Abstract: 3-year-old children were presented with easy and difficult tasks and their emotional responses of shame and pride were observed. No shame was shown when subjects succeeded on the tasks and no pride was shown when they failed. Significantly more shame was shown when subjects failed easy tasks than when they failed difficult tasks, and significantly more pride was shown when subjects succeeded on difficult than on easy tasks. While there were no sex differences in task failures, girls showed more shame than boys. There were no sex differences in pride when subjects succeeded.

Abstract: The recent surge in knowledge of cellular and molecular mechanisms of intestinal sugar transport has fueled an enormous interest in adaptive mechanisms regulating sugar transport. We first review several functional considerations that help us interpret the different patterns of adaptation for different nutrients. We then distinguish nonspecific adaptive mechanisms leading to parallel changes in transport of different nutrients from specific adaptive mechanisms only affecting the transport of a single nutrient. Nonspecific adaptive mechanisms include changes in mucosal surface area and in the ratio of transporting to nontransporting cells; specific mechanisms include changes in site density of transporters and in affinity constants. We also enumerate the patterns of regulation and describe how sugar transport is affected by changes in diet, energy budgets, and environmental salinity as well as by intestinal resection, starvation, stress, and age. We relate the various signals linking these stimuli to adaptive mechanisms and make predictions about the nature of these signals. Finally, we describe the significance of the interactions among sugar, fluid, and electrolyte transport mechanisms and of the paracellular pathway to transepithelial transport of sugars. We close by drawing attention to promising directions for future research.

Abstract: Many commentators view the conversion of small, independent primary care practices into patient-centered medical homes as a vital step in creating a better-performing health care system. The country's first national medical home demonstration, which ran from June 1, 2006, to May 31, 2008, and involved thirty-six practices, showed that this transformation can be lengthy and complex. Among other features, the transformation process requires an internal capability for organizational learning and development; changes in the way primary care clinicians think about themselves and their relationships with patients as well as other clinicians on the care team; and awareness on the part of primary care clinicians that they will need to make long-term commitments to change that may require three to five years of external assistance. Additionally, transforming primary care requires synchronizing practice redesign with development of the health care "neighborhood," which is made up of a broad range of health and health care resources available to patients. It also requires payment reform that supports practice development and a policy environment that sets reasonable expectations and time frames for the adoption of appropriate innovations.

Abstract: Telomeres play a central role in cellular senescence and cancer pathobiology and are associated with age-related diseases such as atherosclerosis and dementia. Telomere length varies between individuals of the same age, is influenced by DNA-damaging factors such as oxidative stress, and is heritable. We performed a quantitative-trait linkage analysis using an ∼10-cM genomewide map for mean leukocyte terminal-restriction fragment (TRF) lengths measured by Southern blotting, in 2,050 unselected women aged 18–80 years, comprising 1,025 complete dizygotic twin pairs. Heritability of mean batch-adjusted TRF was 36% (95% confidence interval [CI] 18%–48%), with a large common environmental effect of 49% (95% CI 40%–58%). Significant linkage was observed on chromosome 14 (LOD 3.9) at 14q23.2, and suggestive linkage at 10q26.13 (LOD 2.4) and 3p26.1 (LOD 2.7). This is the first report of loci, mapped in a sample of healthy individuals, that influence mean telomere variation in humans.
