Abstract: Introduction There is a high prevalence of congenital heart disease (CHD) in Down syndrome (DS) patients. Children with DS and CHD also present greater susceptibility to pulmonary infections than those without CHD. Aim To investigate the prevalence and types of CHD and their association with severe infections in children with DS in southern Brazil seen in a reference outpatient clinic. Methods Children aged between six and 48 months with a diagnosis of DS were included consecutively in the period May 2001 to May 2012, and the presence of CHD and severe infections (pneumonia and sepsis) was investigated, classified and analyzed. Results A total of 127 patients were included, of whom 89 (70.1%) had some type of CHD, 33 (37.7%) of them requiring surgical correction. Severe infections (pneumonia and sepsis) were seen in 23.6% and 5.5%, respectively. Of the cases of pneumonia, 70% had associated CHD (p=0.001) and of those with sepsis, 85% presented CHD (p=0.001). Conclusions Our study showed a high prevalence of CHD and its association with severe infections in children with DS seen in southern Brazil.

Abstract: This article argues that the issue of ethics in academic practice must receive the same attention dedicated to it in other professional practices. In fact, a difficulty faced in the debate concerning ethics in academic practice is the attitude of disregarding that the latter also consists in a social practice, conditioned by the same problems faced by contemporary society, especially in its economic dimension. Thus, this paper discusses Utilitarianism in academic practice, having Jurgen Habermas’s discursive ethics framework as a basis, and exploring some concerns about Brazilian academic productivism. Methodologically, the theoretical essay format is used as an argumentative procedure in order to build a less formal and more provocative text, instigating reflection and debate, within the scenario, on the indicated issues. Specifically, we notice the problem of quantitative appreciation of production without regard to the quality of academic work as an attempt to measure academic activity, as well as a lack of adequate reflection about the ethical and moral values encompassing the scientific and educational activity in Brazil. At last, to illustrate the perspective presented, we resort to hypothetical situations of academic practice, and make conjectures about four correlations between its pragmatic framework and ethical issues, taking into account the Habermasian construct of strategic action.

Abstract: CONTEXT AND OBJECTIVE: Increased life expectancy among people with Down syndrome (DS) has introduced new environmental factors that may affect blood pressure (BP) and/or lead to obesity in this population. The aim here was to investigate BP levels and body mass index (BMI) in adults with DS, correlating these data with the patients' sex and age. DESIGN AND SETTING: Analytical cross-sectional observational study conducted in special schools in Curitiba (PR), Brazil. METHODS: 97 adult patients were included. BP was measured in accordance with the established guidelines. BMI was calculated by dividing the weight by the height squared (kg/m2). RESULTS: Sex had no influence on BMI; nor did systolic BP (SBP) or diastolic BP (DBP). The age range was from 18 to 56 years. No correlation was observed between increasing age and greater BMI or BP. Eighty-six individuals (88.7%) presented normal BP, eleven (11.3%) prehypertension and none hypertension. Twenty patients (20.4%) presented BP lower than 90 × 60 mmHg. BMI ranged from 18 to 48 kg/m2 (mean of 28.8 ± 3.92 kg/m2): 21.9% had normal weight; 40.7% were overweight; and 25.3% had obesity class I, 9.9% class II and 2.2% class III. Higher BMI was associated with significantly greater SBP and DBP (P = 0.0175 and P = 0.0015). CONCLUSION: Sex and age did not influence SBP, DBP or BMI in Brazilian adults with DS. Higher BMI was associated with greater BP (both systolic and diastolic).
