Abstract: Much of the research in epidemiology and clinical science is based upon longitudinal designs which involve repeated measurements of a variable of interest in each of a series of individuals. Such designs can be very powerful, both statistically and scientifically, because they enable one to study changes within individual subjects over time or under varied conditions. However, this power arises because the repeated measurements tend to be correlated with one another, and this must be taken into proper account at the time of analysis or misleading conclusions may result. Recent advances in statistical theory and in software development mean that studies based upon such designs can now be analysed more easily, in a valid yet flexible manner, using a variety of approaches which include the use of generalized estimating equations, and mixed models which incorporate random effects. This paper provides a particularly simple illustration of the use of these two approaches, taking as a practical example the analysis of a study which examined the response of portable peak expiratory flow meters to changes in true peak expiratory flow in 12 children with asthma. The paper takes the reader through the relevant practicalities of model fitting, interpretation and criticism and demonstrates that, in a simple case such as this, analyses based upon these model-based approaches produce reassuringly similar inferences to standard analyses based upon more conventional methods.

Abstract: Objectives To explore the effect of drivers’ use of mobile (cell) phones on road safety. Design A case-crossover study. Setting Perth, Western Australia. Participants 456 drivers aged ≥ 17 years who owned or used mobile phones and had been involved in road crashes necessitating hospital attendance between April 2002 and July 2004. Main outcome measure Driver’s use of mobile phone at estimated time of crash and on trips at the same time of day in the week before the crash. Interviews with drivers in hospital and phone company’s records of phone use. Results Driver’s use of a mobile phone up to 10 minutes before a crash was associated with a fourfold increased likelihood of crashing (odds ratio 4.1, 95% confidence interval 2.2 to 7.7, P < 0.001). Risk was raised irrespective of whether or not a hands-free device was used (hands-free: 3.8, 1.8 to 8.0, P < 0.001; hand held: 4.9, 1.6 to 15.5, P = 0.003). Increased risk was similar in men and women and in drivers aged ≥ 30 and < 30 years. A third (n = 21) of calls before crashes and on trips during the previous week were reportedly on hand held phones.

Abstract: Post-activation potentiation (PAP) is induced by a voluntary conditioning contraction (CC), performed typically at a maximal or near-maximal intensity, and has consistently been shown to increase both peak force and rate of force development during subsequent twitch contractions. The proposed mechanisms underlying PAP are associated with phosphorylation of myosin regulatory light chains, increased recruitment of higher order motor units, and a possible change in pennation angle. If PAP could be induced by a CC in humans, and utilized during a subsequent explosive activity (e.g. jump or sprint), it could potentially enhance mechanical power and thus performance and/or the training stimulus of that activity. However, the CC might also induce fatigue, and it is the balance between PAP and fatigue that will determine the net effect on performance of a subsequent explosive activity. The PAP-fatigue relationship is affected by several variables including CC volume and intensity, recovery period following the CC, type of CC, type of subsequent activity, and subject characteristics. These variables have not been standardized across past research, and as a result, evidence of the effects of CC on performance of subsequent explosive activities is equivocal. In order to better inform and direct future research on this topic, this article will highlight and discuss the key variables that may be responsible for the contrasting results observed in the current literature. Future research should aim to better understand the effect of different conditions on the interaction between PAP and fatigue, with an aim of establishing the specific application (if any) of PAP to sport.
