Abstract: Objectives (i) To examine attitudes of medical and nursing staff towards reporting incidents (adverse events and near-misses), and (ii) to identify measures to facilitate incident reporting. Design Qualitative study. In March 2002, semistructured questions were administered to five focus groups--one each for consultants, registrars, resident medical officers, senior nurses, and junior nurses. Participants and setting 14 medical and 19 nursing staff recruited using purposive sampling from three metropolitan public hospitals in Adelaide, South Australia. Main outcome measures Attitudes and barriers to incident reporting; differences in reporting behaviour between disciplines; how to facilitate incident reporting. Results Cultural differences between doctors and nurses, identified using Triandis' theory of social behaviour, were found to underpin attitudes to incident reporting. Nurses reported more habitually than doctors due to a culture which provided directives, protocols and the notion of security, whereas the medical culture was less transparent, favoured dealing with incidents "in-house" and was less reliant on directives. Common barriers to reporting incidents included time constraints, unsatisfactory processes, deficiencies in knowledge, cultural norms, inadequate feedback, beliefs about risk, and a perceived lack of value in the process. Conclusions Strategies to improve incident reporting must address cultural issues.

Abstract: More than 2.5 billion people on the globe rely on groundwater for drinking and providing high-quality drinking water has become one of the major challenges of human society. Although groundwater is considered as safe, high concentrations of heavy metals like arsenic (As) can pose potential human health concerns and hazards. In this paper, we present an overview of the current scenario of arsenic contamination of groundwater in various countries across the globe with an emphasis on the Indian Peninsula. With several newly affected regions reported during the last decade, a significant increase has been observed in the global scenario of arsenic contamination. It is estimated that nearly 108 countries are affected by arsenic contamination in groundwater (with concentration beyond maximum permissible limit of 10 ppb recommended by the World Health Organization. The highest among these are from Asia (32) and Europe (31), followed by regions like Africa (20), North America (11), South America (9) and Australia (4). More than 230 million people worldwide, which include 180 million from Asia, are at risk of arsenic poisoning. Southeast Asian countries, Bangladesh, India, Pakistan, China, Nepal, Vietnam, Burma, Thailand and Cambodia, are the most affected. In India, 20 states and 4 Union Territories have so far been affected by arsenic contamination in groundwater. An attempt to evaluate the correlation between arsenic poisoning and aquifer type shows that the groundwater extracted from unconsolidated sedimentary aquifers, particularly those which are located within the younger orogenic belts of the world, are the worst affected. More than 90% of arsenic pollution is inferred to be geogenic. We infer that alluvial sediments are the major source for arsenic contamination in groundwater and we postulate a strong relation with plate tectonic processes, mountain building, erosion and sedimentation. Prolonged consumption of arsenic-contaminated groundwater results in severe health issues like skin, lung, kidney and bladder cancer; coronary heart disease; bronchiectasis; hyperkeratosis and arsenicosis. Since the major source of arsenic in groundwater is of geogenic origin, the extend of pollution is complexly linked with aquifer geometry and aquifer properties of a region. Therefore, remedial measures are to be designed based on the source mineral, climatological and hydrogeological scenario of the affected region. The corrective measures available include removing arsenic from groundwater using filters, exploring deeper or alternative aquifers, treatment of the aquifer itself, dilution method by artificial recharge to groundwater, conjunctive use, and installation of nano-filter, among other procedures. The vast majority of people affected by arsenic contamination in the Asian countries are the poor who live in rural areas and are not aware of the arsenic poisoning and treatment protocols. Therefore, creating awareness and providing proper medical care to these people remain as a great challenge. Very few policy actions have been taken at international level over the past decade to reduce arsenic contamination in drinking water, with the goal of preventing toxic impacts on human health. We recommend that that United Nations Environment Programme (UNEP) and WHO should take stock of the global arsenic poisoning situation and launch a global drive to create awareness among people/medical professionals/health workers/administrators on this global concern.

Abstract: C-reactive protein (CRP) is an inflammatory-response protein that is a strong, independent predictor of cardiovascular mortality. CRP is positively associated with body mass index (BMI). In this study, we investigated the effects of dynamic weight loss on CRP in 83 healthy, obese women (mean BMI, 33.8+/-0.4 kg/m(2); range, 28.2 to 43.8 kg/m(2)). Subjects were placed on very-low-fat, energy-restricted diets (5700 kJ, 15% fat) for 12 weeks. Weight, waist and hip circumferences, plasma lipids, glucose, and CRP were measured at baseline and after 12 weeks. CRP was positively associated with BMI (r=0.281, P=0.01) and waist circumference (r=0.278, P=0.01) but was not related to other atherosclerosis risk factors. BMI was significantly different between groups split above or below the median for CRP (34.8+/-0.6 kg/m(2) vs 33.0+/-0.5 kg/m(2), P=0.02). After 12 weeks, weight loss was 7.9+/-0.3 kg. CRP was significantly decreased by 26% (P<0.001), and a correlation was observed between weight loss and the change in CRP (r=0.309, P=0.005). The variance in the change in CRP was partly explained by initial CRP (13.6%), energy intake (5.4%), and percentage weight loss (4.6%, P=0.001). This study confirms recent observations that BMI is associated with CRP, a marker for low-grade systemic inflammation. Furthermore, we observed that CRP was lowered in proportion to weight loss.

Abstract: Summary In this review we compare the structure and function of the interfaces between symbionts in biotrophic associations. The emphasis is on biotrophic fungal parasites and on mycorrhizas, although necrotrophic parasitic associations and the Rhizobium/legume symbiosis are mentioned briefly. We take as a starting point the observations that in the parasitic associations nutrient transport is polarized towards the parasite, whereas in mutualistic associations it is bidirectional. The structure and function of the interfaces are then compared. An important common feature is that in nearly all cases the heterotrophic symbiont (whether mutualistic or parasitic) is located topologically outside the cytoplasm of the host cells, in an apoplastic compartment. This means that nutrient movements across the interface must involve transport into and out of this apoplastic region through membranes of both organisms. Basic principles of membrane transport in uninfected cells are briefly reviewed to set the scene for a discussion of transport mechanisms which may operate in parasitic and mycorrhizal symbioses. The presence and possible roles of ATPases associated with membranes at the interfaces are discussed. We conclude that cytochemical techniques (used to demonstrate the activity of these enzymes) need to he extended and complemented by biochemical and biophysical studies in order to confirm that the activity is due to transport ATPases. Nevertheless, the distribution of activity appears to he in accord with polarized transport mechanisms in some pathogens and with bidirectional transport in mycorrhizas. The absence of ATPases on many fungal membranes needs re-examination. We emphasize that transport mechanisms between mycorrhizal symbionts cannot be viewed simply as the exchange of carbon for phosphate. Additional features include provision for transport of carbon and nitrogen as amino acids or amides and for ions such as K+ and H+ involved in the maintenance of charge balance and pH regulation, processes which also occur in parasitic associations. Interplant transport of nutrients via mycorrhizal hyphae is discussed in the context of these complexities. Some suggestions for the directions of future work are made.
