If BAO is recognised early and confirmed with multimodal CT or MRI, intravenous thrombolysis or endovascular treatment can be undertaken, however, the best treatment approach to improve clinical outcome still needs to be ascertained.
Abstract
The clinical presentation of basilar artery occlusion (BAO) ranges from mild transient symptoms to devastating strokes with high fatality and morbidity. Often, non-specific prodromal symptoms such as vertigo or headaches are indicative of BAO, and are followed by the hallmarks of BAO, including decreased consciousness, quadriparesis, pupillary and oculomotor abnormalities, dysarthria, and dysphagia. When clinical findings suggest an acute brainstem disorder, BAO has to be confirmed or ruled out as a matter of urgency. If BAO is recognised early and confirmed with multimodal CT or MRI, intravenous thrombolysis or endovascular treatment can be undertaken. The goal of thrombolysis is to restore blood flow in the occluded artery and salvage brain tissue; however, the best treatment approach to improve clinical outcome still needs to be ascertained.

The essential neurological examination of the unconscious patient in the emergency room.

TL;DR: To determine whether neurologists with long‐term experience in the emergency room are in general agreement about the essential components of the neurological examination used on unconscious patients in whom an obvious cause for coma is lacking, a survey of experts in the field is conducted.

Basilar artery occlusion after acute myocardial infarction

TL;DR: A 53-year old patient was admitted to the emergency department with acute chest pain, nausea and dyspnea, and the neurological assessment showed left sided facial palsy, Horner's syndrome and skewdeviation.

First-line contact aspiration versus first-line stent retriever for acute posterior circulation strokes: an updated meta-analysis.

TL;DR: In this paper, a meta-analysis was conducted according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis) statement, where random-effects models were performed to pool the outcomes and the value of I2 was calculated to assess the heterogeneity.

Mechanical thrombectomy in anterior vs. posterior circulation stroke: A systematic review and meta-analysis.

TL;DR: MT achieves similar rates of recanalization with a similar safety profile in PC-L VO and AC-LVO patients and patients with PC- LVO are less likely to achieve functional independence after MT.
