Abstract: Kwashiorkor, an enigmatic form of severe acute malnutrition, is the consequence of inadequate nutrient intake plus additional environmental insults. To investigate the role of the gut microbiome, we studied 317 Malawian twin pairs during the first 3 years of life. During this time, half of the twin pairs remained well nourished, whereas 43% became discordant, and 7% manifested concordance for acute malnutrition. Both children in twin pairs discordant for kwashiorkor were treated with a peanut-based, ready-to-use therapeutic food (RUTF). Time-series metagenomic studies revealed that RUTF produced a transient maturation of metabolic functions in kwashiorkor gut microbiomes that regressed when administration of RUTF was stopped. Previously frozen fecal communities from several discordant pairs were each transplanted into gnotobiotic mice. The combination of Malawian diet and kwashiorkor microbiome produced marked weight loss in recipient mice, accompanied by perturbations in amino acid, carbohydrate, and intermediary metabolism that were only transiently ameliorated with RUTF. These findings implicate the gut microbiome as a causal factor in kwashiorkor.

Abstract: Background Amphotericin B has long been the standard treatment for candidemia, but its use is complicated by its toxicity. More recently, fluconazole, a water-soluble triazole with activity against candida species and little toxicity, has become available. We conducted a multicenter randomized trial that compared amphotericin B with fluconazole as treatment for candidemia. Methods To be eligible, patients had to have a positive blood culture for candida species, a neutrophil count ≥ 500 per cubic millimeter, and no major immunodeficiency. Patients were randomly assigned to receive either amphotericin B (0.5 to 0.6 mg per kilogram of body weight per day) or fluconazole (400 mg per day), each continued for at least 14 days after the last positive blood culture. Outcomes were assessed by a group of investigators blinded to treatment assignment. Results Of the 237 patients enrolled, 206 met all entry criteria. Abstract: Although deep neural networks (DNNs) have achieved great success in many tasks, they can often be fooled by \emph{adversarial examples} that are generated by adding small but purposeful distortions to natural examples. Previous studies to defend against adversarial examples mostly focused on refining the DNN models, but have either shown limited success or required expensive computation. We propose a new strategy, \emph{feature squeezing}, that can be used to harden DNN models by detecting adversarial examples. Feature squeezing reduces the search space available to an adversary by coalescing samples that correspond to many different feature vectors in the original space into a single sample. By comparing a DNN model's prediction on the original input with that on squeezed inputs, feature squeezing detects adversarial examples with high accuracy and few false positives. This paper explores two feature squeezing methods: reducing the color bit depth of each pixel and spatial smoothing. These simple strategies are inexpensive and complementary to other defenses, and can be combined in a joint detection framework to achieve high detection rates against state-of-the-art attacks.
