TLDR
The assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation, and the concept of de‐escalation of therapy is introduced in the treatment assessment scheme.Abstract:
This Executive Summary of the Global Strategy for the Diagnosis, Management and Prevention of COPD, Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 Report focuses primarily on the revised and novel parts of the document. The most significant changes include: (i) the assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation. ABCD groups are now proposed to be derived exclusively from patient symptoms and their history of exacerbations; (ii) for each of the groups A to D, escalation strategies for pharmacological treatments are proposed; (iii) the concept of de-escalation of therapy is introduced in the treatment assessment scheme; (iv)non-pharmacological therapies are comprehensively presented and (v) the importance of co-morbid conditions in managing COPD is reviewed.read more

TL;DR: In this article, the effects of antibiotics in the management of acute COPD exacerbations on treatment failure and other patient-important outcomes (mortality, adverse events, length of hospital stay) were assessed.

TL;DR: For example, Dirksen et al. as discussed by the authors investigated whether restoration of the balance between neutrophil elastase and its inhibitor, a 1 -antitrypsin, can prevent the progression of pulmonary emphysema in patients with a deficiency.

TL;DR: Evidence of moderate lower lobe bronchiectasis on HRCT is common in COPD and is associated with more severe COPD exacerbations, lower airway bacterial colonization, and increased sputum inflammatory markers.
E-cigarettes: an evidence update. A report commissioned by Public Health England.
TL;DR: There is no evidence that EC are undermining the long-term decline in cigarette smoking among adults and youth, and may in fact be contributing to it, and new regulations currently planned should also maximise the public health opportunities of EC.

TL;DR: The addition of noninvasive positive-pressure ventilation to long-term oxygen therapy in stable chronic obstructive pulmonary disease patients with chronic ventilatory failure slightly decreased the trend to carbon dioxide retention in patients receiving oxygen at home and improved dyspnoea and health-related quality of life.
