Abstract: The authors evaluated 4 sequential sampling models for 2-choice decisions--the Wiener diffusion, Ornstein-Uhlenbeck (OU) diffusion, accumulator, and Poisson counter models--by fitting them to the response time (RT) distributions and accuracy data from 3 experiments. Each of the models was augmented with assumptions of variability across trials in the rate of accumulation of evidence from stimuli, the values of response criteria, and the value of base RT across trials. Although there was substantial model mimicry, empirical conditions were identified under which the models make discriminably different predictions. The best accounts of the data were provided by the Wiener diffusion model, the OU model with small-to-moderate decay, and the accumulator model with long-tailed (exponential) distributions of criteria, although the last was unable to produce error RTs shorter than correct RTs. The relationship between these models and 3 recent, neurally inspired models was also examined.

Abstract: Overproduction of IL-6, a proinflammatory cytokine, is associated with a spectrum of age-related conditions including cardiovascular disease, osteoporosis, arthritis, type 2 diabetes, certain cancers, periodontal disease, frailty, and functional decline To describe the pattern of change in IL-6 over 6 years among older adults undergoing a chronic stressor, this longitudinal community study assessed the relationship between chronic stress and IL-6 production in 119 men and women who were caregiving for a spouse with dementia and 106 noncaregivers, with a mean age at study entry of 7058 (SD = 803) for the full sample On entry into this portion of the longitudinal study, 28 of the caregivers' spouses had already died, and an additional 50 of the 119 spouses died during the 6 years of this study Levels of IL-6 and health behaviors associated with IL-6 were measured across 6 years Caregivers' average rate of increase in IL-6 was about four times as large as that of noncaregivers Moreover, the mean annual changes in IL-6 among former caregivers did not differ from that of current caregivers even several years after the death of the impaired spouse There were no systematic group differences in chronic health problems, medications, or health-relevant behaviors that might have accounted for caregivers' steeper IL-6 slope These data provide evidence of a key mechanism through which chronic stressors may accelerate risk of a host of age-related diseases by prematurely aging the immune response

Abstract: Background and Methods. Superficial and systemic fungal infections are a major problem among severely immunocompromised patients who undergo bone marrow transplantation. We performed a doubleblind, randomized, multicenter trial in which patients receiving bone marrow transplants were randomly assigned to receive placebo or fluconazole (400 mg daily). Fluconazole or placebo was administered prophylactically from the start of the conditioning regimen until the neutrophil count returned to 1000 per microliter, toxicity was suspected, or a systemic fungal infection was suspected or proved. Results. By the end of the treatment period, 67.2 percent of the 177 patients assigned to placebo had a positive fungal culture of specimens from any site, as compared with 29.6 percent of the 179 patients assigned to fluconazole. Among these, superficial infections were diagnosed in 33.3 percent of the patients receiving placebo and in 8.4 percent of the patients receiving fluconazole (P<0.001). Systemic fungal in...
