Abstract: An ultrasound applicator for thermal therapy, having a multi-element array of plastic or rubber coated piezoceramic tubular transducers, each with separate power control and supported by a coaxially disposed semi-flexible central tube, which can be placed directly within the tumor or target region, or within a body lumen such as a blood vessel or body cavity such as the urethra, or within a separate delivery catheter or system. Thermocouple sensors embedded in the coating over each transducer can be used for continuous monitoring of the tissue/applicator interface temperatures for feedback control of the power applied to each transducer. The transducers are resonant across the wall thickness and radiate energy in the radial direction. The applicator provides an air-cooling system which can control the temperature of the transducers and thereby improve heating characteristics. In the primary configuration, the central tube is a catheter which is compatible with remote afterloaders and standard brachytherapy technology, and facilitates the delivery of interstitial thermal therapy in conjunction with simultaneous brachytherapy or radiotherapy from within the applicator. For alternate forms of therapy or when the radiation source is not in the catheter, the central catheter can be used to circulate temperature regulated air or water to control the surface temperature of the applicator, or to infuse drugs. Further, the applicator can be placed within a thin-walled implant catheter and flushed with temperature controlled circulating coupling fluid to control the temperature of the catheter/tissue interface, or used in conjunction with other conventional delivery systems, instead of being used interstitially.

Abstract: Disclosed are new methods comprising the use of in situ hybridization to detect abnormal nucleic acid sequence copy numbers in one or more genomes wherein repetitive sequences that bind to multiple loci in a reference chromosome spread are either substantially removed and/or their hybridization signals suppressed. The invention termed Comparative Genomic Hybridization (CGH) provides for methods of determining the relative number of copies of nucleic acid sequences in one or more subject genomes or portions thereof (for example, a tumor cell) as a function of the location of those sequences in a reference genome (for example, a normal human genome). The intensity(ies) of the signals from each labeled subject nucleic acid and/or the differences in the ratios between different signals from the labeled subject nucleic acid sequences are compared to determine the relative copy numbers of the nucleic acid sequences in the one or more subject genomes as a function of position along the reference chromosome spread. Amplifications, duplications and/or deletions in the subject genome(s) can be detected. Also provided is a method of determining the absolute copy numbers of substantially all RNA or DNA sequences in subject cell(s) or cell population(s).

Abstract: Against the background of recent methodological debates pitting ethnography against interviewing, this paper offers a defense of the latter and argues for methodological pluralism and pragmatism and against methodological tribalism. Drawing on our own work and on other sources, we discuss some of the strengths and weaknesses of interviewing. We argue that concern over whether attitudes correspond to behavior is an overly narrow and misguided question. Instead we offer that we should instead consider what interviewing and other data gathering techniques are best suited for. In our own work, we suggest, we have used somewhat unusual interviewing techniques to reveal how institutional systems and the construction of social categories, boundaries, and status hierarchies organize social experience. We also point to new methodological challenges, particularly concerning the incorporation of historical and institutional dimensions into interview-based studies. We finally describe fruitful directions for future research, which may result in methodological advances while bringing together the strengths of various data collection techniques.

Abstract: We describe a message authentication algorithm, UMAC, which can authenticate messages (in software, on contemporary machines) roughly an order of magnitude faster than current practice (e.g., HMAC-SHA1), and about twice as fast as times previously reported for the universal hash-function family MMH. To achieve such speeds, UMAC uses a new universal hash-function family, NH, and a design which allows effective exploitation of SIMD parallelism. The "cryptographic" work of UMAC is done using standard primitives of the user's choice, such as a block cipher or cryptographic hash function; no new heuristic primitives are developed here. Instead, the security of UMAC is rigorously proven, in the sense of giving exact and quantitatively strong results which demonstrate an inability to forge UMAC-authenticated messages assuming an inability to break the underlying cryptographic primitive. Unlike conventional, inherently serial MACs, UMAC is parallelizable, and will have ever-faster implementation speeds as machines offer up increasing amounts of parallelism. We envision UMAC as a practical algorithm for next-generation message authentication.

Abstract: Background Therapeutic targets have been defined for diseases like diabetes, hypertension or rheumatoid arthritis and adhering to them has improved outcomes. Such targets are just emerging for spondyloarthritis (SpA). Objective To define the treatment target for SpA including ankylosing spondylitis and psoriatic arthritis (PsA) and develop recommendations for achieving the target, including a treat-to-target management strategy. Methods Based on results of a systematic literature review and expert opinion, a task force of expert physicians and patients developed recommendations which were broadly discussed and voted upon in a Delphi-like process. Level of evidence, grade and strength of the recommendations were derived by respective means. The commonalities between axial SpA, peripheral SpA and PsA were discussed in detail. Results Although the literature review did not reveal trials comparing a treat-to-target approach with another or no strategy, it provided indirect evidence regarding an optimised approach to therapy that facilitated the development of recommendations. The group agreed on 5 overarching principles and 11 recommendations; 9 of these recommendations related commonly to the whole spectrum of SpA and PsA, and only 2 were designed separately for axial SpA, peripheral SpA and PsA. The main treatment target, which should be based on a shared decision with the patient, was defined as remission, with the alternative target of low disease activity. Follow-up examinations at regular intervals that depend on the patient9s status should safeguard the evolution of disease activity towards the targeted goal. Additional recommendations relate to extra-articular and extramusculoskeletal aspects and other important factors, such as comorbidity. While the level of evidence was generally quite low, the mean strength of recommendation was 9–10 (10: maximum agreement) for all recommendations. A research agenda was formulated. Conclusions The task force defined the treatment target as remission or, alternatively, low disease activity, being aware that the evidence base is not strong and needs to be expanded by future research. These recommendations can inform the various stakeholders about expert opinion that aims for reaching optimal outcomes of SpA.
