Abstract: Purpose Reactivation of hepatitis B virus (HBV) infection is a well-recognized complication in cancer patients with chronic HBV (hepatitis B surface antigen [HBsAg] positive) undergoing cytotoxic chemotherapy. In patients who have resolved HBV (HBsAg negative and antibody to hepatitis B core antigen [anti-HBc] ± antibody to hepatitis B surface antigen [anti-HBs] positive), such incidence has been much less common until recent use of rituximab. In this study on HBsAg-negative/anti-HBc–positive lymphoma patients, the objectives were to determine the HBV reactivation rate in patients treated with rituximab-containing chemotherapy and to compare it with the rate in patients treated without rituximab. Patients and Methods Between January 2003 and December 2006, all patients diagnosed with CD20+ diffuse large B-cell lymphoma (DLBCL) had HBsAg determined before anticancer therapy. Abstract: The development of the wireless body area sensor network (BASN) is imperative for modern telemedicine and m-health, but security remains a formidable challenge yet to be resolved. As nodes of BASN are expected to be interconnected on or in the human body, the body itself can form an inherently secure communication pathway that is unavailable to all other kinds of wireless networks. This article explores the use of this conduit in the security mechanism of BASN; that is, by a biometrics approach that uses an intrinsic characteristic of the human body as the authentication identity or the means of securing the distribution of a cipher key to secure inter-BASN communications. The method was tested on 99 subjects with 838 segments of simultaneous recordings of electrocardiogram and photoplethysmogram. By using the interpulse interval (IPI) as the biometric trait, the system achieved a minimum half total error rate of 2.58 percent when the IPIs measured from signals, which were sampled at 1000 Hz, were coded into 128-bit binary sequences. The study opens up a few key issues for future investigation, including compensation schemes for the asynchrony of different channels, coding schemes, and other suitable biometric traits.

Abstract: Zoledronic acid 5 mg (ZOL) annually for 3 years reduces fracture risk in postmenopausal women with osteoporosis. To investigate long-term effects of ZOL on bone mineral density (BMD) and fracture risk, the Health Outcomes and Reduced Incidence with Zoledronic acid Once Yearly-Pivotal Fracture Trial (HORIZON-PFT) was extended to 6 years. In this international, multicenter, double-blind, placebo-controlled extension trial, 1233 postmenopausal women who received ZOL for 3 years in the core study were randomized to 3 additional years of ZOL (Z6, n = 616) or placebo (Z3P3, n = 617). The primary endpoint was femoral neck (FN) BMD percentage change from year 3 to 6 in the intent-to-treat (ITT) population. Secondary endpoints included other BMD sites, fractures, biochemical bone turnover markers, and safety. In years 3 to 6, FN-BMD remained constant in Z6 and dropped slightly in Z3P3 (between-treatment difference = 1.04%; 95% confidence interval 0.4 to 1.7; p = 0.0009) but remained above pretreatment levels. Other BMD sites showed similar differences. Biochemical markers remained constant in Z6 but rose slightly in Z3P3, remaining well below pretreatment levels in both. New morphometric vertebral fractures were lower in the Z6 (n = 14) versus Z3P3 (n = 30) group (odds ratio = 0.51; p = 0.035), whereas other fractures were not different. Significantly more Z6 patients had a transient increase in serum creatinine >0.5 mg/dL (0.65% versus 2.94% in Z3P3). Nonsignificant increases in Z6 of atrial fibrillation serious adverse events (2.0% versus 1.1% in Z3P3; p = 0.26) and stroke (3.1% versus 1.5% in Z3P3; p = 0.06) were seen. Postdose symptoms were similar in both groups. Reports of hypertension were significantly lower in Z6 versus Z3P3 (7.8% versus 15.1%, p < 0.001). Abstract: Background Major depressive disorder (MDD) is a leading cause of disability worldwide. Aims To examine the: (a) 12-month prevalence of DSM-IV MDD; (b) proportion aware that they have a problem needing treatment and who want care; (c) proportion of the latter receiving treatment; and (d) proportion of such treatment meeting minimal standards. Method Representative community household surveys from 21 countries as part of the World Health Organization World Mental Health Surveys. Results Of 51 547 respondents, 4.6% met 12-month criteria for DSM-IV MDD and of these 56.7% reported needing treatment. Among those who recognised their need for treatment, most (71.1%) made at least one visit to a service provider. Among those who received treatment, only 41.0% received treatment that met minimal standards. This resulted in only 16.5% of all individuals with 12-month MDD receiving minimally adequate treatment. Conclusions Only a minority of participants with MDD received minimally adequate treatment: 1 in 5 people in high-income and 1 in 27 in low-/lower-middle-income countries. Scaling up care for MDD requires fundamental transformations in community education and outreach, supply of treatment and quality of services.

Abstract: Detecting actions in untrimmed videos is an important yet challenging task. In this paper, we present the structured segment network (SSN), a novel framework which models the temporal structure of each action instance via a structured temporal pyramid. On top of the pyramid, we further introduce a decomposed discriminative model comprising two classifiers, respectively for classifying actions and determining completeness. This allows the framework to effectively distinguish positive proposals from background or incomplete ones, thus leading to both accurate recognition and localization. These components are integrated into a unified network that can be efficiently trained in an end-to-end fashion. Additionally, a simple yet effective temporal action proposal scheme, dubbed temporal actionness grouping (TAG) is devised to generate high quality action proposals. On two challenging benchmarks, THUMOS14 and ActivityNet, our method remarkably outperforms previous state-of-the-art methods, demonstrating superior accuracy and strong adaptivity in handling actions with various temporal structures.
