Global Strategy for the Diagnosis, Management and Prevention of Chronic Obstructive Lung Disease 2017 Report: GOLD Executive Summary

TLDR
The assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation, and the concept of de‐escalation of therapy is introduced in the treatment assessment scheme.
Abstract
This Executive Summary of the Global Strategy for the Diagnosis, Management and Prevention of COPD, Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 Report focuses primarily on the revised and novel parts of the document. The most significant changes include: (i) the assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation. ABCD groups are now proposed to be derived exclusively from patient symptoms and their history of exacerbations; (ii) for each of the groups A to D, escalation strategies for pharmacological treatments are proposed; (iii) the concept of de-escalation of therapy is introduced in the treatment assessment scheme; (iv)non-pharmacological therapies are comprehensively presented and (v) the importance of co-morbid conditions in managing COPD is reviewed.

Community determinants of COPD exacerbations in elderly patients in Poland: protocol for a retrospective Big Data observational cohort study.

TL;DR: This study will estimate the prevalence of chronic obstructive pulmonary disease (COPD) and its exacerbations in elderly patients in the Lodz region, Poland and evaluate local community factors potentially associated with disease exacerbations and rank local communities according to health and local community indicators.

Case Report: Laparoscopic hepatectomy in an elderly patient with major comorbidities.

TL;DR: An 81-year-old male with hepatocellular carcinoma in the segment VI of the liver and several major cardiovascular, pulmonary and metabolic comorbid illnesses is referred to the department, indicating that laparoscopic liver resections could be applied even to elderly patients with majorComorbidities after optimization of their medical status.

Correlation Between 2D Strain and Classic Echocardiographic Indices in the Diagnosis of Right Ventricular Dysfunction in COPD.

TL;DR: In this article, the authors defined which of the right ventricular myocardial deformation indices best correlates with the classic echocardiographic measurements and indices of right-ventricular dysfunction in patients with stable chronic obstructive pulmonary disease (COPD).

Evaluation of a Novel Ear Pulse Oximeter: Towards Automated Oxygen Titration in Eyeglass Frames

TL;DR: The OxyFrame cavum conchae pulse oximeter was successfully evaluated, and demonstrated accurate SpO2 measurements, compliant with ISO 80601-2-61:2017, and potential solutions were proposed, which might be valuable for optimization of the sensor hardware.

Screening for obstructive lung disease in hospitalized psychiatric patients

TL;DR: This study illustrates that screening for COPD and asthma is feasible even in a psychiatric population, and the number needed to screen to find one new case is low at 3.4–4.5.

Reduced lung-cancer mortality with low-dose computed tomographic screening.

TL;DR: Screening with the use of low-dose CT reduces mortality from lung cancer, as compared with the radiography group, and the rate of death from any cause was reduced.

The Body-Mass Index, Airflow Obstruction, Dyspnea, and Exercise Capacity Index in Chronic Obstructive Pulmonary Disease

TL;DR: The BODE index, a simple multidimensional grading system, is better than the FEV1 at predicting the risk of death from any cause and from respiratory causes among patients with COPD.

A self-complete measure of health status for chronic airflow limitation. The St. George's Respiratory Questionnaire.

TL;DR: The St. George's Respiratory Questionnaire is a valid measure of impaired health in diseases of chronic airflow limitation that is repeatable and sensitive andMultivariate analysis demonstrated that SGRQ scores summed a number of areas of disease activity.
