TL;DR: Three mechanisms underlying local progression in the remnant pancreas are identified: residual microscopic disease at the resection margin, intraparenchymal spread of neoplastic cells, leading to an anatomically separate but genetically related recurrence, and multifocal disease with genetically distinct lesions.
