TLDR
The assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation, and the concept of de‐escalation of therapy is introduced in the treatment assessment scheme.Abstract:
This Executive Summary of the Global Strategy for the Diagnosis, Management and Prevention of COPD, Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 Report focuses primarily on the revised and novel parts of the document. The most significant changes include: (i) the assessment of chronic obstructive pulmonary disease has been refined to separate the spirometric assessment from symptom evaluation. ABCD groups are now proposed to be derived exclusively from patient symptoms and their history of exacerbations; (ii) for each of the groups A to D, escalation strategies for pharmacological treatments are proposed; (iii) the concept of de-escalation of therapy is introduced in the treatment assessment scheme; (iv)non-pharmacological therapies are comprehensively presented and (v) the importance of co-morbid conditions in managing COPD is reviewed.read more

Prevalence of lung cancer in chronic obstructive pulmonary disease: A systematic review
TL;DR: Although the prevalence of lung cancer in individuals with COPD is low, it is greater than the prevalence in the general population, and knowledge of the co-occurrence of Lung cancer in COPD will inform the screening of lung cancers for patients who have COPD.

Classification and Treatment of Chronic Obstructive Pulmonary Disease Outpatients in China According to the Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017: Comparison With GOLD 2014
TL;DR: Overtreatment was observed in many patients, and physicians should reexamine treatment patterns for patients reclassified into low-risk groups, as the risk of exacerbation increased with decreased BMI or education levels.

Evaluation of Droplet Digital Polymerase Chain Reaction (ddPCR) for the Absolute Quantification of Aspergillus species in the Human Airway
Tuang Yeow Poh,Nur A'tikah Binte Mohamed Ali,Louisa L Y Chan,Pei Yee Tiew,Pei Yee Tiew,Sanjay H. Chotirmall +5 more
TL;DR: In this paper, the molecular approaches of qPCR and ddPCR were applied to DNA fungal extracts in n = 20 sputum specimens obtained from non-diseased (n = 4), chronic obstructive pulmonary disease (COPD; n = 8) and non-cystic fibrosis bronchiectasis (n= 8) patients where Aspergillus status was known.

Cytotoxic lymphocytes in COPD airways: increased NK cells associated with disease, iNKT and NKT-like cells with current smoking
Jonas Eriksson Ström,Jamshid Pourazar,Robert Linder,Anders Blomberg,Anne Lindberg,Anders Bucht,Annelie F. Behndig +6 more
TL;DR: Interestingly, NK cell percentages did not normalize in COPD subjects that had quit smoking, indicating that these cells might play a role in the continued disease progression seen in COPd even after smoking cessation.

Qualitative and quantitative assessment of pendelluft: a simple method based on electrical impedance tomography
Ling Sang,Zhanqi Zhao,Po-Jen Yun,Inéz Frerichs,Knut Möller,Feng Fu,Xiaoqing Liu,Nanshan Zhong,Yimin Li +8 more
TL;DR: The proposed EIT-based method can be used to evaluate the degree of pendelluft in dimension of phase shift and amplitude difference and demonstrated in well-known pendellUft scenarios in 6 patients.

TL;DR: Screening with the use of low-dose CT reduces mortality from lung cancer, as compared with the radiography group, and the rate of death from any cause was reduced.
