built environment and urban infrastructure in response to climate change and resource constraints. To inform transitions to urban sustainability, key stakeholders' perceptions were sought though a participatory backcasting and scenario foresight process in order to illuminate challenging but realistic socio-technical scenarios for the systemic retrofit of core UK city-regions. The challenge of conceptualizing complex urban transitions is explored across multiple socio-technical ‘regimes’ (housing, non-domestic buildings, urban infrastructure), scales (building, neighbourhood, city-region), and domains (energy, water, use of resources) within a participatory process. The development of three archetypal ‘guiding visions’ of retrofit city-regional futures developed through this process are discussed, along with the contribution that such foresight processes might play in ‘opening up’ the governance and strategic navigation of urban sustainability.

Abstract: SummaryBackground Home parenteral nutrition (HPN) is currently the management of choice for patients with chronic intestinal failure. Aim To summarise the major issues in delivering long-term parenteral nutrition (>3 months) and assess outcome as per complications, mortality and quality of life. To assess the evidence for the therapeutic use of trophic factors such as teduglutide and to review evolving therapeutic options in the treatment of chronic intestinal failure. Methods A literature search using PubMed and MEDLINE databases was performed. Results Safe delivery of HPN relies upon individualised formulations of parenteral nutrition administered via carefully maintained central venous catheters by trained patients or carers, supported by a skilled multidisciplinary team. Early diagnosis and treatment of complications including catheter-associated blood stream infection (reported incidence 0.14–0.83 episodes/patient-year on HPN) and central venous thrombosis (reported incidence 0.03 episodes/patient-year) is important to minimise mortality and morbidity. There is a significant variation in the reported incidence of both hepatobiliary complications (19–75%) and advanced liver disease (0–50%). Five-year survival rates in large centres are reported between 60% and 78% with survival primarily related to underlying diagnosis. Long-term survival remains higher on HPN than with intestinal transplantation. The role of intestinal lengthening procedures is yet to be validated in adults. Conclusions Home parenteral nutrition delivered by skilled nutrition teams has low incidences of catheter-related complications. Most deaths relate to the underlying disease. Therapies such as teduglutide and small bowel transplantation appear promising, but home parenteral nutrition appears likely to remain the bedrock of management in the near term.

Abstract: The aim of this work is to design a layer of porous material with a high value of the absorption coefficient in a wide range of frequencies. It is shown that low frequency performance can be significantly improved by embedding periodically arranged resonant inclusions (slotted cylinders) into the porous matrix. The dissipation of the acoustic energy in a porous material due to viscous and thermal losses inside the pores is enhanced by the low frequency resonances of the inclusions and energy trapping between the inclusion and the rigid backing. A parametric study is performed in order to determine the influence of the geometry and the arrangement of the inclusions embedded in a porous layer on the absorption coefficient. The experiments confirm that low frequency absorption coefficient of a composite material is significantly higher than that of the porous layer without the inclusions.

Abstract: Over the years, there have been many studies of delay in construction, and this type of study continues to be popular in construction management research. A synthesis and critical evaluation of delay studies in developing countries reveals that poor project management is cited as one of the main causes of delay. However, despite significant consensus, most published studies fall short of providing clear recommendations for the improvement of project management practice. Moreover, the majority of recommendations are general and not devoted to solving the difficulties associated with particular causes of delay. This paper aims to demonstrate that the root cause of this state of affairs is that typical research into delay tends to be descriptive and explanatory, making it inadequate for solving persistent managerial problems in construction. It is contended that many problems in construction could be mitigated via alternative research approaches, i.e. action and constructive research. Such prescriptive research methods can assist in the development and implementation of innovative tools tackling managerial problems of construction, including that of delay. In so doing, those methods will better connect research and practice, and thus strengthen the relevance of academic construction management.

Abstract: This paper reviews the current state of loudspeaker-based spatial sound reproduction methods from technical perspective as well as perceptual perspective. A nomenclature is developed that allows for a strict separation between these two perspectives. The physical fundamentals, practical realization, and results from perceptual studies are discussed for a number of well-established and emerging reproduction techniques. Further, the paper outlines novel approaches to spatial sound evaluation in terms of perceived quality and provides a comparison of current approaches.

Abstract: Public service broadcasters (PSBs) are a central part of national news media landscapes. In many countries, PSBs are the first choice of citizens when it comes to news providers. And in perhaps more countries still, PSBs are thought of as specialists in provision of hard news. We test this proposition here using survey data from a large crossnational survey involving indicators of current affairs knowledge and media consumption. Specifically, we examine whether exposure to public versus commercial news influences the knowledge citizens possess about current affairs, both domestically and internationally. We also test, using propensity score analysis, whether there is variation across PSBs in this regard. Results indicate that compared to commercial news, watching PSB has a net positive influence on knowledge of hard news, though not all PSBs are equally effective in contributing to knowledge acquisition. This knowledge gap between PSB and commercial news media consumption appears to be mitigated by factors such as de jure independence,proportion of public financing, and audience share.

Abstract: Background The mechanism of development of delayed cerebral ischemia (DCI) after aneurysmal subarachnoid hemorrhage (SAH) is poorly understood. Inflammatory processes are implicated in the development of ischemic stroke and may also predispose to the development of DCI following SAH. The objective of this study was to test whether concentrations of circulating inflammatory markers (C-reactive protein (CRP), interleukin-6 (IL-6) and interleukin 1 receptor antagonist (IL-1Ra)) were predictive for DCI following SAH. Secondary analyses considered white cell count (WCC) and erythrocyte sedimentation rate (ESR). Methods This was a single-center case-control study nested within a prospective cohort. Plasma inflammatory markers were measured in patients up to 15 days after SAH (initial, peak, average, final and rate of change to final). Cases were defined as those developing DCI. Inflammatory markers were compared between cases and randomly selected matched controls. Results Among the 179 participants there were 46 cases of DCI (26%). In primary analyses the rate of change of IL-6 was associated with DCI (OR 2.3 (95% CI 1.1 to 5.0); p=0.03). The final value and rate of change of WCC were associated with DCI (OR 1.2 (95% CI 1.0 to 1.3) and OR 1.3 (95% CI 1.0 to 1.6), respectively). High values of ESR were associated with DCI (OR 2.4 (95% CI 1.3 to 4.6) initial; OR 2.3 (95% CI 1.3 to 4.2) average; OR 2.1 (95% CI 1.1 to 3.9) peak; and OR 2.0 (95% CI 1.2 to 3.3) final value). Conclusions Leucocytosis and change in IL-6 prior to DCI reflect impending cerebral ischemia. The time-independent association of ESR with DCI after SAH may identify this as a risk factor. These data suggest that systemic inflammatory mechanisms may increase the susceptibility to the development of DCI after SAH.
