Guidelines for the Management of Rheumatoid Arthritis 2002 Update

TLDR
The guideline reviewed in this month's column describes the recommended care of patients who have been previously diagnosed with RA.
Abstract
Rheumatoid arthritis (RA) is a progressive polyarthritis that is responsible for over nine million office visits annually. It is likely that most nurse practitioners will care for one or more patients with RA because approximately 1% of the adult population is affected by this disabling disorder. The guideline reviewed in this month's column describes the recommended care of patients who have been previously diagnosed with RA.

Can clinical factors at presentation be used to predict outcome of treatment with methotrexate in patients with early inflammatory polyarthritis

TL;DR: Routine clinical and laboratory factors were poor at predicting outcome of treatment with MTX, with the strongest predictor of inefficacy at both time points being shared epitope positivity.

Will pharmacogenetics allow better prediction of methotrexate toxicity and efficacy in patients with rheumatoid arthritis

TL;DR: Patients with RA either do not benefit from the drug or are unable to tolerate it, and pharmacogenetic approaches may help optimise treatment.

Adherence to a treat-to-target strategy in early rheumatoid arthritis: results of the DREAM remission induction cohort

TL;DR: Evaluating the adherence to a T2T strategy aiming at remission (Disease Activity Score in 28 joints (DAS28) < 2.6) in early rheumatoid arthritis in daily practice demonstrates that a T1T strategy is feasible in RA in daily clinical practice.

Etanercept: an updated review of its use in rheumatoid arthritis, psoriatic arthritis and juvenile rheumatoid arthritis.

TL;DR: Etanercept provides rapid and sustained improvements in disease activity in patients with early and DMARD-refractory rheumatoid arthritis and has been shown to inhibit radiographic progression in those with early disease.

Lack of head-to-head trials and fair control arms: randomized controlled trials of biologic treatment for rheumatoid arthritis.

TL;DR: The control arms and the treatment received by patients in recently conducted or ongoing randomized controlled trials of biologic disease-modifying antirheumatic drugs (DMARDs) for rheumatoid arthritis are described.

Measurement of patient outcome in arthritis.

TL;DR: A structure for representation of patient outcome is presented, together with a method for outcome measurement and validation of the technique in rheumatoid arthritis, and these techniques appear extremely useful for evaluation of long term outcome of patients with rheumatic diseases.
