Abstract: Objective. To determine the long-term effects of iron deficiency in infancy. Design. Longitudinal follow-up study of children who had been tested and treated for iron deficiency as infants. Setting. Periurban community near San Jose, Costa Rica. Participants. Of the original 191 participants, 87% were reevaluated at 11 to 14 years old (average age: 12.3 years). The children were free of iron deficiency and growing normally by US standards. Those who had chronic, severe iron deficiency in infancy ( n = 48) were compared with those who had good iron status before and/or after iron therapy in infancy ( n = 114). Outcome Measures. Comprehensive set of cognitive, socioemotional, and motor tests and measures of school functioning. Results. Children who had severe, chronic iron deficiency in infancy scored lower on measures of mental and motor functioning. After control for background factors, differences remained statistically significant in arithmetic achievement and written expression, motor functioning, and some specific cognitive processes (spatial memory, selective recall, and tachistoscopic threshold). More of the formerly iron-deficient children had repeated a grade and/or been referred for special services or tutoring. Their parents and teachers rated their behavior as more problematic in several areas, agreeing in increased concerns about anxiety/depression, social problems, and attention problems. Conclusions. Severe, chronic iron deficiency in infancy identifies children who continue at developmental and behavioral risk >10 years after iron treatment.

Abstract: The purpose of this report is to summarize and integrate the findings
of the Diabetes Control and Complications Trial (DCCT), a randomized controlled
clinical trial, and the succeeding observational follow-up of the DCCT cohort
in the Epidemiology of Diabetes Interventions and Complications (EDIC) study,
regarding the effects of intensive treatment on the microvascular complications
of type 1 diabetes mellitus. The DCCT proved that intensive treatment reduced
the risks of retinopathy, nephropathy, and neuropathy by 35% to 90% compared
with conventional treatment. The absolute risks of retinopathy and nephropathy
were proportional to the mean glycosylated hemoglobin (HbA1c) level
over the follow-up period preceding each event. Intensive treatment was most
effective when begun early, before complications were detectable. These risk
reductions, achieved at a median HbA1c level difference of 9.1%
for conventional treatment vs 7.3% for intensive treatment have been maintained
through 7 years of EDIC, even though the difference in mean HbA1c
levels of the 2 former randomized treatment groups was only 0.4% at 1 year
(P<.001) (8.3% in the former conventional treatment
group vs 7.9% in the former intensive treatment group), continued to narrow,
and became statistically nonsignificant by 5 years (8.1% vs 8.2%, P = .09). The further rate of progression of complications from their
levels at the end of the DCCT remains less in the former intensive treatment
group. Thus, the benefits of 6.5 years of intensive treatment extend well
beyond the period of its most intensive implementation. Intensive treatment
should be started as soon as is safely possible after the onset of type 1
diabetes mellitus and maintained thereafter, aiming for a practicable target
HbA1c level of 7.0% or less.

Abstract: Purpose – The purpose of this paper is to show that development of competencies needed to be effective managers and leaders requires program design and teaching methods focused on learning. This is the introductory essay to this special issue of JMD.Design/methodology/approach – Competencies are defined and an overview is provided for the eight papers that will follow with original research on competencies, their link to performance in various occupations, and their development.Findings – Emotional, social and cognitive intelligence competencies predict effectiveness in professional, management and leadership roles in many sectors of society. In addition, these competencies can be developed in adults.Research limitations/implications – As an introductory essay, this lays the foundation for the papers in this issue.Practical implications – Competencies needed to be effective can be developed.Originality/value – Despite widespread application, there are few published studies of the empirical link between co...

Abstract: Study Objectives: Limited experimental data suggest that sleep restriction acutely elevates blood pressure; however, little is known about the relationship between usual sleep duration and hypertension. This study assesses the relationship between usual sleep duration and hypertension in a community-based cohort. Design: Cross-sectional observational study. Setting: The Sleep Heart Health Study, a community-based prospective study of the cardiovascular consequences of sleep-disordered breathing. Participants: Two thousand eight hundred thirteen men and 3097 women, aged 40 to 100 years. Interventions: None. Measurements and Results: Usual weekday and weekend sleep durations were obtained by questionnaire, and their weighted average were categorized as less than 6, 6 to less than 7, 7 to less than 8, 8 to less than 9, and 9 or more hours per night. Hypertension was defined as a systolic blood pressure of 140 mm Hg or greater, a diastolic blood pressure of 90 mm Hg or greater, or use of medication to treat hypertension. The relationship between sleep duration and hypertension was examined using categorical logistic regression with adjustment for age, sex, race, apnea-hypopnea index, and body mass index. Compared to subjects sleeping 7 to less than 8 hours per night, those sleeping less than 6 and between 6 and 7 hours per night had adjusted odds ratios for hypertension of 1.66 (95% confidence interval 1.35-2.04) and 1.19 (1.02-1.39), respectively, whereas those sleeping between 8 and 9 and 9 or more hours per night had adjusted odds ratios for hypertension of 1.19 (1.04-1.37) and 1.30 (1.04-1.62), respectively (p <.0001 for association of sleep duration with hypertension). These associations persisted when analyses were further adjusted for caffeine and alcohol consumption, current smoking, insomnia symptoms, depression symptoms, sleep efficiency, and prevalent diabetes mellitus or cardiovascular disease. Conclusions: Usual sleep duration above or below the median of 7 to less than 8 hours per night is associated with an increased prevalence of hypertension, particularly at the extreme of less than 6 hours per night.
