Pancreatic cancer: A review of clinical diagnosis, epidemiology, treatment and outcomes

TL;DR: This review aims to outline the most up-to-date knowledge of pancreatic adenocarcinoma risk, diagnostics, treatment and outcomes, while identifying gaps that aim to stimulate further research in this understudied malignancy.

TL;DR: A conservative approach is recommended for asymptomatic MCN and IPMN, and Lifelong follow-up of IPMN is recommended in patients who are fit for surgery.

Pancreatic adenocarcinoma, Version 2.2021

Margaret A. Tempero, +35 more
Abstract: Pancreatic cancer is the fourth leading cause of cancer-related death among men and women in the United States. A major challenge in treatment remains patients' advanced disease at diagnosis. The NCCN Guidelines for Pancreatic Adenocarcinoma provides recommendations for the diagnosis, evaluation, treatment, and follow-up for patients with pancreatic cancer. Although survival rates remain relatively unchanged, newer modalities of treatment, including targeted therapies, provide hope for improving patient outcomes. Sections of the manuscript have been updated to be concordant with the most recent update to the guidelines. This manuscript focuses on the available systemic therapy approaches, specifically the treatment options for locally advanced and metastatic disease.

Cystadenomas of the pancreas: is enucleation an adequate operation?

TL;DR: In this paper, the authors concluded that enucleation is an adequate procedure for benign cystic neoplasms of the pancreas and that the recurrence rate is low after this procedure.

Comparative performance of MDCT and MRI with MR cholangiopancreatography in characterizing small pancreatic cysts.

TL;DR: MRI enables more confident assessment of the morphology of small cysts than MDCT, but the accuracy of the two imaging techniques for cyst characterization is comparable.

Preoperative GNAS and KRAS Testing in the Diagnosis of Pancreatic Mucinous Cysts.

TL;DR: The combination of GNAS and KRAS testing was highly specific and sensitive for IPMNs; however, the lack of sensitivity for MCNs highlights the need for additional markers to improve the detection of pancreatic mucinous neoplasms.

Outcomes following resection of invasive and noninvasive intraductal papillary mucinous neoplasms of the pancreas.

TL;DR: Patients with the invasive form of IPMN will have a similarly poor survival as those with ductal adenocarcinoma and in patients thought to have a benign IPMN, these lesions should be removed to avoid conversion to invasive cancer.

Imaging features to distinguish malignant and benign branch-duct type intraductal papillary mucinous neoplasms of the pancreas: a meta-analysis.

TL;DR: Presence of mural nodules should be regarded highly suspicious for malignancy warranting a surgical excision whereas cyst size greater than 3 cm, MPD dilatation (5–9 mm), or thick septum/wall may better be managed by careful observation and/or further evaluation.
