Abstract: Background We investigated whether sirolimus-based immunosuppression improves outcomes in liver transplantation (LTx) candidates with hepatocellular carcinoma (HCC). Methods In a prospective-randomized open-label international trial, 525 LTx recipients with HCC initially receiving mammalian target of rapamycin inhibitor-free immunosuppression were randomized 4 to 6 weeks after transplantation into a group on mammalian target of rapamycin inhibitor-free immunosuppression (group A: 264 patients) or a group incorporating sirolimus (group B: 261). The primary endpoint was recurrence-free survival (RFS); intention-to-treat (ITT) analysis was conducted after 8 years. Overall survival (OS) was a secondary endpoint. Results Recurrence-free survival was 64.5% in group A and 70.2% in group B at study end, this difference was not significant (P = 0.28; hazard ratio [HR], 0.84; 95% confidence interval [95% CI], 0.62; 1.15). In a planned analysis of RFS rates at yearly intervals, group B showed better outcomes 3 years after transplantation (HR, 0.7; 95% CI, 0.48-1.00). Similarly, OS (P = 0.21; HR, 0.81; 95% CI, 0.58-1.13) was not statistically better in group B at study end, but yearly analyses showed improvement out to 5 years (HR, 0.7; 95% CI, 0.49-1.00). Interestingly, subgroup (Milan Criteria-based) analyses revealed that low-risk, rather than high-risk, patients benefited most from sirolimus; furthermore, younger recipients (age 60) also benefited, as well sirolimus monotherapy patients. Serious adverse event numbers were alike in groups A (860) and B (874). Conclusions Sirolimus in LTx recipients with HCC does not improve long-term RFS beyond 5 years. However, a RFS and OS benefit is evident in the first 3 to 5 years, especially in low-risk patients. This trial provides the first high-level evidence base for selecting immunosuppression in LTx recipients with HCC.

Abstract: Phosphatase is a key-enzyme in the marine environment, although life in the sea is normally not P limited. Expression of phosphatase in algae is generally regulated by the prevailing external concentration of inorganic phosphate, but the internal N:P ratio may also play a role. For bacteria, additional mechanisms like their C and N demands may be important. This is suggested by high phosphatase activities occasionally measured in eutrophic or deep water in the presence of relatively high phosphate concentrations. The distribution of phosphatase activity among the particulate and the dissolved fractions is highly variable. In particular, the dissolved fraction can contribute considerably to the total phosphatase activity (up to 70%), which differs from the pattern of other hydrolytic ectoenzymes. Parts of this fraction may originate from marine protozoa. The contribution of bacteria and phytoplankton to the particle-associated fraction of phosphatase is extremely variable, depending on P-availability, the dominant organisms, water depth and environmental factors. Community analysis revealed that bacteria attached to marine snow and N2-fixing cyanobacteria were frequently strong producers of phosphatase. Field studies carried out on a great variety of marine regions suggest that phosphatase activity is generally a good indicator of the P status of phytoplankton. Several heat-stable or heat-labile phosphatases, isolated from marine organisms living in extreme or other environments have been recommended for biotechnological applications.

Abstract: Infrared studies have revealed debris likely related to planet formation in orbit around ~30% of youthful, intermediate mass, main-sequence stars. We present evidence, based on atmospheric pollution by various elements heavier than helium, that a comparable fraction of the white dwarf descendants of such main-sequence stars are orbited by planetary systems. These systems have survived, at least in part, through all stages of stellar evolution that precede the white dwarf. During the time interval (~200 million years) that a typical polluted white dwarf in our sample has been cooling it has accreted from its planetary system the mass of one of the largest asteroids in our solar system (e.g., Vesta or Ceres). Usually, this accreted mass will be only a fraction of the total mass of rocky material that orbits these white dwarfs; for plausible planetary system configurations we estimate that this total mass is likely to be at least equal to that of the Sun's asteroid belt, and perhaps much larger. We report abundances of a suite of eight elements detected in the little studied star G241-6 that we find to be among the most heavily polluted of all moderately bright white dwarfs.

Abstract: The application of a dynamic voltage restorer (DVR) connected to a wind-turbine-driven doubly fed induction generator (DFIG) is investigated. The setup allows the wind turbine system an uninterruptible fault ride-through of voltage dips. The DVR can compensate the faulty line voltage, while the DFIG wind turbine can continue its nominal operation as demanded in actual grid codes. Simulation results for a 2 MW wind turbine and measurement results on a 22 kW laboratory setup are presented, especially for asymmetrical grid faults. They show the effectiveness of the DVR in comparison to the low-voltage ride-through of the DFIG using a crowbar that does not allow continuous reactive power production.
