TLDR
The assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation, and the concept of de‐escalation of therapy is introduced in the treatment assessment scheme.Abstract:
This Executive Summary of the Global Strategy for the Diagnosis, Management and Prevention of COPD, Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 Report focuses primarily on the revised and novel parts of the document. The most significant changes include: (i) the assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation. ABCD groups are now proposed to be derived exclusively from patient symptoms and their history of exacerbations; (ii) for each of the groups A to D, escalation strategies for pharmacological treatments are proposed; (iii) the concept of de-escalation of therapy is introduced in the treatment assessment scheme; (iv)non-pharmacological therapies are comprehensively presented and (v) the importance of co-morbid conditions in managing COPD is reviewed.read more

TL;DR: In this paper, the authors used segmental multi-frequency bioelectrical impedance analysis (BIA) and functional status to assess body composition analysis and investigate their relationship with the COPD acute exacerbation risk.

Volumetric Texture Analysis Based on Three-Dimensional Gaussian Markov Random Fields for COPD Detection
TL;DR: The proposed 3D GMRF-based descriptor for volumetric texture image classification achieves a high classification accuracy on a 3D synthetic texture database and is applied on a clinical dataset to exploit its discriminatory power, achieving a high classified accuracy in COPD detection.

Diaphragmatic Motor Cortex Hyperexcitability in Patients With Chronic Obstructive Pulmonary Disease
TL;DR: Central cortico-diaphragmatic motor system is affected in COPD patients with heterogeneity of both sides that is correlated with pulmonary function and its evaluation could help in personalization of COPD management especially pulmonary rehabilitation programs.

TL;DR: Cardioselective beta-blockers are preferred, and recent meta-analyses have shown reduced rates in mortality and exacerbations in COPD patients treated with beta- blockers.

TL;DR: This trial could provide some knowledge about the real impact of home monitoring for patients with advanced HF and/or RF within a multi-level model of integrated care and incremental cost-effectiveness analysis will be performed.
