Abstract: Background: Despite considerable multidisciplinary research on return to work (RTW), there has been only modest progress in implementation of study results, and little change in overall rates of work disability in developed countries. Methods: Thirty RTW researchers, representing over 20 institutions, assembled to review the current state of the art in RTW research, to identify promising areas for further development, and to provide direction for future investigations. Results and Conclusion: Six major themes were selected as priority areas: early risk prediction; psychosocial, behavioral and cognitive interventions; physical treatments; the challenge of implementing evidence in the workplace context; effective methods to engage multiple stakeholders; and identification of outcomes that are relevant to both RTW stakeholders and different phases of the RTW process. Understanding and preventing delayed RTW will require application of new concepts and study designs, better measures of determinants and outcomes, and more translational research. Greater stakeholder involvement and commitment, and methods to address the unique challenges of each situation are required.

Abstract: Background: Prenatal exposure to methylmercury (MeHg) and polychlorinated biphenyls (PCBs) has been associated with impaired performance on attention tasks in previous studies, but the extent to which these cognitive deficits translate into behavioral problems in the classroom and attention deficit/hyperactivity disorder (ADHD) remains unknown. By contrast, lead (Pb) exposure in childhood has been associated with ADHD and disruptive behaviors in several studies. Objectives: In this study we examined the relation of developmental exposure to MeHg, PCBs, and Pb to behavioral problems at school age in Inuit children exposed through their traditional diet. Methods: In a prospective longitudinal study conducted in the Canadian Arctic, exposure to contaminants was measured at birth and at school age. An assessment of child behavior (n = 279; mean age = 11.3 years) was obtained from the child’s classroom teacher on the Teacher Report Form (TRF) from the Child Behavior Checklist, and the Disruptive Behavior Disorders Rating Scale (DBD). Results: Cord blood mercury concentrations were associated with higher TRF symptom scores for attention problems and DBD scores consistent with ADHD. Current blood Pb concentrations were associated with higher TRF symptom scores for externalizing problems and with symptoms of ADHD (hyperactive-impulsive type) based on the DBD. Conclusions: To our knowledge, this study is the first to identify an association between prenatal MeHg and ADHD symptomatology in childhood and the first to replicate previously reported associations between low-level childhood Pb exposure and ADHD in a population exposed to Pb primarily from dietary sources.

Abstract: Fermi-liquid theory (the standard model of metals) has been challenged by the discovery of anomalous properties in an increasingly large number of metals. The anomalies often occur near a quantum critical point--a continuous phase transition in the limit of absolute zero, typically between magnetically ordered and paramagnetic phases. Although not understood in detail, unusual behaviour in the vicinity of such quantum critical points was anticipated nearly three decades ago by theories going beyond the standard model. Here we report electrical resistivity measurements of the 3d metal MnSi, indicating an unexpected breakdown of the Fermi-liquid model--not in a narrow crossover region close to a quantum critical point where it is normally expected to fail, but over a wide region of the phase diagram near a first-order magnetic transition. In this regime, corrections to the Fermi-liquid model are expected to be small. The range in pressure, temperature and applied magnetic field over which we observe an anomalous temperature dependence of the electrical resistivity in MnSi is not consistent with the crossover behaviour widely seen in quantum critical systems. This may suggest the emergence of a well defined but enigmatic quantum phase of matter.
