Abstract: OBJECTIVE. Cerebral malaria affects >785000 African children every year. We previously documented an increased frequency of cognitive impairment in children with cerebral malaria 6 months after their initial malaria episode. This study was conducted to determine the long-term effects of cerebral malaria on the cognitive function of these children. METHODS. Children who were 5 to 12 years of age and presented to Mulago Hospital, Kampala, Uganda, with cerebral malaria (n = 44) or uncomplicated malaria (n = 54), along with healthy, asymptomatic community children (n = 89), were enrolled in a prospective cohort study of cognition. Cognitive testing was performed at enrollment and 2 years later. The primary outcome was presence of a deficit in ≥1 of 3 cognitive areas tested. RESULTS. At 2-year follow-up testing, 26.3% of children with cerebral malaria and 12.5% with uncomplicated malaria had cognitive deficits in ≥1 area, as compared with 7.6% of community children. Deficits in children with cerebral malaria were primarily in the area of attention (cerebral malaria, 18.4%, vs community children, 2.5%). After adjustment for age, gender, nutrition, home environment, and school level, children with cerebral malaria had a 3.67-fold increased risk for a cognitive deficit compared with community children. Cognitive impairment at 2-year follow-up was associated with hyporeflexia on admission and neurologic deficits 3 months after discharge. CONCLUSIONS. Cerebral malaria is associated with long-term cognitive impairments in 1 of 4 child survivors. Future studies should investigate the mechanisms involved so as to develop interventions aimed at prevention and rehabilitation.

Abstract: High plasma HIV-1 RNA concentrations are associated with an increased risk of HIV-1 transmission. Although plasma and genital HIV-1 RNA concentrations are correlated, no study has evaluated the relationship between genital HIV-1 RNA and the risk of heterosexual HIV-1 transmission. In a prospective study of 2521 African HIV-1 serodiscordant couples, we assessed genital HIV-1 RNA quantity and HIV-1 transmission risk. HIV-1 transmission linkage was established within the partnership by viral sequence analysis. We tested endocervical samples from 1805 women, including 46 who transmitted HIV-1 to their partner, and semen samples from 716 men, including 32 who transmitted HIV-1 to their partner. There was a correlation between genital and plasma HIV-1 RNA concentrations: For endocervical swabs, Spearman’s rank correlation coefficient r was 0.56, and for semen, r was 0.55. Each 1.0 log10 increase in genital HIV-1 RNA was associated with a 2.20-fold (for endocervical swabs: 95% confidence interval, 1.60 to 3.04) and a 1.79-fold (for semen: 95% confidence interval, 1.30 to 2.47) increased risk of HIV-1 transmission. Genital HIV-1 RNA independently predicted HIV-1 transmission risk after adjusting for plasma HIV-1 quantity (hazard ratio, 1.67 for endocervical swabs and 1.68 for semen). Seven female-to-male and four male-to-female HIV-1 transmissions (incidence <1% per year) occurred from persons with undetectable genital HIV-1 RNA, but in all 11 cases, plasma HIV-1 RNA was detected. Thus, higher genital HIV-1 RNA concentrations are associated with greater risk of heterosexual HIV-1 transmission, and this effect was independent of plasma HIV-1 concentrations. These data suggest that HIV-1 RNA in genital secretions could be used as a marker of HIV-1 sexual transmission risk.

Abstract: There is an increased global demand for activated carbon (AC) in application of water treatment and purification. Water pollutants that have exhibited a greater removal efficiency by AC included but not limited to heavy metals, pharmaceuticals, pesticides, natural organic matter, disinfection by-products, and microplastics. Granular activated carbon (GAC) is mostly used in aqueous solutions and adsorption columns for water treatment. Commercial AC is not only costly, but also obtained from non-renewable sources. This has prompted the search for alternative renewable materials for AC production. Biomass wastes present a great potential of such materials because of their availability and carbonaceous nature. This in turn can reduce on the adverse environmental effects caused by poor disposal of these wastes. The challenges associated with biomass waste based GAC are their low strength and attrition resistance which make them easily disintegrate under aqueous phase. This paper provides a comprehensive review on recent advances in production of biomass waste based GAC for water treatment and highlights future research directions. Production parameters such as granulation conditions, use of binders, carbonization, activation methods, and their effect on textural properties are discussed. Factors influencing the adsorption capacities of the derived GACs, adsorption models, adsorption mechanisms, and their regeneration potentials are reviewed. The literature reveals that biomass waste materials can produce GAC for use in water treatment with possibilities of being regenerated. Nonetheless, there is a need to explore 1) the effect of preparation pathways on the adsorptive properties of biomass derived GAC, 2) sustainable production of biomass derived GAC based on life cycle assessment and techno-economic analysis, and 3) adsorption mechanisms of GAC for removal of contaminants of emerging concerns such as microplastics and unregulated disinfection by-products.
