Abstract: Laparoscopic colorectal resection has become popular. The recently developed da Vinci Surgical System promises to facilitate endoscopic surgery and overcome its disadvantages. This study therefore aimed to compare the short-term results between robotic tumor-specific mesorectal excision (R-TSME) using the da Vinci Surgical System and conventional laparoscopic tumor-specific mesorectal excision (L-TSME) in rectal cancer patients. Between April 2006 and February 2007, 36 patients were randomly assigned to receive R-TSME or L-TSME. During the study, 18 patients underwent robotic low anterior resection using the da Vinci Surgical System, and 18 patients had conventional laparoscopic low anterior resection. Patient characteristics, perioperative clinical results, complications, and pathologic details were compared between the two groups. The patient characteristics were not significantly different between the two groups. The mean operating time, hemoglobin change, and conversion rate were not significantly different between the groups. Complications were treated conservatively and did not require surgical intervention in the R-TSME group. The average length of stay was 6.9 ± 1.3 days in the R-TSME group and 8.7 ± 1.3 days in the L-TSME group (p < 0.001). The specimen quality of the R-TSME group was acceptable. Tumor-specific mesorectal excision was performed safely and effectively using the da Vinci Surgical System and the perioperative outcomes were acceptable.

Abstract: To investigate the electronic structure of the electrochemically Li-ion deintercalated Li 1 - x CoO 2 system, soft X-ray absorption spectroscopy (XAS) for the oxygen K-edge and the Co L I I , I I I -edge has been carried out intensively with compositional x value variation, compared with Co K-edge X-ray absorption near edge structure (XANES) spectroscopy. To get reasonably good XAS spectra for the electrochemically Li-ion deintercalated Li 1 - x CoO 2 system, we made a binder-free LiCoO 2 film electrode using the electrostatic spray deposition (ESD) technique. The oxygen K-edge XAS for Li 1 - x CoO 2 shows more effective spectral change with respect to Li-ion content than the Co L I I , I I I -edge XAS. The dependence of the absorption peak feature on the Li content is described in terms of the ground state of the Co and O atoms, showing the systematic variation of the hole-state site distribution between Co and oxygen atoms. From the Co L I I , I I I -edge XAS, it is found that the broad peak shift to higher energy with the Li-ion deintercalation is due to rehybridization between Co and O atoms under the local structural distortion of CoO 6 octahedra, which is also confirmed by the formation of two additional absorption peaks below the threshold energy corresponding to the oxygen 2p hole state hybridized with the 3d orbital of Co ion in the distorted CoO 6 octahedral site. In the O K-edge XAS spectra for the deintercalated Li 1 - x CoO 2 film, the shoulder absorption peak in the energy region higher than the threshold energy could be assigned to the higher oxidation state of the oxygen site on Li deintercalation. From the Co L I I , I I I -edge and O K-edge XAS results for the Li-ion deintercalated Li 1 - x CoO 2 film, it is concluded that the charge compensation for the Li-ion deintercalation process could be achieved mainly in the oxygen site and Co metal atomic site simultaneously. O K-edge and Co L I I , I I I -edge XAS results for cycled LiCoO 2 film show that the capacity fading of the LiCoO 2 system is related to the decrease of Co-O bond covalency by the local structural distortion of CoO 6 octahedra remaining in the cycled LiCoO 2 .

Abstract: Obesity is currently epidemic in many countries worldwide and is strongly related to diabetes and cardiovascular disease. This study investigated the differences in metabolomic profiling between overweight/obese and normal-weight men. Overweight/obese (n=30) and age-matched, normal-weight men (n=30) were included. Anthropometric parameters, conventional metabolites, and biomarkers were measured. Metabolomic profiling was analyzed with UPLC-Q-TOF MS. Overweight/obese men showed higher levels of HOMA-IR, triglycerides, total cholesterol, and LDL-cholesterol, and lower levels of HDL-cholesterol and adiponectin than lean men. Overweight/obese men showed higher proportion of stearic acid and lower proportion of oleic acid in serum phospholipids. Additionally, overweight/obese individuals showed higher fat intake and lower ratio of polyunsaturated fatty acids to saturated fatty acids. We identified three lyso-phosphatidylcholine (lysoPC) as potential plasma markers and confirmed eight known metabolites for overweight/obesity men. Especially, overweight/obese subjects showed higher levels of lysoPC C14:0 and lysoPC C18:0 and lower levels of lysoPC C18:1 than lean subjects. Results confirmed abnormal metabolism of two branched-chain amino acids, two aromatic amino acids, and fatty acid synthesis and oxidation in overweight/obese men. Additionally, the amount of dietary saturated fat may influence the proportion of saturated fatty acids in serum phospholipids and the degree of saturation of the constituent acyl group of plasma lysoPC.

Abstract: The present study investigated how reports of satisfaction with specific versus global domains can be used to assess a disposition towards positivity in subjective well-being reports. College students from 41 societies (N = 7167) completed measures of life satisfaction and ratings of global and specific aspects of their lives. For example, participants rated satisfaction with their education (global) and satisfaction with their professors, textbooks, and lectures (specific). It was hypothesized that global measures would more strongly reflect individual differences in dispositional positivity, that is, a propensity to evaluate aspects of life in general as good. At both the individual and national levels, positivity predicted life satisfaction beyond objective measures. Also, positivity was associated with norms about ideal life satisfaction such that countries and individuals who highly valued positive emotions were more likely to display positivity. The difference between more global versus more concrete measures of satisfaction can be used as an indirect and subtle measure of positivity.

Abstract: Aim of the study In patients from the Dutch TME trial patterns of local recurrence (LR) in rectal cancer were studied. The purpose was to reconstruct the most likely mechanisms of LR and the effect of preoperative radiotherapy. Methods 1417 patients were analyzed; 713 were randomized into preoperative radiotherapy and total mesorectal excision (RT + TME), 704 into TME alone. Of the 114 patients with LR, the subsites of LR were determined and related to tumor and treatment factors. Results Overall 5-year LR-rate was 4.6% in the RT + TME group and 11.0% in the TME group. Presacral local recurrences occurred most in both groups. Radiotherapy reduced anastomotic LR significantly, except when after low anterior resection (LAR) distal margins were less than 5 mm. Abdominoperineal resection (APR) mainly resulted in presacral LR. Even after resection with a negative circumferential resection margin, LR-rates were high. Thirty percent of the patients had advanced tumors, which resulted in 58% of all LRs. Lateral LR comprised 20% of all LR. Presacral and lateral LR resulted in a poor prognosis, in contrast to anterior or anastomotic LRs with a relatively good prognosis. Conclusions RT reduces LR in all subsites and is especially effective in preventing anastomotic LR after LAR. APR-surgery mainly results in presacral LR, which may be prevented by a wider resection. In the TME trial many advanced tumors were included, rather requiring chemoradiotherapy instead of RT. Currently, with good imaging techniques, better selection can take place. Especially lateral LR might be a problem in the future.
