Assessment and Transplantation of Orphan Donor Livers: A Back‐to‐Base Approach to Normothermic Machine Perfusion
Janske Reiling,Nick Butler,Nick Butler,Andrew H. Simpson,Peter Hodgkinson,Peter Hodgkinson,Catherine M. Campbell,David S. R. Lockwood,David S. R. Lockwood,Kim R. Bridle,Kim R. Bridle,N. Santrampurwala,N. Santrampurwala,Laurence J. Britton,Darrell H. G. Crawford,Darrell H. G. Crawford,Cornelius H. C. Dejong,Jonathan Fawcett +17 more
TL;DR: The OrganOx metra using a back‐to‐base approach has enabled the safe use of 10 high‐risk orphan donor livers with 100% 6‐month patient and graft survival and has enabled a safe expansion of the donor pool.

TL;DR: The assay of arterial LCEOT ≥5 mmol/L appears to be a strong predictor of early graft outcomes and may be used as an endpoint in studies assessing the impact of perioperative management.
Journal ArticleDOI
Normothermic Machine Preservation of the Liver: State of the Art.
TL;DR: Evidence to date suggests that NMP facilitates the enhanced preservation of liver grafts with improved early post-transplant outcomes, and provides the exciting potential for liver-directed therapeutic interventions.

TL;DR: HOPE of ECD grafts compared to the traditional SCS preservation method is associated with lower dysfunction rates and better graft survival, and that patients in the HOPE group were associated with higher graft survival one year after LT.
Journal ArticleDOI
The dawn of liver perfusion machines.
TL;DR: Machine perfusion is a promising strategy to expand the donor pool by reconditioning and assessing viability of DCD, elderly and steatotic grafts during the preservation period and will serve as a platform to facilitate graft intervention and modification to further optimize marginal grafts.

TL;DR: There was a lack of survival benefit in DCD livers utilized in patients with model for end‐stage liver disease (MELD) ≤30 or those not on organ‐perfusion support, as graft survival was significantly lower compared to DBD patients, however, DCD and DBD subjects transplanted with MELD >30 or on organ-perfusions support had similar graft survival, suggesting a potentially greater benefit of DCDLivers in critically ill patients.
